This Change Order to the Contract document shall supplement, amend, and shall become a part of, and be
incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change Order:
Phase III: South Side Improvements — july 31, T September 15, 2017
Phase IV: North Side Improvements — September 15 2017 OGt ti, r 40L, 2 October 26, 2017
Phase V: Fixtures, Trees, Fencing, and Landscaping — OGtGbGF 13, 2 October 31, 2017
The completion dates were extended because:
1) The contractor is adding another layer to the Pave Drain under bed (Add 4 days: 2 South Side, 2
North Side).
2) The pavement section at the intersection of Olive & Western on the south side was adjusted (0.5
days)
3) The contractor extended a larger water service than what was called for in the plans (add 1 day)
4) The contractor encountered fiber optic ducts in the way of the storm sewer and needed to raise
the fiber optic ducts and existing manhole. (add 1 day)
5) The contractor encountered fiber optic under the light foundations and required extra time to
install reinforcement around the conduit. (add 2 days)
6) The contractor performed additional work to address voids under the pavement (add 2.5 days)
7) Additional walk on South Side of Kaley Street (add 1 day)
The overall final completion date was changed to November 10, 2017,

SPECIFICATIONS:
The following special provisions have been amended or added to the Contract Documents and are issued
as part of this Change Order:
XIII. PROSECUTION AND PROGRESS
Revise Section 13.1:
1. The project will have a final completions date of QGtGber 25, 2November 10 2017 for all
work. The contract time will start when the Notice to Proceed is delivered and signed.

3. The project will have intermediate completion dates as listed in the tale below for substantial
completion:
Phase I & II: Kaley Street and Meade Street Sewer and Water Crossings — June 30, 2017

a. The project will have an intermediate substantial completion date of June 30, 2017 for
storm sewer to be constructed from the intersection of Western Avenue and Kaley Street to
its northern connection on Kaley Street and for water crossings at Meade Street and at
Kaley Street. A minimum of one travel lane and one parking lane along Western Avenue
shall be open to traffic during the lane closure.
b. The project will have an intermediate substantial completion date of 44�L ; 28� August
31, 2017 September 15, 2017 for construction of curb, sidewalk, roadway, lighting, pavers,
and all project elements complete and in -place on the southern side of the roadway
centerline except for pavement markings, benches, bike racks, landscaping, and trees. A
minimum of one travel lane and one parking lane along Western Avenue shall be open to
traffic during the lane closure period. Temporary parking lane closures will be permitted for
water service connections. Phase III of the project can occur simultaneously with Phase Il,
provided that the contractor maintains a minimum of one travel lane along Western Avenue
and submits a revision to the Maintenance of Traffic Plan that is approved by the City prior
to construction.
c. The project will have an intermediate substantial completion date of September 15, 2017
October 26, 2017 for construction of curb, sidewalk, roadway, lighting,
pavers, pavement markings, and all project elements complete and in -place on the north
side of the roadway centerline except for benches, bike racks, landscaping, and trees. A
minimum of one travel lane and one parking lane along Western Avenue shall be open to
traffic during the lane closure period,
d. The project will have an intermediate substantial completion date of
October 31, 2017 for installation of benches, bike racks, landscaping, fencing, and trees. A
minimum of two travel lanes on Western Avenue shall be open to traffic during the lane
closure period.
LX. PERMEABLE PAVERS
Revise Section B.
8. The block shall be laid on 1" of No. 11 limestone on INDOT No. 8 coarse aggregate as shown
in the plans. The aggregate will be paid for separately.
9. A support layer of Tensar BX1100, or approved equal, shall be laid within No. 8 stone at a level
8" to 10" below the bottom of the pavers. An additional layer of Tensar BX1100, or approved
equal, shall be between the No. 8 aggregate and No. 11 aggregate, approximately 1" below the

e. Water services 2" and larger shall be ductile iron, Class 50, 150 psi working pressure and in
accordance with AWWA C-151-91. Ductile iron pipe and valves shall be in accordance with
the project specifications. The water service shall include the ductile iron pipe, tapping
sleeve and valve, connection to existing service, and all mechanical joint fittings.

7. In the event that fiberoptic conduit crosses the proposed foundation location, the Contractor
shall modify reinforcement and formwork to allow for the conduit to pass through the foundation
and shall be paid for Lighting Foundation Extra at the unit prices as set forth in the proposal.
LXXIII. DRILLED SHAFT FOUNDATION TYPEI4A
Revise Section BA:
B. Additions
1. Drilled shaft foundation shall be installed as noted on the plans and in accordance with INDOT
Standard Drawing E805-TSCS-re-15.
LXXXX. FLOWABLE BACKFILL. NON -REMOVABLE
Add:
A. Prevailing Specifications:
2016, INDOT Standard Specifications Section 213
B. Additions:
Flowable backfrll shall be utilized where indicated on the plans. Flowable backfill will be
measured and paid for as FLOWABLE BACKFILL, NON -REMOVABLE (CYS).
LXXXXI. CENTER LINE EXPOSURE
Add:
A. Prevailing Specifications:
2016, iNDOT Standard Specifications Section 202 and 213
B. Additions:
1. Voids in the subgrade are suspected beneath the pavement from Kaley Street to Meade Street
over the length of the existing sanitary sewer.
2. Contractor shall saw cut the existing pavement to remove two feet of existing pavement over
the existing sewer. The existing asphalt and concrete pavement shall be removed from the
site and properly disposed of.
3. The existing subgrade shall be inspected to determine the location and extent of the potential
voids.
4. Contractor shall place Flowable fill in the exposed trench to fill the voids and the excavated
trench to a level necessary to obtain a minimum of 2" of HMA Wedge and Level for the bond
breaker course for the proposed grade.

5. The approximately 6" above the flowable fill shall be filled with covered or filled with temporary
asphalt, or other acceptable material, which will be removed during surface preparation for
placement of bond breaker beneath the proposed concrete overlay.
6. Contractor is responsible to maintain the approximately 6" depth temporary material at
intersections to maintain traffic.
7. The flowable fill shall be in accordance with specifications in this addendum.
8. The saw cut and excavation of concrete and asphalt pavements shall be measured by the
square yard and paid for as CENTER LINE EXPOSURE (SYS).
