This Change Order to the Contract document shall supplement, amend, and shall become a part of, and be
incorporated as an Addendum to the Contract.
The following Is a summary and description of the revisions to the Contract by this Change Order:
1) Division A has been removed from the project. This reduces the overall project by $32,744,
2) Environmental allowance has been removed from the project due to non -necessity. This reduces
the overall project by $10,000.
3) BuIIdIng Repair has been added to the project to address the exposure of the wall at 3610 W
Western Avenue as part of the removal of the wall at 3602-2308 W Wester Avenue. This has
increased the cost by $17,464 and extended the completion time by 40 days.
The overall final completion date has been Increased by 40 days.

SPECIFICATIONS:
The following special provisions have been amended or added to the Contract Documents and are issued
as part of this Change Order:
XIV. PROSECUTION AND PROGRESS
A. Prevailing Specifications: 2016, INDOT Standard Specifications Sec. 108
B. Additions:
1. The project will have a completion date of sixty (60 100) calendar clays for all work. The
contract time will start when the Notice to Proceed is delivered and signed.
2. The Contractor shall have twenty-four (24) calendar days to complete demolition work on
each site once work has begun on that site. The Contractor shall have twenty-four (14)
calendar days to complete building repair work on each site once building repair work has
begun on that site.
XLIII. BUILDING REPAIR
A. Prevailing Specifications: None
B. Additions:
1. Work to be performed shall include furnishing all labor, services, materials, Insurance and
equipment to install vinyl siding and repair features associated with the east face wall of
3610 Western Avenue. Such work shall include:
a. Insulating the existing exposed wall cavity with batt insulation.
b. Furnishing and installing new plywood sheathing to the exterior of the wall.

c. Wrapping newly sheathed wall with Tyvek (or equal) wrap and tape all joints.
d. Furnishing and installing vinyl siding that matches the existing on the south side or
meets the property owner's approval.
e. Furnishing and installing new gutter and downspout on the east wall.
f. Repairing roof edge on east wall.
g. Furnishing and installing metal corner pieces at northeast corner of the building to
encapsulate the cut brick and block.
2. Payment for BUILDING REPAIR shall be the Lump Sum Unit Price for BUILDING
REPAIR as set forth in the bid/proposal.
