This is Attachment 1, consisting of pages, to
Amendment No. 1, dated , 2017.
Modifications
1. Engineer shall perform the following Additional Services:
SCOPE OF SERVICES, section I is modified to add the following paragraphs:
G. Architectural Services for Colfax Parking Garage Entry Modifications
1. One site visit to verify conditions
2. Plans and Specification based on one option
a) Prepare floor plans, signage, and specifications for the purpose of open
bidding of the project, including recommended locations for the electrical
conduit
b) Prepare plans and specifications for replacement of trench drain located at
garage entry/exit
c) Prepare cost estimate based on Construction Documents
d) Submit bid documents to the Owner for review
e) Incorporate comments into the bid documents
f) Engineer will submit to the State of Indiana DHS for Design Release
g) Review by agency (State of Indiana DHS for Design Release)
3. Bidding and Construction
a) Respond to RFI's
b) Review up to two shop submittals as required in the Architectural work
specifications
c) Make one site visit for final punch list
H. Transportation Services
1. Develop up to three alternatives for reconfiguring the garage entry/exit
2. Perform traffic analysis on each alternative to determine issues and queues
3. Prepare turning movement diagrams to accommodate turning vehicles
4. Provide a summary memorandum documenting the alternatives and identifying a
preferred alternative
SCOPE OF SERVICES is modified to add the following sections:

III. Bidding and Construction
A. Review up to two shop submittals as required in the structural work specifications.
Review of submittals shall be for general conformance with the information given and
design concept expressed in the contract documents.
B. Respond to Requests for Information to clarify any design questions that arise during
construction.
C. Review reports from the testing and inspection agencies to determine if the agency
has verified conformance of the reported item of work with contract documents.
D. Initiate appropriate action in response to testing and inspection reports, if required.
E. Make up to two site visits at appropriate stages of construction.
F. Engineer will observe and become generally familiar with the quality and progress of
construction work relative to elements designed and specified by American
Structurepoint.
G. These site visits are for structural field observation only and do not constitute
inspection of the work. Inspection of the work will be performed by qualified
individuals or agencies as indicated in the contract documents.
H. Engineer shall have no responsibility for oversight or supervision of the contractors
or their employees, for the means and methods of construction, or for the safety of
persons on or off the job site. We shall have no responsibility to inspect for, or
remove, hazardous materials.
IV. Concept Review Meeting
A. One discipline meeting with the Owner to discuss concepts and confirm preferred
alternative for final design and Construction Documents
V. Additional Services
A. Design or construction phase services requested by the Owner above and beyond
those items listed in this Amendment 1
4. For the Additional Services or the modifications to services set forth above, Owner shall pay
Engineer the following additional or modified compensation:
