Herewith is a summary and explanation for the Change Order being submitted:
A pay item for Temporary Approach has been added to the project in order to maintain access to
properties between the time when approaches are removed and new approaches are constructed.
The overall final completion date was not changed.
There was a net increase of $5,000 to overall contract amount.

This Change Order to the Contract document shall supplement, amend, and shall become a part of, and be
incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change Order:

SPECIFICATIONS:
The following special provisions have been amended or added to the Contract Documents and are issued
as part of this Change Order:
xxxvlll. TEMPORARY APPROACH
A. Prevailing Specifications: 2016, INDOT Standard Specifications Section 205
B. Additions
1. The Contractor shall construct a temporary approach immediately after removal of an
approach that is to be replaced and maintain such approach until a new approach has been
constructed. No property with an approach shall be without a temporary or new approach
between 5 pm and 8 am.
2. A temporary approach shall match INDOT's definition of a temporary stable construction
entrance. It is a sediment control measure consisting of a stabilized aggregate pad over
geotextile. See INDOT Standard Drawing E 205-TECP-01 for details.
3. Payment shall be made at the Unit Price specified in the Proposal as follows:
