By: Title: Gregor lnski, Road Group Leader Date: j Page 1 of 1 201401351 This is Attachment 1, consisting of 2 pages, to Amendment No. 1 Modifications Engineer shall perform the following Additional Services: Exhibit A, Part I.A is revised by adding the following: Design and plan development for the addition of an eastbound to northbound left -turn lane at the intersection of Sample Street and Main Street. Design services shall include additional topographic survey (Exhibit B, attached hereto and incorporated herein), road design, utility coordination, traffic signal modification, project meetings with City staff and INDOT, right of way engineering for two parcels, bidding phase services, and construction phase services. Right-of-way engineering will include title work, title work updates, plats and legal descriptions, staking and appraisal problem analysis (APAs). Review submittals will include preliminary field check plans (40% complete), final plans (95% complete), and Final Tracings (100% complete). Prepare a separate submittal for independent bid package of said area to be bid by Owner (Exhibit E, attached hereto and incorporated herein). 6. Prepare a separate submittal for an independent quote package from the northwest corner of Main Street and Washington Street, in front of the Chase Building, for work between back of curb and face of building, including plan sheet(s), quantities, and specifications relating to construction of said area. 7. Adjust signal timings along Martin Luther King, Main Street, and Lafayette Street between Sample Street and Bartlett Street as outlined in Exhibit C, attached hereto and incorporated herein. The retiming of signals shall include the following: signal system inventory assessment, analysis and timing development, and deployment. Exhibit A, Part 1.13 is added to read as follows: B. Services Not Included in Engineer's Scope: 1. Hydraulics analysis and design of storm sewer 2. Geotechnical investigation Subsurface utility engineering 4. Lighting design and plan development 5. Sanitary and water line design and plan development 6. Land Acquisition Services 2. For the Additional Services or the modifications to services set forth above, Owner shall pay Engineer the following additional or modified compensation: Section 7.0 ] A. For services other than right-of-way engineering, an amount equal to the cumulative hours charged to the Project by each class of Engineer's employees times standard hourly rates for each applicable billing class for all services performed on the Project, plus reimbursable expenses and Engineer's consultants' charges, if any, not to exceed 9 8k1 $157,1_00. Section 7.01A.2 is added to read as follows: 2. The ENGINEER shall be compensated for right -of way engineering services based on the specific cost per unit multiplied by the actual units of work. Estimated unit costs are shown in Exhibit D, which is attached Hereto and incorporated herein. The final amount will be adjusted according to the actual subconsultant invoices; however, the final amount shall not exceed $6,900 unless and until a supplemental agreement is executed. RE: Eastbound Sample Street Left Turn South Bend, Indiana Topographic Survey and Right of Way Analysis Dear Mr. Zielinski: Lawson -Fisher Associates P.C, (LFA) is presenting a sub -contractor scope of work, manhour justification and scope limit exhibit based upon your request for proposal dated May 24, 2017 and subsequent discussions regarding survey scope. These survey services will support American Structurepoint, Inc. (Structurepoint) as they respond to the City of South Bend, Indiana (CITY) request for professional design services pertaining to a proposed left turn lane at Sample Street and Main Street for eastbound traffic on Sample Street. Scope of Services A. LFA shall provide: 1. One time necessary topographic survey and utility representation, for roadway design purposes, within the areas indicated on the attached survey scope map. 2. A recorded Location Control Route Surrey along Sample Street from Lafayette Boulevard to Main Street described below: All of the work above shall be in accordance with Indiana Code (I,C. 25- 21.5); Indiana Administrative Code (865 I.A.C. 1-12), and the Design Manual, Indiana Department of Transportation, Part III, Location Surveys (Survey Manual). Provided survey data will be supplemental to other completed work and in the same coordinate system. 2. Electronic copy of all field notes. 3. Survey summary language to accompany drawing. 4. Location Control Route Survey Plat in .pdf format. C. This proposal does not include: 1. Updated INDOT-style fieldbook. 2. Additional control monumentation. 3. Revised, overall base drawing of previously surveyed data. 4. Collection of information regarding underground storage tanks. Owner -Provided Data The following items will be provided by Structurepoint through the CITY or otherwise considered an additional service not included with the described professional services. 1. Structurepoint through CITY shall provide any construction plans of any past improvements in their records. 2. it is understood that LFA shall have access to all CITY plans andlor records in its possession and at its disposal pertaining to this work. Proposed Schedule LFA proposes to submit to Structurepoint a supplemental survey drawing and associated deliverables within: 1. 30 calendar days of notice to proceed, for topographic survey deliverables. 2. 60 calendar days of notice to proceed, for Location Control Route Survey Plat. Proposed Fee The LFA fee for the scope of services detailed above is submitted as lump sum as follows: Topographic Survey $1,700.00 Location Control Route Survey Plat $7,000.00 Please indicate your scope selection by initialing next to the requested services.
