AMENDMENT TO OWNER -ENGINEER AGREEMENT Amendment No. 1 1. Background Data: a. Effective Date of Owner -Engineer Agreement: July 12, 2016 b. Owner: Board of Public Works, City of South Bend, Indiana c. Engineer: Lawson -Fisher Associates P.C. d. Project: South Bend Darn Underwater Inspection and Bathymetric Survey City Project No. 116-071 2. Description of Modifications: a. Engineer shall perform or furnish the following Additional Services: In addition to the contracted Scope of Work for the South Bend Dam Underwater Inspection and Bathymetrie Survey, LFA will prepare the Dam Safety Surveillance and Monitoring Report (DSSMR) to be submitted to the FERC. The City will provide LFA with all necessary completed tables that have been developed since the last submitted DSSMR (April 1, 2011), This DSSMR will not include the underwater inspection which will be performed once flows permit and will be submitted separately. b. For the Additional Services or the modifications to services set forth above, Owner shall pay Engineer the following additional or modified compensation: $3,000.00 C. The schedule for rendering services is modified as follows: • Completion of DSSMR by July 10, 2017. • Completion of undewater inspection (River Flow Permitting): September 22, 2017. 3.
