SUBJECT: Guardrail Removal and Impact Attenuators at CSX RR Bridge along Main Street
ISSUE DATE: April21, 2017
This Change Order to the Contract document shall supplement, amend, and shall become a
part of, and be incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change
Order:
1. Remove Guardrail in Median and Exterior Lane Along Main Street between
Bronson and South Streets at CSX Railroad
2. Install stand-alone Impact Attenuator on each end of median pier to protect the
CSX RR Bridge Pier
The following sections of the Contract Documents are revised as a result of the above changes:

This Change Order to the Contract document shall supplement, amend, and shall become a
part of, and be incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change
Order:
1. Revise pavement markings to add parking spaces north of the intersection of
Michigan Street and Sample Street
2. Add lane indication arrows west of the intersection of Western Avenue and St.
Joseph Street.
3. Add pavement markings along Colfax Avenue west of Lafayette Blvd.
4. Realign markings for thru lane and bike lane on Colfax Avenue east of St. Joseph
Street.
