Gibson -Lewis, L.L.C. is pleased to offer the following proposal for
your review.
Scope of Work:
1. Remove existing flooring and ceiling in large storage room in preparation for creating
(2) separate spaces.
2. Construct a new wall to create two separate rooms.
3. Furnish and install (1) new door, frame, and hardware to match existing.
4. Remove the existing water heater and save for relocation.
5. Disconnect and remove all plumbing piping back to a point suitable for reconnection.
6. Remove the existing return grille and cap the ductwork above the ceiling.
7. Remove the supply grille and water heater B-vent to a point suitable for
reconnection.
8. Perform the necessary layout for the required floor cutting in the space.
9. Furnish and install a complete Schedule 40 PVC sanitary waste and vent system for
the new restroom and utility room and connect to the existing piping.
10. We cannot tell what the existing underground is in this room that is feeding the floor
drain. If we remove the floor drain and find a 3" line, we will install a normal pressure
assist tank toilet. If we remove the floor drain and find a 2" line, we will install a
pumped toilet system.
11. The new vent piping for the fixtures will connect to the existing vent piping above the
existing men's locker room.
12. The new sanitary will pick up the new water closet and lavatory in the restroom as
well as a new floor drain in the utility room.
13. Furnish and install a complete Type "L" copper domestic water system for the new
restroom and connect to the existing piping.
14. Furnish and install a complete Schedule 40 carbon steel natural gas system for the
relocated water heater and connect to the existing piping in the space.
15. Furnish and install the following plumbing fixtures and equipment:
• 1— Water Closet: Kohler pressure assist tank toilet or Zoeller pump toilet
system with ADA height bowl, seat and all applicable trim.
• 1— Lavatory: Kohler wall hung lavatory with Kohler chrome single handle
faucet, ADA trap guard, point -of -use mixing valve (code required) and all
applicable trim.
1— Water Heater (Relocated): Relocate the existing water heater, reconnect
to required piping and provide relief valve discharge to floor drain.

s 1— Floor Drain: J. R. Smith cast iron floor drain with 5" round brass top (in
utility room).
16, Furnish and install all applicable B-vent flue pipe for the relocated water and reconnect
to the existing flue.
17. Furnish and install one (1) ceiling mounted exhaust fan and connect to the existing
exhaust duct system in the men's locker room. Fan is 120V and is to be interlocked with
the lighting in the space by others.
18. Furnish and install two (2) supply air diffusers (1 for the restroom and 1 for the utility
room) and reconnect to the existing supply duct system.
19. Supply labor and materials to relocate Data box high on wall to accommodate new entry
wall and door opening. (Please note this will only move as far as the existing cables will
allow)
20. Supply labor and materials to relocate (1) 2x 4 fluorescent fixtures to center of storage
room.
21. Supply labor and materials to rework the lighting circuit to accommodate the new
fixture location.
22. Supply labor and materials to install (1) occupancy sensor light switch.
23. Supply labor and materials to install (1) double duplex convenience receptacle.
24. Supply labor and materials to relocate (1) 2x4 fluorescent fixtures to center of rest
room.
25. Supply labor and materials to rework the existing lighting circuit to accommodate the
new location.
26. Supply labor and materials to install (1) vanity light over mirror to operate off same
switch as fluorescent light fixture.
27. Supply labor and materials to install (1) occupancy sensor switch.
28. Supply labor and materials to install (1) GFI receptacle near sink utilizing local circuitry.
29. Supply labor and materials to install power to (1) exhaust fan. Supplied and installed by
EJ White utilizing the existing lighting circuit and controls.
30. Supply labor and materials to install (1) dedicated receptacle for toilet as required.
31. Supply labor and materials to install (1) data cable in wall that is outside of wall at the
present time.
32. Furnish and install new 2' x 2' ceiling tile. We have figured USG Radar 2220 tile in our
proposal.
33. Install wood blocking for toilet accessories.
34. Patch holes in walls after plumbing and electrical work is completed.
35. Furnish and install new VCT and vinyl base in both rooms.
36. Install ADA compliant vanity, grab bars, and mirror.
37. Paint walls and door frames (new wood door to come prefinished).
38. All cleanup associated with our work.

Qualifications:
1. Normal working hours are figured.
2. Sales tax is excluded.
3. Bonds are excluded.
4. All shelving and equipment currently in the storage room to be removed by others.
