We would like to thank you for giving us this opportunity to submit this proposal to you.
ADA Bathroom Installation:
1. New bathroom to be as layout shown on provided drawings.
2. Remove existing VCT flooring. Saw cut floor as needed for waste lines. Saw cutting to
be done on a Saturday. Covering of all office fixtures to be done by Wastewater Treatment
Plant staff.
3. Remove existing water heater. Install new 10 gallon electric water heater in utility room.
4. Install new supply lines and waste lines as needed to complete bathroom and tie into
existing lines from removed water heater.
5. Remove and repair portions of suspended ceiling to allow for placement of new wall.
Frame in door leading to utility room. This door to match existing doors as close as possible.
6. Install 518" mold resistant drywall taped and finished. Repair walls in new bathroom.
Prime and paint new drywall and repaired areas. Apply 2 top coats. Color numbers to
be supplied by others.
7. Install new VCT flooring and vinyl base.
8. Install new ADA stool, wall hung lav, ADA mirror, soap dispenser, paper towel dispenser,
and ADA grab bars.
9. Clean up all debris from site.
10. Secure all necessary permits.

Scope of Work: Contractor shall furnish all labor and material necessary to comply with this
contract, including the attached specifications and the designated plans, if required. The
foregoing documents, together with any other documents specifically identified in the paragraphs
below shall be referred to collectively as the contract documents, and shall constitute the entire
agreement between the parties.
Assurance of Financing: Buyer represents and warrants to contractor that it now has or will
have within thirty (30) days from the date of this contract cash on hand, or a firm commitment
for a mortgage loan, in an amount sufficient to pay the entire purchase price and all of buyer's
other obligations under this contract, and that it will furnish written evidence thereof to the
contractor. In the absence of such written evidence, contractor shall have a discretion to delay
construction or cancel this contract and refund the dawn payment. Buyer hereby authorizes it's
mortgage lender to make payments directly to contractor.
Payment of Purchase Price: Buyer agrees to pay contractor the balance of the purchase price
and all other monies due under this contract under substantial completion of the work. The
making of final payment by buyer shall constitute a waiver of all claims by buyer against
contractor, except only those arising from unsettled liens, or agreed punch list items.
Changes: Changes in the contract documents are to be made only by written change orders,
signed by buyer and approved by contractor. The cost of any changes shall be added to or
subtracted from the purchase price as appropriate.
Allowances: Dollar amounts designated as allowances are understood to be the contractor's
estimate of the cost of the items indicated. If the contractor's actual cost exceeds the
allowance, the amount of the excess shall be added to the purchase price. If the contractor's
actual cost is less than the allowance, the difference shall be deducted from the purchase price.
Exclusive Warranty: Contractor warrants all materials and workmanship for a period of (1) year
from the date of closing. Contractor's obligation shall be limited to repair or replacement, in
contractor's discretion, of the defect or complaint. This is the sole and exclusive warranty given
by the contractor.
Permits: Contractor will furnish any applicable building permits and health permits.
Insurance: Contractor will provide Builder's Liability Insurance, Builder's Risk Insurance, and
Workman's Compensation Insurance for it's employees, and any other insurance required by
law. A Certificate of Insurance is available upon request.
Referral List: A referral list is available upon request.

Twenty -Four Thousand Six and 50/100.
Payments to be made based on percentages of completion with final balance due upon
completion of project.
ACCEPTANCE OF PROPOSAL: The above prices, specifications, and conditions are
satisfactory and are hereby accepted. You are authorized to do the work as specified.
Acceptance of this proposal makes this a legal and binding contract.
