Sponsor shall reimburse the Board for the actual cost to the City for the event, if deemed necessary. 2. Sponsor shall provide to the Board a Certificate of Insurance showing a liability policy in full force and effect with limits of $300,000.00 per occurrence and $5,000,000.00 aggregate and the City of South Bend listed as an additional named insured for this event. 3. Sponsor shall provide to the Board all additional licenses, permits and documentation required for the event, 4. Sponsor agrees to abide by all terms and conditions of the Board's policy governing walks, runs, parades or other similar event adopted by the Board on March 3, 1985. 5. In order to ensure public safety during the event, the Board agrees to furnish traffic planning, materials, equipment and personnel as deemed necessary by the Police Department Traffic Bureau, the Bureau of Traffic and Lighting, and, where applicable, the Board of Park Commissioners. 6. Sponsor acknowledges that the Police Department reserves the right to change this route for safety purposes. 7. In consideration for approval by the Board and the use of the sidewalks for the purposes set out above, the undersigned agrees and undertakes to hold the Civil City of South Bend, Indiana, free and harmless from any liability loss, costs, costs, damages or expenses, including attorney fees, which the Civil City of South Bend may suffer or incur, as a result of any claims or actions which may be made by any person, including a participant in said activity, arising out of the approval of the request to use the sidewalks indicated in the City of South Bend. The undersigned certifies that he/she is authorized to bind the above mentioned sponsor to the terms hereof. 8. Notification of approval/denial of this request will be issued by return of this form, upon signer/ authorization by the Board of Public Works I understand the above rules and regulations and that this application may be denied based on any false or incomplete information. The American Legion riders Chapter 357 in South Bend is requesting your help to put on a benefit for the flag awareness program. The ride is scheduled for Saturday, May 27, 2017. The Stars and Stripes Program visits local schools, makes a presentation about the history of the American Flag and educates young minds about the significance and symbolism from the eyes of a veteran. With the participation of the students, the old torn and faded American flags are replaced with new ones. Your support, with a donation of $100.00 will get your name or business name on the back of this year's t-shirts. are also looking for monetary service, or merchandise to be used as prizes. Sponsors will receive t-shirts if there are any available after the ride. Participating riders will be given a t-shirt illustrating the ride on front and ride sponsors on the back.'T-shirts while supplies last. The ride starts at Wal-Mart on Ireland road, South Bend, IN and will end at the American Legion Post 357 where there will be food, refreshments, and entertainment for all. The cutoff date is May 1 st, 2017 so the shirts can be made and ready for the ride. There is only room for 40 sponsors so the first 40 received will be on the shirts. Please include the way you want your name or company name appear on the shirts. THE CAUSE OR BENEFIT FOR THE EVENT: The American Legion Riders Post 357 in South Bend will be conducting a ride to support the Stars and Stripes Flag program. The Stars and Stripes Flag program is a group of veterans from Post 357 that visits local schools and make a presentation about the history of the American Flag and teach young minds about the significance and symbolism from the eyes of the Veteran. The old and tattered flags that fly about the schools are replaced with new, ones with the students participation involved. The American Legion Riders of Post 357 provide support and assistance to many local Veteran Groups.. THE EVENTAGENDA: The Stars & Stripes Forever Ride is approximately 60 mile escorted ride for motorcyclists starting at Wal-mart on 700 South Ireland Rd. in South Bend.
