This Change Order to the Contract document shall supplement, amend, and shall become a
part of, and be incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change
Order:

1. CO#2 Supplement
Change Order #2 was approved at $94,500.00. This change order, however, was
intended to be replaced before approval with an amount of $101,500.00. The adjusted amount
never made it to the BOPW. The $101,500.00 was carried throughout the remainder of the
project as if it had been approved. All subsequent change orders and closeout documents
assumed this dollar amount had been approved as Change Order #2.
