On April 11, 2017 the Board of Public Works opened and read quotes for the above project.
All quotes were found to be correct.
The quotes ranked as follows:

Errors were found in two separate quotes:
1. TX Concrete filled in the total cost of each line item but did not include unit prices. Jose
Quijada, from TX Concrete, verbally confirmed unit prices over the phone that were
determined by back -calculating from the total costs and the line item quantities. The
quantity for concrete curb removal does not divide evenly into the total cost of the line
item. Therefore, a unit price was determined that increases the total cost by $0.13. This
increase does not impact the results of the quote.
2. Willis Construction made a multiplication error that, when corrected, results in an
increase of $69.00 to the originally reported final amount. This was communicated to
Willis Construction via phone. The quote tabulation reflects the corrected final amount.
This increase does not impact the results of the quote.
recommend award of the contract for Project 117-013 Fire Dept. Training Center Site
Improvements, to the lowest responsive/responsible bidder, TX Concrete in the amount of
8 545.13.
The funds have been appropriated through the Repairs and Maintenance account, funding
number 101-0901-422.36-01.

PUBLIC WORKS CONTRACT
THIS AGREEMENT, made and entered into this 1 Ith Day of April, 2017, by and between., TX
Concrete, 24899 Fillmore Road, South Bend, IN 46619, HEREINAFTER called the Contractor,
and the Board of Public Works herein called the "Board".
WITNESSETH: That the Contractor covenants and agrees to make the following improvement,
as fully set out in the Request for Quotations, Standard Specifications, Special Provisions, Plans,
and Bid Proposals, all of which are set forth as a part of this Contract, for:
PROJECT NO. 117-013
DESCRIPTION: FIRE DEPARTMENT TRAINING CENTER SITE
IMPROVEMENTS
COMPLETION DATE: SEE SPECIFICATIONS
AMOUNT: $8,545.13
FUNDING: REPAIR AND MAINTENANCE — SERVICES/BUILDINGS
The unit prices for this improvement were those prices as received and accepted by the Board on
the Day of , 2016.
The Contractor further agrees to notify the Engineer when this improvement is completed. This
notification shall be in the form of a Completion Affidavit, signed by the Contractor. Upon final
acceptance of the improvement by the Engineer, the Contractors final estimate will be presented
to the Board for final Payment with one (1) copy of the Completion Affidavit and one (1) copy of
a Waiver of Lien.
