Description of Modifications:
DLZ Indiana, LLC was selected by the City of South Bend (Owner) to perform a
topographic survey of the north half of Sample Street 200 feet east and west of Sheridan
Street, for a total of approximately 400 feet, and on Sheridan Street north of Sample
Street for approximately 50 feet. The data was combined with previously surveyed data

collected as part of Nello Site Project performed for Panzica Building Corporation, to
provide one continuous surface model.
During review of the information by the Owner, it has been requested that additional
elevations for previously surveyed data. This will account for the elevations of the new
pavement, curb, and ramp which will be needed to tie into with the Sheridan
improvements, see Exhibit H. Verification will also be provided for the fence located
on the west side of Sheridan Street to determine if it is still within the right of way.
