DORAU Amendment No. 1 ��T3�r• .�fiRilZQ1jbi This Amendment, made and entered into this 28'h day of February, 2017, by and between the Board of Public Works of City of South Bend, Indiana, acting by and through its proper officials (hereinafter referred to as LPA) and Abonmarche Consultants Inc, (hereinafter referred to as CONSULTANT). WHEREAS, on April 26, 2016, LPA entered into a contract with the CONSULTANT for City of South Bend Safe Routes to School near Monroe Primary Center and Studebaker Elementary School in South Bend, Indiana. WHEREAS the LPA has requested modifications to the locations of sidewalk improvements; and WHEREAS, the LPA has concurred with CONSULTANT's determination of additional work and has requested the additional services to be performed by the Consultant. NOW THEREFORE, it has been determined by LPA and the CONSULTANTthat the contract shall be amended as follows: Amend Agreement Change: One Hundred, Fifty -Eight Thousand, Five Hundred Dollars and no cents ($158,500.00) to One Hundred, Eighty Thousand, Three Hundred Dollars and no cents ($180,300.00). Amend Appendix "A" A. Topographic Survey Add: aa. Southeast & southwest corners of High Street and Donmoyer Street intersection. bb. Intersection of Woodside Street and Carroll Street intersection. CC. South and east side of Dayton Street and Marietta Street intersection. dd. South side of Woodside Street from 234 Woodside Street to Woodside Street and St. Joe. ee. Intersection of Woodside Street and High Street. ff. North side of Donmoyer Street from west property line of 317 Donmoyer Street to intersection of Donmoyer Street and Fellows Street. gg. Intersection of Woodside Street and Fellows Street. hh. North side of Woodside from Fellows Street to Carroll Street. ii. North side of Woodside Street from 211 Woodside to 233 Woodside. E. Geotechnical Investigation Add: 2. Perform up to eleven (11) exploratory test borings to a depth of 5 feet each. The actual location, depth and number of borings will be dependent on the soil conditions encountered. SPT sampling will be performed at 2.5 foot intervals. 3. Perform laboratory tests consistent with INDOT requirements including visual soil classification, hand penetrometer readings, pH, suflate content, moisture content, grain size analysis and Atterberg limit determinations. 4. Prepare a technical report which will include a summary of findings and recommendations. F. Project Design Add: aa. Southeast & southwest corners of High Street and Donmoyer Street intersection;. bb. Amend Appendix "D" Change To: The Total Lump Sum Amount of $158,500.00 to $180,300. Change: a. Topographic Survey $44,200-to $49,200 d. Sidewalk Design and Plans $77 709 to $91,500. All other terms of the agreement remain in effect. In Witness Whereof, LPA and CONSULTANT have, through duly authorized representatives, entered into this Amendment.
