The repairs on this home DO NOT fall under the HTJD Lead-Based paint regulations because repairs
are not being performed in a child occupied facility and the home is strictly occupied by an elderly
person, per 24 CFR 35.900 subpart J. Because the surfaces to be disturbed are presumed to be Lead
Based Paint, safe work practices will be requested when windows are being replaced. Only worker
trained in safe work practices or workers under the direct supervision of a Lead Trained Supervisor
will be allowed to perform the needed repairs on this home.

l 3 750. ~-Q Remove sashes and needed sash stops on wood window units repair needed frames, sill,
andi or casings to ready new units. Replacements of sashes are to be performed from the exterior of home
if possible while having the interior sealed from possible dust infiltration. Install new thermo-pane
windows units with full screens and divided light grills as replacements for existing units .Prepare window
openings for installation; fill voids with fiberglass insulation, and seal to prevent drafts. Do not disturb
existing interior trim, apply an interior stop if needed, and wrap exterior frames on all replacement
windows with .019 ga. white aluminum. All aluminum wrap installations shall be free of waves having all
edges and seams sealed with a matching minimum 20 year silicon caulking. Interior surfaces of frame and
casings are to be prepped for finishing and then caulked and painted, if needed, to match existing.

air conditioning unit appropriately sized for homes heating requirements. Installation to include.
needed new wiring, Energy Star Rated setback thermostat or approved equal, duct work,
condensing unit, coil, refrigerant lines, condensation evacuation pump, PVC exhaust duct and
exterior combustion air intake, and carbon monoxide detector to efficiently heat and cool the
home. Contractor shall check supply and return duct work for blockage or restrictions, and be
responsible for proper operation and or replacement if needed; all supply duct points are to be
sealed to prevent air leaks. Installation shall conform to local codes. Remove and haul away old
furnace, duct work, and.debris.

power Direct Vent gas water heater with minimum 6 year manufactures warranty, installation is to
include PVC side wall vent pipe. Water heater shall include new shut off valves on hot and cold
water lines, pressure relief valve and copper or galvanized drain pipe installed within 6" of concrete
floor. Installation shall include all hardware and needed electrical circuitry for proper installation
and function.

The above furnace has afive-year factory warranty on all parts as well as a lifetime
factory warranty on the heat exchanger, equipment only. The air-conditioner also has a
five-year factory warranty on parts. There is one year warranty on all labor.
Installation of 1-Richmond, 40 gallon, power direct vent gas water heater with 6 year
manufacturer warranty and one year on labor. This installation includes vent pipe, shut
off valves and pressure relief valve, and new gas pipe for water heater only.

Dear Mr. Young,
Upon electrical inspection, a damaged 100 amp fuse panel was found. The electrical
panel can be replaced with a 100 amp breaker panel without the need for an entire upgrade.
A safety upgrade should be done. Garman Electric will:
• Replace existing fuse panel with new 100 amp
• Bring bonding /grounding up to code
• Add battery operated smoke alarms
• Add basement stairs light
• Repair back outside fixture
• Run new dedicated circuits for AC/furnace
• Add # 12 circuit for kitchen countertop*
*wiremold boxes may have to be used
• Add weatherproof GFCI's in front and back of house
• GFCI protect garage receptacles
• Add # 12 GFCI in bathroom
Our price for the above described work is $2,540.00.
Our pricing is based on current trade prices and is good for 30 days.
All work is to be done during the hours of 7:00 a.m. - 4:30 p.m., Monday through Friday,

AGREEMENT FOR SUPERVISION OF REAL PROPERTY
REHABILITATION
This Agreement is entered into by and between the City of South Bend
Redevelopment Commission and Diane C. Curneal Owner ,residing at 2238 Beverly
Place, South Bend, IN 46616.
1. Owner has entered into an agreement with City of South Bend-
Redevelopment Commission dated November 6, 2009 for a Home Improvement Loan
in the amount of Six Thousand Six Hundred 8v 00/ 100 ($6,600.00) to be used for the
sole purpose of rehabilitating the premises commonly 2238 Beverly Place, South Bend,
IN, 46616 and more full described as:
Lot 442 Woodlawn 2°d Add.
2. Owner has entered into an Agreement for Rehabilitation of Real Property
dated with Christian Builders, Ernsperger Cooling 8v Heating and
Garmen Electric (Contractor) for the rehabilitation of said premise.
3. Owner 8s Commission hereby authorizes the Division of Community
Development to monitor and inspect the rehabilitation work, to approve or disapprove
Contractor's performance, to approve or disapprove proposed changes, to issue stop
work orders when Supervisor determines that such action is in the Owner's best
interest and to perform all other acts necessary to insure that Contractor fulfills its
obligations under the Agreement for Rehabilitation of Real Property, including all
terms and conditions thereof.
4. Owner 8s Commission hereby authorizes the Supervisor to disburse the
proceeds of Owner's Home Improvement Loan to pay the costs of rehabilitation
performed at the above described premises, pursuant to the Agreement for
Rehabilitation of Real Property. Disbursement of said loan proceeds shall be made
pursuant to the terms and conditions of said Agreement for Rehabilitation of Real
Property.
