We will begin work as instructed by the contract document or earlier (if allowed). We will
utilize one crew of 8-10 men to tear off the existing roof system and install the new roof
system. We plan to complete the work within the time frame allowed by the contract
documents.
2. Please list the names and addresses of all subcontractors (i.e. persons or firms outside your own firm
who have performed part of the work) that you have used on public works projects during the past five (5)
years along with a brief description of the work done by each subcontractor.

The most recent roofing work was completed in 2003. This work included a 10 year full
system warranty that has since expired.
1.3 In response to a Pre -Bid Meeting question regarding the locations that require masonry work
prior to the roof repair being completed:
We intend to provide an intermediate completion date for the masonry work that will require
that part of the buildings masonry to be completed by a certain date to provide enough time
for the roof repair to be completed in that area.

It. Project Overview
The work shall be carried out under one prime contract.
A. Base Bid covers the removal of portions of the existing roof and installation of a new roof
according to the intent of the Plans and Specifications.

will start when the Notice to Proceed is delivered and signed.

Plans and Specifications
The Contract Documents are on file and available for public inspection commencing on
the first advertise date during regular working hours at the Department of Public Works
(1316 County -City Building, South Bend, Indiana), and at MACIAF 3215-A Sugar Maple
Court, South Bend, IN 46628. Additionally, the Contract Documents will be available that

located at 1303 Northside Blvd., South Bend, Indiana, 46615), http://www.e-aro.com,
(574) 287-2944, toll free at (800) 783-7231. There will be a non-refundable charge for
reproduction as set by ARC for every set of documents for all bidders.
Bids are due at the Office of the Board of Public Works, County -City Building Room
1316, 227 West Jefferson Blvd, South Bend, Indiana, 46601 until the hour of 10:30 a.m.,
Local Time, on December 22, 2016.
A checklist for bidders is included in the Contractor's Bid for Public Works, sheet 1.
Acknowledgement of receipt of any Addendum to be included in bid submission.
Bids must be on the City of South Bend Contractor's Bid for Public Work Form,
accompanied by a Certified Check or Bid Bond in the amount of not less than five percent
(5%) of the base bid plus any alternates, in a sealed envelope noting the project name,
number and your company's information on the front.
Upon award, the successful Bid provider shall supply the following bonds:
• Payment Bond within seven (7) days of the Notification of Award for an amount
equal to one hundred percent (100%) of the contract amount.
• Performance Bond within seven (7) days of the Notification of Award for an
amount equal to one hundred twenty-five percent (125%) of the contract amount.
Maintenance bond within ten (10) days of acceptance of the project by the City of
South Bend, for an amount equal to ten percent (10%) of the final contract price,
guaranteeing for a period of three (3) years after the date of acceptance of the
project by the City of South Bend.
MBENVBE goal is 7.1 % of the total bid amount. MWBE Forms must be completed,
signed, and handed in with the bid submission. These forms are shown in the bid
proposal, Contractor's Bid for Public Works, sheets 8, 9, and 10.
The Contractor shall apply for and obtain any and all required permits for the work from
local, state, and federal agencies and shall comply with permit requirements, including
the St. Joseph County / City of South Bend Building Department.
Traffic closure requests, if required, will need to be submitted to the City of South Bend
Permit Office (dhanson(a)southbendin.00v) a minimum of 2 days prior to the shift in
traffic. The permit office is open M-F from 9-3, except holidays. The traffic closure request
form will be shown in Addendum 1.
As-builts shall be performed and stamped by a professional engineer or professional land
surveyor in the State of Indiana. One (1) pdf file of the entire set shall be submitted upon
completion of the work.
Payments for work completed during this project will be made every thirty (30) calendar
days. Consistent with provisions of IC 36-1-12-14, the Board of Public Works shall retain
a percentage of payments throughout the duration of the project.
Materials and properties purchased under contract with the Owner that becomes a
permanent part of the structure or facilities constructed are not subject to the Indiana
Gross Retail Tax (Sales Tax). The exemption number will be furnished to the Contractor
upon award.

The Contractor and Surety are bound to the Owner in the amount set forth above, for the payment of which the Contractor and Surety bind
themselves, their heirs, executors, administrators, successors and assigns, jointly and severally, as provided herein. The conditions of this
Bond are such that if the Owner accepts the bid of the Contractor within the time specified in the bid documents, or within such time period
as may be agreed to by the Owner and Contractor, and the Contractor either (1) enters into a contract with the Owner in accordance with
the terms of such bid, and gives such bond or bonds as may be specified in the bidding or Contract Documents, with a surety admitted in
the jurisdiction of the Project and otherwise acceptable to the Owner, for the faithful performance of such Contract and for the prompt
payment of labor and material furnished in the prosecution thereof,, or (2) pays to the Owner the difference, not to exceed the amount of
this Bond, between the amount specified in said bid and such larger amount for which the Owner may in good faith contract with another
party to perform the work covered by said bid, then this obligation shall be null and void, otherwise to remain in full force and effect. The
Surety hereby waives any notice of an agreement between the Owner and Contractor to extend the time in which the Owner may accept the
bid. Waiver of notice by the Surety shall not apply to any extension exceeding sixty (60) days in the aggregate beyond the time for
acceptance of bids specified in the bid documents, and the Owner and Contractor shall obtain the Surety's consent for an extension beyond
sixty (60) days.
If this Bond is issued in connection with a subcontractor's bid to a Contractor, the term Contractor in this Bond shall be deemed to be
Subcontractor and the term Owner shall be deemed to be Contractor.
When this Bond has been furnished to comply with a statutory or other legal requirement in the location of the Project, any provision in
this Bond conflicting with said statutory or legal requirement shall be deemed deleted herefrom and provisions conforming to such
statutory or other legal requirement shall be deemed incorporated herein. When so furnished, the intent is that this Bond shall be construed
as a statutory bond and not as a common law bond.
