Thank you for your consideration of Ideal Consolidated as a potential partner for the HVAV
Preventive Maintenance at Century Center, the Cove and Martin Luther King, Jr Recreation
Center. The following information will provide you with detailed information on our Company,
our capabilities and specifics that will explain why we believe we are the best partner to furnish
all labor, materials and equipment to fully maintain the HVAC systems based on the
specification provided.
About Ideal Consolidated, Inc.
Founded in 1956, Ideal Consolidated has been serving South Bend, Indiana, and the
surrounding area for over 60 years. In 2010, Ideal Consolidated and its sister company, Martell
Electric, were purchased by John and Bonnie Martell. Ideal is a mechanical contractor
specializing in commercial, industrial and institutional piping, plumbing and sheet metal work. A
unique aspect of our operation is that all project managers and estimators have extensive field
training and experience, with most having completed their respective apprenticeship programs.
At Ideal Consolidated, safety is our number one priority and is a major part of our culture both
within the office and out in the field. We have written programs that provide a basis for our
aggressive program of training, weekly safety meetings and jobsite safety audits.
We have two main operating divisions — construction and service. The construction division
specializes in both plan & spec and design build mechanical systems ranging from $5,000 to
$5M. Our service department focuses on plumbing and HVAC service as well as predictive and
preventive maintenance.

Scope of Work
As a local leader in predictive and preventive maintenance, Ideal Consolidated is prepared to
furnish all labor, supervision, tools, materials and equipment necessary to fully maintain all
HVAC at the facilities specified in the Request for Proposal.
This work is based on the following scope.
COVE - AIR HANDLING UNITS (AHU)
1. Inspect coils for blockage.
2. Inspect drain line and drain pan.
3. Lubricate fan bearings as per manufacturer's recommendations.
4. Lubricate motor bearings as per manufacturer's recommendations.
5. Check belt and sheaves.
6. Check motor mounts and vibration pads.
7. Inspect wiring and tighten electrical connections.
8. Check fan operation.
9. Lubricate and adjust dampers and linkage.
10. Check motor operating voltage and amperage.
11. Inspect air filters; change if included in agreement.
12. Check fan assembly.
13. Check and calibrate temperature controls.
COVE - BOILERS
1. Check operation of flame safeguard controls.
2. Check boiler vessel visually for leaks.
3. Check blow down valve.
4. Inspect relief valve for proper rating and operation.
5. Check fuel lines for leaks.
6. Inspect wiring and tighten electrical connections.
7. Check settings and test all operating controls.
8. Check operation of low water cut-off and feed controls.
9. Check burner operation and combustion air equipment.
10. Inspect fireside of boiler.
11. Check expansion tank for proper air cushion or flooding.
12. Check boiler for proper water fill and check PRV valve (fill valve) for proper operation.
IDEAL
13. Pull and clean strainer in PRV valve.
COVE - CONDENSING UNIT
1. Check all compressor screws and bolts for tightness.
2. Check compressor crankcase heater.
3. Test and adjust all safety controls including high pressure, low pressure and oil pressure
switches.
4. Check oil pump pressure if possible.
5. Check compressor oil level if possible.
6. Check compressor operating voltage and amperage.
7. Inspect wiring and tighten electrical connections.
8. Check all contactors and relays for wear.
9. Check refrigerant sight glass.
10. Check compressor suction and discharge pressures.
11. Check for visual indication of a refrigerant leak.
12. Check solenoids and expansion valves for proper operation.
13. Clean condenser if called for in preventative maintenance.
14. Check condenser fan blades.
15. Lubricate bearing as needed.
16. Check and adjust condenser fan belt if applicable.
17. Check condenser motor amperage and voltage.
COVE - PACKAGED ROOFTOP HEATING/COOLING UNITS
1. Inspect coils for blockage and cleanliness.
2. Inspect drain line and drain pan.
3. Lubricate fan bearings as per manufacturer's recommendations.
4. Lubricate motor bearings as per manufacturer's recommendations.
5. Check belt and sheaves (if applicable).
6. Check motor mounts and vibration pads.
7. Inspect wiring and tighten electrical connections.
8. Check fan operation.
9. Lubricate and adjust dampers and linkage.
10. Check motor operating voltage and amperage.

7. Brush and clean coil (water and steam units).
8. Check steam trap.
9. Check operating fan current and voltage.
10. Check entering and exiting air temperature.
11. Check entering and leaving water temperature (water units only).
CENTURY CENTER - AERCO BOILERS
1. Check and record inlet gas pressure.
2. Check gas cushion diaphragm assembly for fatigue.
3. Replace gas cushion diaphragms.
4. Check settings and test all operating controls.
5. Check settings and test all safety controls.
6. Check operation of combustion prove switch.
7. Check operation of program timer.

Many of Ideal Consolidated's key employees have been with the Company for 20 or more years.
The Service Department, led by Mr. Fred Mohler, is fully capable of meeting the news of the
City of South Bend outlined in this proposal.

Not only is our team signatory with Local 172: Plumbers & Pipefitters, but, as a Company, we
are certified to work on Aerco Boilers.
At present time, Ideal Consolidated has 54 employees, 6 of which are in the Service Department
for HVAC. All Service Technicians are licensed plumbers with one being an apprentice. All have
their Freon Certification and one is certified for Aerco Boilers. Collectively, our Service
Technicians have over 100 years of experience in HVAC Service. Other certifications of our
technicians include Backflow testing, Mitsubishi Start-up, Rigging and Confined Space.
Cost
Our pricing for the above scope of work is based on all preventive maintenance taking place
during normal business hours. Price includes labor as well as basic maintenance materials
including belts and filters.

While the HVAC Preventive Maintenance Plan does not require the use of a subcontractor or
offer an opportunity for material to be purchased from a registered DBE/MBE/WBE at this time,
Ideal Consolidated commits to purchasing all replacement equipment during the life of the
contract through a registered WBE material broker.

The undersigned Contractor, being duly sworn upon his/her/its oath, affirms under the penalties of perjury that:
1. Contractor has not, nor has any other member, representative, or agent of the firm, company, corporation or
partnership represented by him, entered into any combination, collusion or agreement with any person relative to the
price to be bid by anyone at such letting nor to prevent any person from bidding nor to induce anyone to refrain from
bidding, and that this bid is made without reference to any other bid and without any agreement, understanding or
combination with any other person in reference to such bidding. Contractor further says that no person or persons,
firms, or corporation has, have or will receive directly or indirectly, any rebate, fee, gift, commission or thing of
value on account of such sale; and
2. Contractor certifies by submission of this proposal that neither contractor nor any of its principals are
presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from
participation in this transaction by any Federal department or agency; and
3. Contractor has not, nor has any successor to, nor an affiliate of, Contractor, engaged in investment activities
in Iran.
a. For purposes of this Certification, "Iran" means the government of Iran and any agency or
instrumentality of Iran, or as otherwise defined at Ind. Code § 5-22-16.5-5, as amended from time -to -
time.
b. As provided by Ind. Code § 5-22-16.5-8, as amended from time -to -time, a Contractor is engaged in
investment activities in Iran if either:
i. Contractor, its successor or its affiliate, provides goods or services of twenty million dollars
($20,000,000) or more in value in the energy sector of Iran; or
ii. Contractor, its successor or its affiliate, is a financial institution that extends twenty million
dollars ($20,000,000) or more in credit to another person for forty-five (45) days or more, if
that person will (i) use the credit to provides goods and services in the energy sector in Iran;
and (ii) at the time the financial institution extends credit, is a person identified on list
published by the Indiana Department of Administration.
4. Contractor does not knowingly employ or contract with an unauthorized alien, nor retain any employee or
contract with a person that the Contractor subsequently learns is an unauthorized alien. Contractor agrees that
he/she/it shall enroll in and verify the work eligibility status of all of Contractor's newly hired employees through
the E-Verify Program as defined by I.C. 22-5-1.7-3. Contractor's documentation of enrollment and participation in
the E-Verify Program is included and attached as part of this bid/quote; and
5. Contractor shall require his/her/its subcontractors performing work under this public contract to certify that
the subcontractors do not knowingly employ or contract with an unauthorized alien, nor retain any employee or
contract with a person that the subcontractor subsequently learns is an unauthorized alien, and that the subcontractor
has enrolled in and is participating in the E-Verify Program. The Contractor agrees to maintain this certification
throughout the term of the contract with the City of South Bend, and understands that the City may terminate the
contract for default if the Contractor fails to cure a breach of this provision no later than thirty (30) days after being
notified by the City.
6. Persons, partnerships, corporations, associations, or joint venturers awarded a contract by the City of South
Bend through its agencies, boards, or commissions shall not discriminate against any employee or applicant for
employment in the performance of a City contract with respect to hire, tenure, terms, conditions, or privileges of
employment, or any matter directly or indirectly related to employment because of race, sex, religion, color, national
origin, ancestry, age, gender expression, gender identity, sexual orientation or disability that does not affect that
person's ability to perform the work.
In awarding contracts for the purchase of work, labor, services, supplies, equipment, materials, or any combination
of the foregoing including, but not limited to, public works contracts awarded under public bidding laws or other
contracts in which public bids are not required by law, the City, its agencies, boards, or commissions may consider
the Contractor's good faith efforts to obtain participation by those Contractors certified by the State of Indiana as a
Minority Business ("MBE") or as a Women's Business Enterprise ("WBE") as a factor in determining the lowest,
responsible, responsive bidder.
In no event shall persons or entities seeking the award of a City contract be required to award a subcontract to an
MBE/WBE; however, it may not unlawfully discriminate against said WBE/MBE. A finding of a discriminatory
practice by the City's MBE/WBE Utilization Board shall prohibit that person or entity from being awarded a City
contract for a period of one (1) year from the date of such determination, and such determination may also be grounds
for terminating the contact for which the discriminatory practice or noncompliance pertains.
7. The undersigned contractor agrees that the following nondiscrimination commitment shall be made a part of
any contract which it may henceforth enter into with the City of South Bend, Indiana or any of its agencies, boards
or commissions.
Contractor agrees not to discriminate against or intimidate any employee or applicant for employment in the
performance of this contract with privileges of employment, or any matter directly or indirectly related to
employment, because of race, religion, color, sex, gender expression, gender identity, sexual orientation, handicap,

I, the undersigned bidder or agent as contractor on a public works project, understand my statutory obligations to the
use of steel products or foundry products made in the United States (I.C. 5-16-8-1). 1 hereby certify that I and all
subcontractors employed by me for this project will use steel products or foundry products made in the United States
on this project if awarded. I understand I have an affirmative duty to notify the City in my bid that my proposal does
not include the use of steel products or foundry products made in the United States. I understand it is my sole
obligation and responsibility to provide ajustification to the City, subject to review and approval, why the cost of
United States made steel or foundry products is unreasonable. Prior to award and upon submission of bid which
does not use steel products or foundry products made in the United States, the City, through its director of public
works, shall make a determination if the price of United States made steel or foundry is unreasonable. I understand
that violations hereunder may result in forfeiture of contractual payments.
