Enclosed is a proposal for a preventive maintenance program at Century Center, Four Winds
Field at Coveleski Stadium and the Martin Luther King Jr. Center.
The inspections will follow the scope of work as outlined in Attachment No. 10. Any needed
repairs to the equipment will be brought to your attention. Approved repairs will be billed on
separate invoices as extras at a reduced rate.
A 5% discount on the preventive maintenance contract is offered if Herrman and Goetz is
awarded all three locations.

agreement between us, and shall become a valid contract after your acceptance. This agreement supersedes all prior
presentations and agreements not incorporated herein.

Annual H.V.A.C. inspections on:
Six (6) Aerco Bench Mark boilers, two (2) Lochinvare water heaters and two (2) Carrier
Chillers.
Chillers to be serviced in the spring.
Boilers and water heaters to be serviced in the fall.

1. Replace oil filter.
2. Replace oil strainer.
3. Replace oil filter drier.
4. Replace drive strainer.
5. Replace refrigerant drier.
6. Perform an oil analysis and provide a report.
7. Clean condenser tubes.

Notes;
• Pricing is based on all refrigerant valves on chillers being in good operating order.
• Tube cleaning does not include chemical treatment to remove excessive scale.

Semi -Annual HN.A.C. inspections on:
One (1) Bryan boiler, four (4) Carrier air handlers and condensing units, three (3) package
heat/cool rooftop units, two (2) make up air units, eleven (11) split air conditioning systems,
sixteen (16) exhaust fans, one (1) duct furnace, ten (10) circulation pumps and one (1) unit
heater.
Spring cooling start up and fall heating start up.

1. Provide and replace belt one time per year.

Semi-annual inspections on:
Two Trane Horizon M.A.U. units
Spring cooling start up and fall heating start up.
• On the ground package units
1. Clean burner.
2. Check operation of burner operating and safety controls.
3. Check gas pressure.
4. Check refrigerant charge.
5. Perform refrigerant leak test.
6. Check condenser coil and evaporator coils for cleanliness.
7. Check compressor amp draw.
8. Check blower amp draw.
9. Check crankcase heaters.
10. Replace blower drive belt once per year.
11. Clean energy recovery media two times per year.
12. Run unit through test modes two times per year.
Notes:
• Replacement of energy recovery media not covered in this agreement.
• Customer to replace air filters and clean condenser coil.
Note; Tasks will be performed where applicable on all the equipment.
Not included in this agreement:
• Labor and/or material required to repair or replace equipment.
• Extra service calls for emergencies (emergency service is available
24/7/365 at additional charges).
Experience:
Herrman and Goetz has been in business and located in South Bend, IN since 1968.
We are large licensed bonded and insured mechanical/electrical contractor employing over one
hundred fifty journeymen plumbers, pipefitters, electricians and service technicians.
We are involved with many area institutions and businesses in construction and service projects.
Some of our customers include: Memorial Hospital, Archway Corporation, City of South Bend
Police Department, City of South Bend Fire Department, Pregis Corporation in Plymouth,
Indiana, Eaton Corporation in South Bend, Indiana. We have worked for the South Bend Waste
Water Division and the south Bend Water Works Department.
Capability/ Skills and Employees:
We have staff that is certified and or licensed in medical gas service and installation, backflow
certifications, high voltage work, refrigeration, H.V.A.C., advanced electronics and controls, all
types of welding and metal fabrication.
We have individuals factory trained and or certified on many major brands of commercial
equipment including but not limited to Lochinvare boilers/water heaters, Bryan boilers, Carrier
H.V.A.C. equipment and Mitsubishi H.V.A.C. equipment.
Our technicians are O.S.H.A. safety certified and are required to have yearly ongoing skills
training.
Herrman and Goetz service department is available seven day a week, twenty four hours per day,
three hundred sixty five day a year.
There are a minimum of two qualified service technician on call at all times.
We also have a full service electrical department with experience in high voltage incoming
power, to low voltage communications and computer network cabling.
Labor rates and hours of operation:
Normal business hours are 7:00 A.M. — 3:30 P.M. Monday through Friday
Time and a half rate from 3:30 P.M. - 11:30 P.M.
Double time rate from 11:30 P.M.-7:00 A.M.
Base hourly rate is $110.24 per hour.
No trip charges.

SPECIAL CONDITIONS
1. This agreement applies only to equipment installed prior to effective date of this agreement
as described on attachment(s). Normal working hours (7:00 am to 3:30 pm) will apply to all
services unless otherwise stated.
This agreement assumes the systems covered to be in maintainable condition. If repairs are
found to be necessary upon initial inspection or initial seasonal start-up, then repair charges
will be submitted for approval. Should these restoration charges be declined, those non -
maintainable items will be eliminated from the program and the agreement price adjusted
accordingly.
It is agreed that the customer will provide reasonable means of access to all equipment that is
to be maintained. H&G Services shall be free to start and stop all primary equipment
incidental to the operation of the mechanical system.
4. It is agreed that the contract price may be adjusted yearly; such adjustments shall be
consistent with current labor rates and material costs. Either party may terminate this
agreement at any time by giving (30) day written notice. Upon cancellation of contract, H&G
Services will compute a final invoice. This final invoice will be a prorated value based upon
current costs to date and original contract margins.
5. If the system is modified, changed, or altered or if any equipment is added or if the system is
removed within the premises or to other premises, then this agreement is immediately
terminated at our option.
6. H&G Services shall not, under any circumstances, be held liable for injury to persons or
damage to property unless such injury or damage is caused by a negligent act of omission or
commission by H&G Services' agent, employees, or subcontractors.
H&G Services and the customer assume the non-occurrence of the following contingencies
which, without limitation, might render performance by H&G Services impractical: strikes,
fires, wars, late or non -delivery by suppliers of H & G Services, and all other contingencies
beyond the reasonable control of H&G Services. Under no circumstances shall H&G
Services be liable for any special or consequential damages whether based upon lost
goodwill, lost resale profits, work stoppage, impairment of other goods or otherwise and
whether arising out of breach of warranty, breach of contract, negligence, or otherwise,
except only in the case of personal injury where applicable law requires such liability.
