C O N T R A C T
TRAFFIC SIGNAL MAINTENANCE
FOR STATE HIGHWAYS IN AND NEAR THE CITY OF SOUTH BEND
SAINT JOSEPH COUNTY, INDIANA

THIS Contract by and between the STATE OF INDIANA, through the INDIANA
DEPARTMENT OF TRANSPORTATION hereinafter referred to as INDOT, and the CITY
OF SOUTH BEND hereinafter referred to as the CITY, for inspection, routine
maintenance, and non -routine maintenance of traffic signals, noted in Exhibit
"A", on INDOT maintained highways in the CITY and surrounding area.
W I T N E S S E T H:
WHEREAS, this Contract is entered into to provide for routine and
emergency maintenance of the entire traffic signal system in the CITY and
surrounding area and to facilitate the movement of traffic therein, and;
WHEREAS, the Exhibits attached and made a part of this Contract may be
revised from time to time by mutual written consent of the parties to this
Contract or as otherwise provided in the Contract, and;
WHEREAS, subject to INDOT approval, the CITY shall determine the
adjustment and timing of all traffic signals on State roads in the City of
South Bend and surrounding areas, and;
WHEREAS, the CITY shall be allowed to modernize signalized
intersections on State roads upon written request and approval by INDOT, and;
WHEREAS, all equipment installed at intersections with State roads,
except as specified in "Special Signal Systems" shall become the property of
INDOT after installation, and;
WHEREAS, the CITY shall have a staff of technicians or a Contract with
an INDOT pre -qualified signal Contractor capable of properly and adequately
maintaining and inspecting the traffic signals on State roads, which are
covered by this Contract, and;
WHEREAS, the parties to this document agree that this Contract will be
in the best interest of safety and convenience for the public, and therefore
to the mutual benefit of both parties;
IT IS THEREFORE AGREED BY AND BETWEEN INDOT and the CITY that the CITY
agrees to perform or have performed the work described herein and INDOT
agrees to compensate the CITY in accordance with the terms and conditions as
follows:

1. TERM OF CONTRACT
a. Effective date: This Contract shall take effect on January 1, 2017. If
the Contract has not been approved by the Attorney General by that
time, then the Contract shall take effect on the first day of the month
that is at least thirty (30) days after the Contract is approved by the
Attorney General, or an authorized representative.
b. Termination date is the earliest of:
1. This Contract shall terminate on the date a lack of funding
determination is made by the State Budget Director as provided in
Section 19 of this Contract.
2. If the State determines there has been an ethics violation, this
Contract shall terminate on the date notice is given to the CITY as
provided in Section 16 of this Contract.
3. This Contract for maintenance of traffic signals shall be for a term
of four (4) years. If renegotiations of a new Contract are not
completed by the expiration date of the Contract, this Contract may
continue in force at the current rates, by mutual written consent of
both parties until a new Contract is signed and approved.
4. This Contract may be terminated by either party on the one (1), two
(2), or three (3) year anniversary of the Effective date described
in Section l.a., provided that the party seeking to terminate gives
at least forty-five (45) days notice.
2. PRE -CONSTRUCTION MEETING
Prior to the first work performed under terms of this Contract, the CITY
shall schedule a Pre -construction Meeting at a time and place convenient
to the INDOT La Porte District Traffic Engineer (the DTE) . If the CITY
intends to utilize a Contractor for portions of the work, representatives
of the Contractor shall attend the meeting. INDOT will provide the CITY
with the names and telephone numbers (or other electronic means of
contact) of INDOT personnel that may be contacted. At the pre -
construction meeting the CITY shall submit to the Engineer, as follows:
a. Schedule of operations for review and approval. This schedule shall
consist of the hours of work for a normal work day.
b. The method or handling, transportation, and storage of the INDOT
materials.
C. List of the traffic signal technicians with copies of Level II Traffic
Signal Technician certificates and resumes detailing the work histories
of the technicians. This list shall also indicate if the technician

will be responding to emergency calls or performing work inside
controller cabinets and can perform the requirements of a Traffic
Signal Technician as defined by this Contract (see EMERGENCY RESPONSE
MAINTENANCE).
d. Contact person - This shall be a local person with whom the DTE will
discuss and route all work. This person is expected to understand all
aspects of traffic signals.
e. Phone numbers for the CITY office (business, emergency, and other
electronic means of contact), answering service, technicians, and
supervisor responsible for the work included in this Contract.
f. List of all Subcontractor(s) with names, phone numbers, and job duties
of the people that may be performing work on this Contract (Loop
cutting, anchor bolt welding, fiber optic work, etc.).
3. MAINTENANCE OF TRAFFIC
a. Maintenance of traffic shall be in accordance with Section 801 of the
IIVD0T Standard Specifications and INDOT's Work Zone Traffic Control
Guidelines: Construction, Traffic, Maintenance and Utility Operations
2013.
b. If unusual working conditions occur, the CITY shall provide the DTE a
proposal for the traffic control measures to be used, subject to
approval. If the CITY finds it necessary to temporarily occupy part of
a shoulder or the traveled width of pavement and restrict traffic, the
CITY shall provide, at its own expense, all cones, barrels, arrow
boards, barricades, lights, flaggers, signs, and other traffic control
devices as specified, and as deemed necessary by the DTE.
c. Construction warning lights shall be in accordance with the applicable
requirement of 801.11, except the plastic drums or construction warning
signs used only for maintaining traffic during daylight hours will not
require a Type A low intensity flashing warning light.
d. When aerial work is being performed, traffic shall not be permitted to
pass beneath personnel working overhead. When aerial work is being
performed which occupies a portion of the traveled way, a minimum of
one flagger will be required. All equipment which is used on or near a
traveled way shall be equipped with revolving amber warning lights.
These warning lights shall be a minimum of 8 inches in diameter. Such
lights shall be clearly visible to approaching vehicles through all
phases of the equipment operation for a minimum distance of 500 feet.
Strobe warning lights may be used provided that they have and output of

2,000,000 candlepower (2,000, 000 cd) during daylight hours and
1, 000, 000 candlepower (1, 000, 000 cd) during other hours.
e. Identification placards shall be affixed to all equipment.
4. SCHEDULE OF OPERATIONS
a. The work day shall consist of any S consecutive hours between 6:OOAM
and 5:30PM. A work week shall consist of 40 hours on Monday through
Friday. All scheduled and non -emergency activities shall occur during
normal work days, unless otherwise approved. work outside of these
hours or days will only be permitted with written request specifying
where, when, and what work is to be done as described in Exhibit "H",
which is attached and made a part of this Contract. Such request shall
be submitted two work days in advance and must be approved by the
Engineer prior to any work. If the Engineer does not approve this
request, the work shall be rescheduled to a time approved by the
Engineer. Each day outside the work week that is requested and
approved for work shall also be considered a work day.
b. All visits by the CITY to any intersection shall be recorded on the
maintenance card located in the controller cabinet. INDOT will provide
additional blank maintenance cards to the CITY to supplement existing
full maintenance cards or to replace those maintenance cards that have
been destroyed or become lost. The traffic signal maintenance card
found in each cabinet shall be signed by the technician in charge of
the lamp replacement, aerial inspection, or loop sealing operation.
This record shall include the date, military time, initials and one of
the following notations: Relamp, Loop Replacement, Aerial Inspection,
or other description to indicate the work performed. The Relamp and
Aerial Inspection report shall also be used to document the work
performed.
5. SPECIAL SIGNAL SYSTEMS
a. Wherever the CITY has installed signal systems utilizing combination
traffic signal/streetlight poles or specialized controller equipment on
State Highways, and where or when the system is controlled and operated
by the CITY, all specialized materials, controllers, conduit, cabling,
and hardware uncommon to INDOT shall be furnished, maintained,
controlled and operated by the CITY. All unique material furnished by
the CITY shall remain the property of the CITY. The CITY shall maintain
an adequate supply of all unique materials to properly maintain said
signal systems on State Highways.

b. As long as a highway remains under the jurisdiction of INDOT, if the
control, operation, or maintenance of a traffic signal on a special
signal system is not satisfactory to INDOT, said signal will be removed
from the special signal system by the CITY and placed on a separate
system controlled by INDOT.
6. DEFINITIONS
a. CITY Holiday:
1. the days annually approved by the South Bend Board of public works
as holidays for CITY employees.
2. shall be defined in Exhibit "F" attached and made a part of this
Contract.
3. updated annual lists shall be provided without
affecting status of this Contract.
4. is prescribed as beginning at 6:00 a.m. of the normal work day it
replaces and ending at 6:00 a.m. the next day. Holiday rates apply
only during this time frame.
S. CITY holidays are overtime pay.
b. Saw Slot for Roadway Loop and Sealer: Includes, cutting the saw slot,
installing loop wire, supplying and installing the cord and sealer, and
splices to existing cable.
c. Signal Bulb Wattage Table: For traffic signal bulb change -out purposes
(routine or non -routine) the table in Exhibit "G" gives the proper
lamp/wattage for various types of indications. Light emitting diode
(LED) lamps shall replace incandescent bulbs when this is feasible.
d. Skilled Labor: Electrician, Foreman, Equipment Operator
e. System Retiming: INDOT will perform a coordination timing analysis in
conjunction with a 4 year schedule. INDOT will provide the CITY with
updated timing sheets showing the implemented changes.
f. Traffic Signal Technicians:
1. must hold a Level II Traffic Signal Electrician certificate which.
has been granted by the International Municipal Signal Association
(IMSA). All electrical wiring terminations and splices; controller
and cabinet set-up; and testing, review, and turn -on of all
operational apparatus at each location shall be done by or in the
presence of and under the responsible charge of an employee of the
CITY who holds a Level II certificate. Supervision of non-
electrical, traffic signal related construction work and traffic
control shall be done by a person holding a Level I Work Zone

Traffic Safety Specialist certification which has been granted by
the IMSA, or an equivalent certification approved by INDOT. The
CITY shall have a minimum of two (2) Traffic Signal Technicians each
with fully equipped vehicles (see Exhibit "N") available at all
times to satisfy the emergency response maintenance requirement.
Each vehicle shall also be equipped with the proper material, tools,
and meters to perform all emergency maintenance activities.
2. Each technician shall be able to diagnose and repair malfunctions
with the equipment listed below:

3. INDOT may install other brands and models of equipment.. The CITY
will be issued manuals for such new equipment.
4. TRAFFIC SIGNAL TECHNICIAN shall understand:
i. How to install timing values into controllers by hand and via
laptop.
ii. How time based coordinators regulate actuated controllers with
background cycles within a coordinated signal system.
iii. Detector input routing and switching throughout the cabinet.

iv. Common aspects of signal phasing including but not limited to
split phasing and lead -lag phasing.
v. Volume Density settings.
5. Field repairs of solid state equipment will be limited to
replacement of faulty equipment and returning such equipment to the
INDOT in exchange for replacement equipment.
g. Trucks:
1. Pick-up truck used by unskilled labor or as parts truck.
2. Aerial truck- vehicle used and charged for as "Base Vehicle" tech
(skilled labor) dispatches; equipment that is capable of lifting one
or more workers and material for the purpose of overhead work.
3. Boom/Auger Truck: Truck mounted equipment capable of lifting and
setting poles or mast arms, and drilling holes.
h. Unskilled Labor: Common Laborer, Escort Service, Flag Person
7. INVENTORY REPORTING
a. INDOT will provide to the CITY a "Stock Transfer Report" listing
materials and equipment provided to the CITY. A list of the items that
the department shall furnish to the CITY is listed in Exhibit "L" INDOT
Provided Materials. A received slip shall be verified and signed by a
delegated representative of the CITY and given to INDOT. INDOT will
provide to the CITY a "Stock Transfer Report" listing materials and
equipment returned by the CITY. A returned slip will be verified and
signed by a representative of INDOT and given to the CITY.
b. The CITY shall supply an itemized report of all material supplied by
INDOT that is in the following status:
1. Stock on hand at beginning of period.
2. Stock obtained during the period.
3. Stock used during the period.
4. Stock on hand at the end of the period.
5. Stock returned to Indiana Department of Transportation.
c. Such report shall reflect the date and where INDOT materials were used,
received, or transferred. Reports shall be submitted within 14 days
following the let day of the months April, July, October, and January
for all material reflecting activity for that three month period. At
the end of the Contract, reports for all material shall be submitted.
d. In addition, the CITY will be required to supply the locations where
the materials are used, by type of material. The format of this report
shall be determined prior to the effective starting date of this

Contract. Reports shall be submitted within 14 days following the 1st
day of the months April, July, October, and January for all material
reflecting activity for that three month period.
e. The CITY shall attach a green tag on all damaged or defective traffic
signal equipment returned to INDOT. This green tag shall be completely
filled out including the location where the equipment was in operation
and a description of the equipment's problem. INDOT will provide green
tags to the CITY.
8. SCHEDULED ROUTINE MAINTENANCE
a. "Routine Maintenance" shall be performed by the CITY (or its
Contractor) upon all existing traffic signals as listed in Exhibit "A"
and any others which may be installed or taken over by INDOT during the
effective period of this Contract.
b. "Routine Maintenance" shall include the repair or change -out of the
following traffic signal equipment, due to any reported Operation
Malfunction during the hours of a normal work day, from the daily start
time up to, and including one (1) hour before the end of a normal work
day:
1. Traffic Signal Controller
2. Detector Amplifiers and Receivers
3. Replacement of burned -out signal bulbs
4. Load relays or switches
5. Conflict Monitors or malfunction management unit, Preempts,
Coordinators, etc.
6. Power Outages
7. All equipment comprising interconnect systems
6. optically Programmable Signal indications
c. "Routine Maintenance" of all types of installations shall also include:
1. Respond to all calls of reported malfunctioning traffic signals
within two (2) hours.
2. Remove all malfunctioning parts and replace same with new or
reconditioned parts to restore proper functioning, except for
emergency vehicle preemption equipment. Emergency vehicle preemption
equipment, causing the traffic signal to malfunction, shall be
disconnected from the controller only.
d. The CITY shall have available a portable two (2) through eight (8)
phase NEMA Digital Controller Tester with harness for the field testing
of all digital traffic signal controllers manufactured in compliance

with NEMA Standard TS-1-1976 dated November 6, 1975 including any
subsequent revisions. The CITY shall also have available a portable two
(2) through eight (8) phase NEMA Digital Controller Tester with harness
for the field testing of all digital traffic signal controllers
manufactured in compliance with NEMA Standard TS-2-1998 including any
subsequent revisions.
e. The CITY shall keep and provide records of all work performed in making
routine repairs. The CITY shall submit a "Signal Work Activity Report"
(Exhibit "C" attached and made a part of this Contract) for each
intersection where work was performed, attached to a properly executed
monthly invoice voucher.
f. The CITY shall complete the form shown in Exhibit "D" attached and made
a part of this Contract in case of conflict monitor or malfunction
management unit flash. Reports shall be submitted with the monthly
invoice.
9. SCHEDULED MAINTENANCE
The CITY shall schedule periodic visits to each intersection shown in
Exhibit "A" at a frequency determined by the time periods designated on
the Preventive Maintenance Check List, shown as Exhibit "B" attached
and made a part of this Contract. The CITY shall perform all work as
noted on the Preventive Maintenance Check List for each intersection.
The work shall be done by or in the presence of and under the
responsible charge of an employee of the CITY who holds a Level II
Traffic Signal Electrician certificate which has been granted by the
IMSA.
a. Loop Testing. This is for testing and recording all results of each
loop detector lead-in cable. This shall include all labor, equipment,
materials, and traffic control required to accomplish this work. This
work shall be performed by a Traffic Signal Technician and completed
between February 1st and May 315z. This work shall include testing each
Lead-in and recording the results for meg-ohm, voltage, resistance, and
micro -henry values and tag each lead-in cable whose tag is missing, not
securely attached or not easily read as needed (see Exhibit "I"
Detector Loop Tagging Table Procedure). If there are any known loops
cut out of the loop system for that lead-in, the `CUTOUT' box shall be
marked. If the loop is operating regardless of loops cut out, the
`WORKS' box shall be marked. Loops that test poorly but still operate
should be left working. All values shall be entered on the Loop

Testing Table in Exhibit "J". If loop systems are found defective and
will not operate, the associated phase may need to be placed on recall
and timings adjusted as directed by the Engineer. The Contractor shall
inform INDOT of non-functional loops verbally and submit the Loop
Testing Table reports with the monthly updates.
b. Annual Signal Bulb Change -out, Lens Cleaning and Aerial Inspection.
1. This includes cleaning all lenses, replacement of all signal and
cabinet bulbs, cabinet filter, and inspection of aerial
installations. This work shall consist of replacing traffic signal
lamps, aerial inspection, and making minor repairs to traffic
signals and flashers at the locations shown in the Exhibit "A".
Minor work shall include aligning signal heads and signs. The CITY
shall replace all signal bulbs with new signal bulbs and also clean
the lenses and reflectors in conjunction with the annual signal bulb
change -out. A Signal Bulb Replacement and Aerial Inspection sheet
needs to be filled out for each location where work was completed
(refer to Exhibit "0").
2. Signalized intersections included in other Contracts and agreements
will not be included in the lamp replacement and inspections portion
of this work. A listing of the locations which will require lamp
replacement and aerial inspection is shown in Exhibit "A". This work
shall be performed between April and September of each year that
this Contract is effective.
3. The CITY shall provide all lamps necessary to complete this work.
All 150 Watt bulbs shall be P25 or A23 size and shape. Lenses for
each type of signal indication shall be washed on both the inside
and outside with an approved cleaning solution. The reflector shall
be washed with the same solution. Both lens and reflector shall be
wiped dry with no smears or residue remaining on the surfaces.
Lamps shall be installed such that the filament of the lamp appears
to be in the shape of a U. This adjustment shall be made by means
of rotating the socket. Lenses which are semi -opaque, cracked, or
include holes shall be replaced. Visors or louvers which are
missing or damaged shall be replaced. The District Traffic Engineer
shall be contacted for availability of replacement visors. No
additional payment will be made for repairs of this nature.
4. If INDOT has installed light emitting diode (LED) indications in
place of the bulb/reflector/refractor assembly, the CITY shall clean

the lens surface of the LED indication.
5. Signal head mounting hardware (i.e. weather head, balance adjuster,
mid -mast assembly, disconnect hanger, horizontal pipe arm and
clamps) found to be worn or damaged shall be noted by the CITY and
the same reported in writing to INDOT. Wires shall not be closed in
doors. Sockets shall not be tightened into terminal strips. Lamps
shall not be overtightened or undertightened. Aligning of signal
heads will not be permitted unless the appropriate set screw, nut,
bolt, or screw is also tightened. A tool appropriate for tightening
shall be used for aligning signal heads, replacing lenses, replacing
visors, or making adjustments. The repairs shall be made as ordered
by INDOT as non -routine maintenance and charged as non -routine
maintenance.
6. Signal bulbs for optically programmable signal indications shall not
be changed -out annually. They shall only be changed -out in case of a
burned out or destroyed bulb. In the case of burned out bulbs for
optically programmable signals, all costs shall be included in
"Routine Maintenance" unit rates.
c. Red Arrow Signal Indications
In accordance with the 2011 Indiana MUTCD, red arrow signal indications
shall be installed at three -section head protected left -turn signals as
follows:
1. When it is feasible to replace a circular red incandescent signal
indication with an LED indication, the circular red shall be
replaced with a red arrow indication.
2. When it is necessary to replace a circular red LED signal
indication, the circular red shall be replaced with a red arrow
indication.
Any existing "LEFT ON ARROW ONLY" signs shall be removed at the time a
circular red indication is replaced with a red arrow indication. A
standard "LEFT ON GREEN ARROW ONLY" (R10-5) sign is not required but
may remain in place.
d. Conflict Monitor Change -out Testing
The CITY shall test all conflict monitors or malfunction management
units installed on traffic signals listed in Exhibit "A" annually. The
following information shall be supplied by the CITY to INDOT upon
completion:
1. CITY warrants to INDOT that each monitor tested by CITY Personnel

still meets or exceeds the recommended values the test equipment
manufacturer (NEMA) recommends.
2. CITY provides INDOT with a copy of the annual certification of
CITY's testing equipment.
3. CITY provides INDOT with test results for each State conflict
monitor or malfunction management unit tested along with the date
tested and the name of the person conducting the test.
e. CITY provides INDOT with copy of the date and name of person who
removed each conflict monitor or malfunction management unit from
cabinet.
5. CITY provides INDOT with copy of the date and name of person who
installed each conflict monitor or malfunction management unit in
cabinet.
A schedule for testing shall be supplied to the CITY by INDOT and shall
be accomplished during the month of January. The monitor change out
shall be done from July through the end of September. Refer to the
Conflict Monitor Change Out Procedure in Exhibit "M".
10. NON -ROUTINE MAINTENANCE
a. "Non -Routine Maintenance" shall be work performed on INDOT owned
traffic signal equipment, other than the "Routine Maintenance"
items, at anytime as a result of traffic accident, vandalism, storm
or further damage prescribed as an act of God. This work often
consists of work as directed by the Project Supervisor/Engineer
through WORK ORDERS. If the traffic signal installation has
emergency vehicle preemption that is the cause of the reported
malfunctioning or damaged traffic signal, preemption repairs are not
covered under this Contract. In lieu of repair to the preemption
equipment, disconnection of the preemption equipment from the
controller is appropriate. If there has been a knock down of traffic
signal apparatus that contains emergency vehicle preemption
equipment, an attempt should be made, using normal diligence, to
salvage the preemption equipment for re -use.
b. Additionally, "Non -Routine Maintenance" shall be the proper response
to reported damaged traffic signal installations. In the event of
extreme emergency problems, it may be necessary to solicit the
immediate assistance of local law enforcement agencies until such
time that emergency repairs can be accomplished.
c. Damaged, inoperative traffic signals shall not be left unattended. A

signal may be placed in a flashing mode while repairs are being
completed. In the performance of Non -Routine Maintenance under terms
of this Contract, the CITY shall:
1. Keep and provide records of all work performed in making Non -
Routine repairs.
2. Submit a "Signal Work Activity Report (Exhibit "C" attached and
made a part of this Contract) for each intersection where work
was performed, attached to a properly executed monthly claim
voucher.
3. Make no alterations of existing installations unless authorized
in writing by INDOT except in cases of emergency when a traffic
signal is out of operation and no one from INDOT can be reached
to authorize the repairs in which case, the CITY shall make the
necessary repairs and submit damaged equipment to INDOT as
evidence of material used to keep the signal in operation.
4. Provide a copy of the Police Department Accident or Officers
Report in reference to damage to traffic signal installations
that INDOT may attempt recovery of damages.
5. Respond to all calls of reported malfunctioning traffic signals
within two (2) hours.
d. INDOT shall inspect all work performed in making Non -Routine
Maintenance repairs, and recommend full, partial or no payment.
INDOT will not pay for travel time to and from any intersection.
11. TIME BASED AND INTERCONNECT SYSTEMS
The CITY shall check that all interconnected and time based systems are
functioning correctly on a biannual basis in conjunction with the
periodic "Preventive Maintenance" for Actuated Signal Controllers. One
system timing pattern shall be checked per visit on an alternating
basis. If a system timing pattern that is not currently in operation is
desired to be checked, the timing pattern may be changed to the desired
timing pattern, however, this change must occur between the hours of
10:00 a.m. and 2:00 p.m. At each visit, the CITY shall check each local
intersection controller to ascertain that the controller is maintaining
the correct time. If the time check is negative, the CITY shall enter
the correct time in the controller. Additionally, for each system
timing pattern checked at the visit, the CITY will check that the local
intersection controller is receiving the transfer command from the
master control unit and is responding to that command. If it is

determined that the communication between the master and secondary is
not functioning properly, the CITY shall determine the cause of the
communication error. Major repair or replacement of interconnect cable
shall be performed during normal working hours only. Splice repair will
be paid on the basis of labor and equipment used, (straight time only).
Refer to the Fiber Optic Specifications in Exhibit "K".
12. MATERIALS
a.INDOT shall furnish all standard major signal equipment and materials
used in making repairs or replacements, and in the performance of all
maintenance on INDOT owned and controlled signals. Such materials
shall include, but will not be limited to the following: signal heads
or faces (including component parts), signal cable, slip fitters,
pipe arm assemblies, span wire hanger assemblies, controllers,
including flashers, detectors, conduit and fittings, signal metal
poles and/or mast arms, pedestal pipe, cast iron or aluminum base
(with anchor bolts, if required).
b.The CITY shall stock and furnish all necessary specialized materials.,
controllers, conduit, cabling, coordinating equipment, combination
traffic signal -street light poles, and hardware uncommon to INDOT to
adequately maintain the specialized traffic control equipment
installed on INDOT highways at intersections listed in Exhibit "A".
c.Any minor material parts not furnished by INDOT (such as electrical
or friction tape, Sta-Kons, clips, wire, small nuts and bolts, screws
and washers), shall be furnished by the CITY at no extra cost to
INDOT.
d. The CITY shall be accountable and responsible for any materials and
equipment furnished by INDOT that are not considered expendable items
(controllers, controller cabinets, poles, detectors, modules, etc.).
e.All damaged equipment removed from signal installations covered in
this Contract shall be stored by the CITY at its facilities for pick-
up by INDOT.
f. Exhibit H through Exhibit 0. These forms shall be utilized as
needed.
13. ADDITIONS OR DELETIONS OF WORK LOCATIONS
a. INDOT shall notify the CITY in writing of any addition or deletion of
traffic signal installations to Exhibit "A", and the effective date of
the addition or deletion.
b. Within the City of South Bend and surrounding areas, all new traffic

signals which are to be installed by private owners on state .roads by
agreement with INDOT shall be submitted to the CITY for their
information prior to INDOT granting permission for such installation.
c. INDOT shall coordinate the design of new signals or the modernization
of existing signals on State roads with the CITY to insure that they
are compatible with the CITY'S Traffic Control System.
d. The CITY shall be allowed to attend the final inspection of newly
installed or modernized traffic signals for which the CITY shall
provide maintenance under this Contract.
e. The CITY shall accept a new signal installation for maintenance after
INDOT has furnished the CITY a copy of its Form I.C. 63GA (Completion
Report) for installation and a copy of the executed Form T-38a (Change
Order for Signal Maintenance Contract) indicating that the new signal
is installed in accordance with the current INDOT Standard
Specifications and is approved as an addition to this Contract. Said
additions to the Contract must be approved by the "CITY's Board of
Public Works", prior to acceptance.
14. RIGHTS AND RESERVATIONS
a. INDOT reserves the right to enter at anytime onto any signalized
intersection covered under this Contract listed in Exhibit "A" or
into any controller cabinet for inspection, alteration or change to
affect emergency repairs, modifications or to correct malfunctions.
b. No change, which will alter its design, phasing or functional
operations, will be madeto any traffic signal without authorization
from INDOT La Porte District Traffic Engineer.
c. In the event that INDOT awards a separate Contract to modernize,
modify, rebuild or remove any signal installation covered under this
Contract, INDOT reserves the right to determine whether its
Contractor or the CITY will be responsible for the maintenance of
the existing and/or temporary signal installation during the term of
the modernization or modification Contract..
d. INDOT reserves the right to inspect stockpiled INDOT furnished
signal equipment being stored by the CITY (or its Contractor) with
two (2) days written notice supplied to the CITY by INDOT.
e. The CITY shall maintain existing signalized intersection(s) and
equipment as long as they are in service, unless otherwise directed.
The CITY shall assume maintenance of new signalized intersections(s)
and equipment when notified by INDOT. These additions and deletions

may be communicated verbally by INDOT. INDOT, within five (5)
working days of the verbal order, will provide a follow up letter by
fax, email or mail.
15. INSURANCE
a. The CITY and/or its Contractor shall carry protective liability
insurance coverage for all work covered under this Traffic Signal
Maintenance Contract. The insurer shall be a company holding a
certificate of authority issued by the Indiana state Insurance
Commissioner to do business in the State. Prior to commencing work
under this Contract, the CITY or its Contractor shall file with
INDOT a certificate of insurance on an acceptable form, which shall
serve as evidence that the CITY or its Contractor has obtained
insurance in accordance with the following requirements and minimum
limits of coverage.
i. A limit of not less than $100,000.00 for all damages arising
out of bodily injury or death in any one occurrence.
ii. A limit of not less than $100,000.00 for all damages, arising
out of an injury to or destruction of property in an
occurrence, an aggregate limit of not less than $300,000.00
for all damages arising out of injury or destruction of
property during thepolicy period.
b. such insurance shall hold harmless and indemnify INDOT from, for and
against any claim of any person in tort, Contract, or otherwise
arising out of the acts or omissions of the CITY in the execution
and implementation of the traffic signal maintenance (routine and/or
emergency) in compliance with the stipulations of this Contract.
c. If the CITY is self -insured under the provisions of Indiana law, the
CITY shall provide protection for public liability and physical
damage arising from the performance of its obligations under this
Contract in amounts not less than those stated in A. and B. above.
In order to hold harmless and indemnify INDOT, the CITY will at its
sole cost and expense keep such self-insurance in full force and
effect during the term of this Contract or will, at its option,
provide equivalent insurance with a responsible carrier as stated
above.
d. Unless otherwise directed by INDOT, it shall be the responsibility
of the CITY to maintain the old signals and its related equipment as
long as they remain in service and to assume maintenance of the new

signal equipment after it has been accepted by INDOT and after it is
so directed by INDOT. In the event the CITY is requested by INDOT to
respond to a reported malfunctioning traffic signal that is being
temporarily maintained by others, the CITY shall be reimbursed for
all calls at non -routine rates.
16. CITY RESPONSIBILITIES
The CITY responsibilities shall include but are not limited to:
a. Wherever in this Contract any obligation or responsibility is
assumed by the CITY, same shall bedeemed an obligation of said
CITY.
b. The CITY shall perform all work permitted under this Contract to
INDOT's reasonable satisfaction, as determined at the discretion of
INDOT and in accordance with all applicable federal, state, local
laws, ordinances, rules, and regulations.
c. Compliance With Laws
1. The CITY shall comply with all applicable federal, state and local
laws, rules, regulations and ordinances, and all provisions
required thereby to be included herein are hereby incorporated by
reference. The enactment of any state or federal statute or the
promulgation of rules or regulations thereunder after execution of
this Contract shall be reviewed by INDOT and the CITY to determine
whether the provisions of this Contract require formal
modification.
2. The CITY and its agents shall abide by all ethical requirements
that apply to persons who have a business relationship with INDOT
as set forth in IC § 4-2-6 et seg., IC § 4-2-7, et. seg., the
regulations promulgated thereunder. If the CITY has knowledge, or
would have acquired knowledge with reasonable inquiry, that a
state officer, employee, or special state appointee, as those
terms are defined in IC 4-2-6-1, has a financial interest in the
Contract, the CITY shall ensure compliance with the disclosure
requirements in IC 4-2-6-10.5 prior to the execution of this
contract. If the CITY is not familiar with these ethical
requirements, the CITY should refer any questions to the Indiana
State Ethics Commission, or visit the Inspector General's website
at http://www.in.gov/ig/. If the CITY or its agents violate any
applicable ethical standards, INDOT may, in its sole discretion,
terminate this Contract immediately upon notice to the CITY. In

addition, the CITY may be subject to penalties under IC §§ 4-2-6,
4-2-7, 35-44.1-1-4, and under any other applicable laws.
3. The CITY certifies by entering into this Contract that neither it
nor its principal(s) is presently in arrears in payment of taxes,
permit fees or other statutory, regulatory or judicially required
payments to INDOT. The CITY agrees that any payments currently
due to INDOT may be withheld from payments due to the CITY.
Additionally, further work or payments may be withheld, delayed,
or denied and/or this Contract suspended until the CITY is
current in its payments and has submitted proof of such payment
to INDOT.
4. The CITY warrants that it has no current, pending or outstanding
criminal, civil, or enforcement actions initiated by INDOT, and
agrees that it will immediately notify INDOT of any such actions.
During the term of such actions, the CITY agrees that INDOT may
delay, withhold, or deny work under any supplement, amendment,
change order or other Contractual device issued pursuant to this
Contract.
5. If a valid dispute exists as to the CITY's liability or guilt in
any action initiated by INDOT or its agencies, and INDOT decides
to delay, withhold, or deny work to the CITY, the CITY may
request that it be allowed to continue, or receive work, without
delay. The CITY must submit, in writing, a request for review to
the Indiana Department of Administration MOM following the
procedures for disputes outlined herein. A determination by IDOA
shall be binding on the parties.
6. Any payments that INDOT may delay, withhold, deny, or apply under
this section shall not be subject to penalty or interest under IC
§ 5-17-5.
7. The CITY warrants that the CITY and its subcontractors, if any,
shall obtain and maintain all required permits, licenses,
registrations, and approvals, as well as comply with all health,
safety, and environmental statutes, rules, or regulations in the
performance of work activities for INDOT. Failure to do so may
be deemed a material breach of this Contract and grounds for
immediate termination and denial of further work with INDOT.

8. The CITY affirms that, if it is an entity described in IC Title
23, it is properly registered and owes no outstanding reports to
the Indiana Secretary of State.
9. As required by IC 5-22-3-7:
i. the CITY and any principals of the CITY certify that (A)
the CITY, except for de minimis and nonsystematic
violations, has not violated the terms of (i) IC 24-4.7
[Telephone Solicitation Of consumers], (ii) IC 24-5-12
[Telephone Solicitations] I or (iii) IC 24-5-14
[Regulation of Automatic Dialing Machines] in the previous
three hundred sixty-five (365) days, even if IC 24-4.7 is
preempted by federal law; and (B) the CITY will not
violate the terms of IC 24-4.7 for the duration of the
Contract, even if IC 24-4.7 is preempted by federal law.
ii. The Contractor and any principals of the Contractor
certify that an affiliate or principal of the Contractor
and any agent acting on behalf of the Contractor or on
behalf of an affiliate or principal of the Contractor (A)
except for de minimis and nonsystematic violations, has
not violated the terms of IC 24-4.7 in the previous three
hundred sixty-five (365) days, even if IC 24-4.7 is
preempted by federal law; and (B) will not violate the
terms of IC 24-4.7 for the duration of the Contract, even
if IC 24-4.7 is preempted by federal law.
Indemnification - The CITY agrees to indemnify, defend, and hold
harmless INDOT, its agents, officers, and employees from all claims
and suits including court costs, attorney's fees, and other expenses
caused by any act or omission of the CITY and/or its Contractors and
subcontractors, if any, in the performance of this Contract. The
State shall not provide such indemnification to the CITY.
Drug -Free Workplace Certification - The CITY hereby covenants and
agrees to make a good faith effort to provide and maintain a drug -
free workplace. The CITY will give written notice to INDOT within
ten (10) days after receiving actual notice that the CITY or an
employee of the CITY in the State of Indiana has been convicted of a
criminal drug violation occurring in the workplace. False
certification or violation of this certification may result in
sanctions including, but not limited to, suspension of Contract

payments, termination of this Contract and/or debarment of
Contracting opportunities with INDOT for up to three (3) years.
In addition to the provisions of the above paragraphs, if the total
Contract amount set forth in this Contract is in excess of
$25,000.00, the CITY hereby further agrees that this Contract is
expressly subject to the terms, conditions, and representations of
the following certification:
This certification is required by Executive Order No. 90-5, April
12, 1990, issued by the Governor of Indiana. No award of a Contract
shall be made, and no Contract, purchase order or agreement, the
total amount of which exceeds $25,000.00, shall be valid, unless and
until this certification has been fully executed by the CITY and
made a part of the Contract or agreement as part of the Contract
documents.
The CITY certifies and agrees that it will provide a drug -free
workplace by:
1. Publishing and providing to all of its employees a statement
notifying them that the unlawful manufacture, distribution,
dispensing, possession or use of a controlled substance is
prohibited in the CITY's workplace, and specifying the actions
that will be taken against employees for violations of such
prohibition;
2. Establishing a drug -free awareness program to inform its
employees of (1) the dangers of drug abuse in the workplace; (2)
the CITY's policy of maintaining a drug -free workplace; (3) any
available drug counseling, rehabilitation and employee assistance
programs; and (4) the penalties that may be imposed upon an
employee for drug abuse violations occurring in the workplace;
3. Notifying all employees in the statement required by subparagraph
(A) above that as a condition of continued employment, the
employee will (1) abide by the terms of the statement; and (2)
notify the CITY of any criminal drug statute conviction for a
violation occurring in the workplace no later than five (5) days
after such conviction;

4. Notifying INDOT in writing within ten (10) days after receiving
notice from an employee under subdivision (C)(2) above, or
otherwise receiving actual notice of such conviction;
5. Within thirty (30) days after receiving notice under subdivision
(C)(2) above of a conviction, imposing the following sanctions or
remedial measures on any employee who is convicted of drug abuse
violations occurring in the workplace: (1) taking appropriate
personnel action against the employee, up to and including
termination; or (2) requiring such employee to satisfactorily
participate in a drug abuse assistance or rehabilitation program
approved for such purposes by a federal, state or local health,
law enforcement, or other appropriate agency; and
6. Making a good faith effort to maintain a drug -free workplace
through the implementation of subparagraphs (A) through (E)
above.
f. Non -Discrimination - Pursuant to the Indiana Civil Rights Law,
specifically including IC § 22-9-1-10, and in keeping with the
purposes of the federal Civil Rights Act of 1964, the Age
Discrimination in Employment Act, and the Americans with
Disabilities Act, the CITY, its agents, contractors and
subcontractors, shall not discriminate against any employee or
applicant for employment relating to this Contract with respect to
hire, tenure, terms, conditions or privileges of employment or any
matter directly or indirectly related to employment, because of the
employee's or applicant's race, color, national origin, religion,
sex, age, disability, ancestry, status as a veteran, or any other
characteristic protected by federal, state, or local law ("Protected
Characteristics"). The CITY certifies compliance with applicable
federal laws, regulations, and executive orders prohibiting
discrimination based on the Protected Characteristics in the
provision of services. Breach of this paragraph may be regarded as
a material breach of the Contract, but nothing in this paragraph
shall be construed to imply or establish an employment relationship
between the State and any applicant or employee of the CITY.
The State is a recipient of federal funds, and therefore, where
applicable, the CITY and any contractors or subcontractors shall
comply with requisite affirmative action requirements, including

reporting, pursuant to 41 CFR Chapter 60, as amended, and Section
202 of Executive Order 11246.
g. Status of Claims - The CITY shall be responsible for keeping INDOT
currently advised as to the status of any claims made for damages
against the CITY resulting from work performed under this Contract.
The CITY shall send notice of claims related to work under this
Contract to:

17. NOTICE
a. Wherever in or under this Contract notice must or may be given by
INDOT to the CITY, such notice shall be deemed given when addressed
to the CITY at 227 W Jefferson Blvd Suite 1300 N, South Bend, IN
46601 and deposited postage paid in the U.S. Mail system of
collection.
b. Wherever in or under this Contract notice must or may be given by
INDOT to the Project Engineer, such notice shall be deemed given
when INDOT transmits the notice verbally or in writing including
electronic communications to the Project Engineer in person or to
the contact numbers or addresses presented at the preconstruction
conference.
c. Wherever in or under this Contract notice must or may be given by
INDOT to the CITY's Contractor or sub -Contractor, such notice shall
be deemed given when INDOT transmits the notice verbally or in
writing including electronic communications to the Contractor or
sub -Contractor in person or to the contact numbers or addresses
presented at the preconstruction conference.
d. Wherever in or under this Contract notice must or may be given by
the CITY to INDOT, such notice shall be deemed given when received
by the DTE at the LaPorte District Office., 315 East Boyd Blvd,
LaPorte, IN 46350.
18. PAYMENTS
All payments by INDOT shall be made in arrears in conformance with
State fiscal policies and procedures and, as required by IC 4-13-2-
14.8, by electronic funds transfer to the financial institution
designated by the ACADEMY in writing unless a specific waiver has been

obtained from the Indiana Auditor of State. No payments will be made in
advance of receipt of the goods or services that are the subject of
this Contract except as permitted by IC 4-13-2-20. For forms and
additional information, the Contractor may visit the Auditor of State's
website at www.in.gov/auditor/forms.
In its normal course of business INDOT will provide payment for:
a. "Routine Maintenance" shall be paid for by INDOT at the flat unit
rate per month for each intersection as listed in Exhibit "A". The
flat rate per -month for each inspection shall include all labor,
vehicles, and equipment used in carrying out "Routine Maintenance, -
and shall be at the unit rates shown in Exhibit "E" attached and
made a part of this Contract.
b. INDOT shall pay the CITY under "Non -Routine Maintenance" for labor,
equipment, and/or material used in making said "Non -Routine" repairs
at the unit rates, shown in Exhibit "E".
c. "Periodic Preventive Maintenance" shall be paid by INDOT at a flat
unit rate per intersection per scheduled visit by type of control
unit as listed in Exhibit "A". The flat unit rate per intersection
shall include all labor, vehicles, and equipment used in carrying
out "Periodic Preventive Maintenance" and shall be at unit rates
shown in Exhibit "E".
d. Annual Activities:
i. "Annual Signal Bulb Change -Out" shall be paid for by INDOT at
a flat rate per intersection. The flat rate per intersection
shall include all labor, vehicles, and equipment used in
carrying out the "Annual Signal Bulb Change -Out", and shall be
at the unit rate shown in Exhibit "E".
ii. "Annual Conflict Monitor or Malfunction Management Unit
Testing", shall be paid for by INDOT at a flat rate per
intersection. The flat rate per intersection shall include all
labor, vehicles, and equipment used in carrying out the
"Annual Conflict Monitor or Malfunction Management Unit
Testing", and shall be at the unit rate shown in Exhibit "E".
e. The annual monetary limitations shall begin when this Contract takes
effect. The annual monetary limitations shall be as follows:
First Year $210,364.06
Second Year $216,674.98
Third Year $223,175.23
Fourth Year $229,870.49

f. If this Contract is terminated or expires, then the current year's
rates will apply during the timeframe that a new contract is being
negotiated.
19. GENERAL PROVISIONS
a. Attorney General Approval - This Contract shall not be effective
unless and until approved by the Attorney General of Indiana, or an
authorized representative, as to form and legality.
b. Assignment - The Contractor binds its successors and assignees to
all the terms and conditions of this Contract. The Contractor shall
not assign or subcontract the whole or any part of this Contract
without INDOT's prior written consent.
c. Debarment and Suspension -
1. The Contractor certifies by entering into this Contract that
neither it nor its principals nor any of its subcontractors are
presently debarred, suspended, proposed for debarment, declared
ineligible or voluntarily excluded from entering into this
Contract by any federal agency or by any department, agency or
political subdivision of INDOT. The term "principal" for
purposes of this Contract means an officer, director, owner,
partner, key employee or other person with primary management or
supervisory responsibilities, or a person who has a critical
influence on or substantive control over the operations of the
Contractor.
2. The Contractor certifies that it has verified the state and
federal suspension and debarment status for all subcontractors
receiving funds under this Contract and shall be solely
responsible for any recoupment, penalties or costs that might
arise from use of a suspended or debarred subcontractor. The
Contractor shall immediately notify INDOT if any subcontractor
becomes debarred or suspended, and shall, at INDOT's request,
take all steps required by INDOT to terminate its Contractual
relationship with the subcontractor for work to be performed
under this Contract.
d. Delay of work. No delay or failure by either party to exercise any
right hereunder, and no partial or single exercise of any such

right, shall constitute a waiver of that or any other right, unless
otherwise expressly provided herein.
e. Force Majeure, Suspension and Termination - In the event that either
party is unable to perform any of its obligations under this
Contract or to enjoy any of its benefits because. of natural disaster
or decrees of governmental bodies not the fault of the affected
party (hereinafter referred to as a "Force Majeure Event"), the
party who has been so affected shall immediately give notice to the
other party and shall do everything possible to resume performance.
Upon receipt of such notice, all obligations under this Contract
shall be immediately suspended. If the period of nonperformance
exceeds thirty (30) days from the receipt of notice of the Force
Majeure Event, the party whose ability to perform has not been so
affected may, by giving written notice, terminate this Contract.
f. Funding Cancellation. When the Director of the State Budget Agency
makes a written determination that funds are not appropriated or
otherwise available to support continuation of this Contract, the
Contract shall be canceled. A determination by the Budget Director
that funds are not appropriated or otherwise available to support
continuation of performance shall be final and conclusive.
g. Governing Laws. This Contract shall be governed, construed, and
enforced in accordance with the laws of the State of Indiana without
regard to its conflict of laws rules. Suit, if any, must be brought
in the State of Indiana.
h. Merger & Modification. This Contract constitutes the entire agreement
between the parties. No understandings, agreements, or
representations, oral or written, not specified within this Contract
will be valid provisions of this Contract. This Contract may not be
modified, supplemented, or amended, except by written agreement signed
by all necessary parties. Except, elements of the traffic signal
maintenance activities and record keeping may be added, deleted or
modified in the normal course of business without submittal to the
Attorney General for approval.
i. Non -Waiver. No delay or failure by either party to exercise any
right hereunder, and no partial or single exercise of any such
right, shall constitute a waiver of that or any other right, unless
otherwise expressly provided herein.

j. Order of Precedence. Any inconsistency or ambiguity in this Contract
shall be resolved by giving precedence in the following order:
(1) This Contract,
(2) Attachments prepared by INDOT,
(3) Attachments prepared by the Contractor.
All of the foregoing are incorporated fully by reference.
k. Severability. Wherever possible, each provision of this Contract
shall be interpreted in such manner as to be effective and valid
under applicable law, but if any provision of this Contract shall be
prohibited by or invalid under applicable law, such provision shall
be ineffective only to the extent of such prohibition or invalidity,
without invalidating the remainder of such provision or the
remaining provisions of this Contract.
1. Subsequent Acts. The parties agree that they will, at any time and
from time to time, from and after the execution of this Contract,
upon request, perform or cause to be performed such acts, and
execute, acknowledge and deliver or cause to be executed,
acknowledged and delivered, such documents as may be reasonably
required for the performance by the parties of any of their
obligations under this Contract.
m. Substantial Performance. This Contract shall be deemed to have been
substantially performed only when fully performed according to its
terms and conditions and any modification thereof.
n. Taxes. The State is exempt from most state and local taxes and many
federal taxes. The State will not be responsible for any taxes levied
on the CITY as a result of this Contract.
o. Previous Contracts Superseded or Revoked. This Contract supersedes
and revokes any previous Contract between said parties hereto
pertaining to traffic signal maintenance.
p. Authorizations. Any person executing this Contract in a

PREVENTIVE MAINTENANCE CHECK LIST
1. Signal Indications: to be performed on the periodic scheduled visits biannually.
A. All lights operating
B. In proper sequence
C. No lenses broken or cracked
2. Signal Heads: to be checked in conjunction with the `annual signal bulb change -out, lens cleaning and
aerial inspection' scheduled visit.
A. Proper height (visual check)
B. Proper Alignment: Horizontal, vertical, and rotation of all types of heads including a check of the
aiming of programmable heads. Aligning of signal heads will not be permitted unless the
appropriate set screw, nut, bolt, or screw is also tightened. A tool appropriate for tightening shall
be used for aligning signal heads, replacing lenses, replacing visors, or making adjustments.
C. No visors missing. Visors or louvers which are missing or damaged shall be replaced. The District
Traffic Engineer shall be contacted for availability of replacement visors.
D. No pinnacles missing
E. Annual check of eye bolt, weatherhead, and balance adjuster- to be done with annual bulb
change/ inspection/ cleaning
F. Terminal block connections snug (in disconnect hangers, if present) to be done with annual bulb
change/inspection/cleaning
G. Check for snugness of all mechanical connections - annual
H. Wires shall not be closed in doors. Sockets shall not be tightened into terminal strips. Lamps shall
not be over tightened or under tightened.
3. Overhead Cables: to be checked in conjunction with the "annual signal bulb change -out, lens cleaning
and aerial inspection" scheduled visit.
A. Correct drip loops: loops should not allow water to flow into signal heads or disconnect hangers.
B. No improper splices and visual inspection of splices, repair, or damaged splices.
C. Right number of cable clamps - annually
D. Proper spacing of cable rings - annually
E. No sagging or loose cables or "A" wires.
F. Down guys tight
G. Pole eye bolts in good condition - annually
H. Check for worn wires at weatherhead, to be done at annual bulb change/inspection/cleaning
4. Service Disconnect Box: to be checked in conjunction with the "annual signal bulb change -out, lens
cleaning and aerial inspection" scheduled visit.
A. Box and conduit mounted securely
B. All connections snug
C. Ground wire secured to pole
D. Ground rod clamp snug - if possible

G. Conflict Monitor or Malfunction Management Unit checking and/or replacement in conjunction
with Logistic Support change out program
H. Accessory equipment functioning properly
6. Signal Controller Cabinet - to be performed on the periodic scheduled visits biannually.
A. Mounted securely to pedestal, pole, or foundation
B. External conduit mounted securely, if present
C. Oil door hinges
D. Proper size fuse of breaker
E. Check to see if convenience bulb works
F. Clocks: check programming and time (including day of week)
G. Properly size interconnect fuses
H. Check flasher contacts
I. Ground wire connection snug
J. Maintenance card present, add additional card, if necessary
K. Sign, put time and date on maintenance card
L. Clean cabinet (vacuuming & cleaning shelves/walls), change filters - every twelve (12) months
M. Check cabinet to be waterproof - every six (6) months
N. Check cabinet condition - note if painting is necessary - every six (6) months

Procedure for completing "Work Activity Report" form.
1. REPORT NO: Activity report number.
Format -month year-DataID-report number: 0102-3098-001 (Office Use only)
2. WORK ORDER NO.- If work being done is for a Work Order then enter the work order number.
3. COMM NO.- Commission number given on the intersection list. This number is on the cards in
the cabinet, prints and timing sheets as well as listed in the intersection list.
4. LOCATION- Intersection name. This should always include the State, US or Interstate.
5. CITY- The CITY where work is being done.
6. REPORTED BY- Who reported the trouble call and their agency (if applicable)
7. ACTIVITY- This designates whether this work is E: Emergency or S: (Non -Routine, Work order
or Scheduled work)
8. ACCIDENT- Indicate if an accident occurred at any time while at the intersection.
9. TIME RECV- The time this problem was reported in military time.
10. DATE RECV- The date the problem was reported.
11. TIME ARRV- Time arrived at the problem location in military time
12. DATE ARRV- The date arrived at location.
13. TIME COMP- Time completed in military time.
14. DATE COMP- The date completed.
15. PROB REP 1 & 2- The malfunction reported and the appropriate code from the "Reported

Detector Loop Tagging Table Procedure
The Loop Tagging Table shall be completed from the signalized intersection plan, including
properly identified loop numbers, vehicle directional movements with associated lane designations,
phase assignments, individual loop numbers and count output numbers.
Loop Number:
The Loop Number shall be the detector rack channel number, for each additional rack added
the channel numbers shall be increased by sixteen (ie: the third detector racks first loop would be 33).
The loop numbers shall be assigned in two groups: Counting and non -counting channels. Each group
shall follow the direction and lane designation convention. To minimize the number of detection
channels required, counting loop channels shall be grouped first followed by non -counting channels.
The order of the loops shall minimize the number of loop amplifiers and detector racks required for
the operation. If micro -loops are used with Canoga amplifiers loops shall be numbered in three
groups. Canoga amplifier channels shall follow non -counting channels
Direction of Vehicle movement:
The loop tagging table shall begin with highest priority route, either the northbound or
eastbound movement containing phase 2. The corresponding first cardinal direction letter for each
movement (N- north, S- south, E- east, W-west) shall be entered in the loop tagging table. The
remaining movements shall be labeled and listed according to plan view, clockwise from the
movement containing phase 2 (followed by the movement containing 4,6 and 8, skipping missing
movements and corresponding phases). Phase designations may vary for specialized signal
operations.
Lane Designation:
Loops shall be listed in the tagging table starting with the farthest left lane for each approach
and continue to be listed sequentially to the right and shall be designated as follows:
Left Turning Lanes: Exclusive Left turning lanes shall be designated by an "L" for left in the
table. Multiple left turn lanes on a single approach shall be designated by an additional alpha
character identifier so that "LA" represents the left most lane, followed by "LB" and "LC"
respectively, and continuing for each subsequent left turn lane for that approach.
Through Lanes: Through lanes or a shared through/right or shared through/left lane shall be
designated with an "A" for the left most lane followed alphabetically for each subsequent through
lane. Loops from different lanes shall not be connected together. Loop(s) placed in the right turn
radius are to be designated in the through lane. A radius loop that is counting shall have an exclusive
lead-in.
Right Turning Lanes: Exclusive Right turning lanes shall be designated as "R" for right.
Multiple right turning lanes on a single approach shall be designated by an additional alpha character
identifier so that "RA" represents the left most right turn lane, followed by "RB" and "RC"
respectively, and continuing for each subsequent right turn lane for that approach.
Phase Number:
The phase number shall be the controller phase number (1-16) assigned to each protected
movement corresponding to each lane.

Loops:
On approaches with stop bar detection, loop number 1 will normally be at the stop bar for the
lane. Loop number 2 shall be the next loop away from the stop bar in the same lane. The loop
numbering shall continue in order, for the through portion of the lane, from the stop bar back. The
next loop number shall be assigned to the adjacent right turn radius loop at the stop bar followed by
the next radius loop away from the stop bar until all radius loops are numbered. The advanced
loop(s) that may be in the same lane as stop bar loops shall be numbered in order following the right
turn radius loops. All loops connected to one lead-in shall be listed in order separated by a comma.
For micro -loops, these numbers shall have a 'M' directly in front of them with no space between the
M and the numbers (ie: M1,2,3 or M4).
Count Output M:
Each lane shall have a count output. The Count Output (#) shall be labeled identically to
controller detector channel that is assigned to log the count pulse. Count output of counting
amplifiers is typically 2 channels higher than the corresponding Presence output, unless an auxiliary
BIU is installed. Where logging the call output of a standard (non -counting) amplifier, the count
output (#) shall be blank. No number shall be used more than once for combined loop and count
output numbers.

FIBER OPTICS SPECIFICATIONS
GENERAL REQUIREMENTS
Materials and Equipment: Materials and equipment shall be the standard products of a manufacturer
regularly engaged in the manufacture of the products. All materials and equipment furnished shall
be new of first quality, of the latest design and be completely free from defects in material and poor
workmanship. All like pieces of equipment shall be of the same type and manufacturer to assure
uniformity, interchangeability of components, single responsibility, and most satisfactory service.
Contractor Qualifications: The fiber optic cable installation shall be supervised by trained and
experienced personnel. The cable terminations shall be done by qualified technicians.
Code Requirements: The Fiber Optic installation shall be in accordance with or exceed all minimal
requirements of State codes, National codes, and manufacturer codes as applicable.
Miscellaneous Equipment: The Contractor shall furnish and install all necessary miscellaneous
equipment to make a complete and operating installation in accordance with the plans, standard
sheets, standard specifications, special provisions, and accepted good practice of the industry.
RECORDS
Cable Records: All cable records shall be the responsibility of the Contractor. Detailed records shall
be kept of fiber optic cable reference locations between termination points. The following is a brief
example:

The reference points shall include: connection points in controllers, base and top of conduit risers,
handholes, junctions with intermediate poles, centerlines of named streets and roads, and any other
well defined points as directed.
MATERIALS AND COMPONENTS
Cable General: The fiber optic cable shall contain 6 stranded multimode, graded index, optical fibers
with a minimum of one non-metallic central strength member. The cable shall be loose tube, all
dielectric construction, suitable for outdoor use in conduit or on aerial supports. Each individual
fiber shall be 62.5/125 micron diameter (core/clad), and each fiber shall be individually encased in its
own gel -filled color -coded buffer. The fiber optic cable shall be constructed with Kevlar braid and
outer polyethylene jackets as a minimum. IF an inner jacket is used it shall be PVC. Maximum
attenuation of the cable shall be 4.0 dB/Km nominal, measured at room temperature at 850 nm. The
bandwidth shall not be less that 160 MHz/Km also at 850 nm. Each fiber shall be continuous with no
factory splices except for jointing standard length cables to form longer, continuous jacketed cable to
fit installation requirements. The cable shall have standard nylon rip cords or distinctively different
color Kevlar rip cords (like red).
The cable shall meet the generic requirements for optical fiber and optical fiber cable per Bellcore
Technical Reference TR-TSY-000020.
The exterior of the polyethylene outer cable jacket shall be stenciled so that every 5w meter on each
reel is marked with a number. The 5th meter of each reel shall be marked with a 5, the 10th meter
marked with a 10, and so on until the end of the reel. The stencil shall be applied to the outer jacket
using permanent ink and shall be permanently engraved into the jacket to provide long-lasting
readability.
Warning Tape: Plastic marking tape shall be acid and alkali resistant polyethylene film, 3 inches (77
mm) wide with minimum thickness of 0.004 of an inch (0.102 mm). The tape shall be of a type
specifically manufactured for marking and locating underground utilities. Tape color shall be orange
and shall bear a continuous printed inscription "CAUTION -FIBER OPTIC CABLE".
Cleaving Tool: The cleaving device shall be a T&B Cleaving Tool - catalog no. 92208 or approved
equal.
Warning Tape: Where conduit is installed in an open trenched warning tape shall be installed
directly above the conduit at a depth of twelve inches below finished grade unless otherwise
approved by the Engineer.
Cable: The Fiber Optic cable shall be installed in continuous runs as indicated on the plans. Splices
shall be allowed only in controller cabinets between the end of the fiber optic interconnect cable and a
factory manufactured pigtail assembly.
Cable Installed in Ducts and Conduits: A suitable cable feeder guide shall be used between the cable
reel and the face of the duct and conduit to protect the cable and guide it into the duct off the reel. It
shall be carefully inspected for jacket defects. If defects are noticed, the pulling operation shall be
stopped immediately and the Engineer notified. Precautions shall be taken during installation to
prevent the cable from being kinked or crushed. A pulling eye shall be attached to the cable and used
to pull the cable through the duct and conduit system. A pulling swivel shall be used to eliminate

twisting of the cable. As the cable is played off the reel into the cable feeder guide, it shall be
sufficiently lubricated with a type of lubricant recommended by the cable manufacturer. A
dynamometer or break away pulling swing shall be used to ensure that the pulling line tension does
not exceed the installation tension value specified by the cable manufacturer. The mechanical stress
placed on a cable during installation shall not be such that the cable is twisted or stretched. The
pulling of cable shall be hand assisted at each controller cabinet. The cable shall not be crushed,
kinked or forced around a sharp corner. If a lubricant is used it shall be of water based type and
approved by the cable manufacturer. Sufficient slack shall be left at each end of the cable to allow
proper cable termination. Also a small slack of cable shall be left in each handhole between
intersections. At the cabinet controller shall be two (2) meters minimum storage at the handholes
adjacent to the cabinet controller six (6) meters storage and at the top of each conduit riser six (6)
meters after the support cable is stripped when figure 8 cable is used.
For storage of additional slack cable adjacent to conduit riser the Contractor shall use Fiber Optic
Strand Storage Unit to store and protect the fiber optics at the top of the risers. The support cable
shall be terminated at the riser wood pole.
Storage of additional slack cable in handholes shall be coiled. The slack coils shall be bound at a
minimum of 3 points around the coil perimeter and supported in their static storage positions.
Storage of additional slack cable adjacent to conduit risers and support poles shall be as detailed on
the following page. At each controller cabinet and handhole the cable shall be visible marked/tagged
as "CAUTION -FIBER OPTIC CABLE". Maximum length of cable pulling tensions shall not exceed
the cable manufacturer's recommendations.
At each handhole and cabinet, the fiber optic cable shall be painted for a length of 12 inches (305 mm)
with 2 coats of long lasting and highly visible paint. The color shall be unique to each roadway as
approved by the Engineer. For example, the cable for the major east -west street may be painted red,
the cable for the major north -south street may be painted yellow, and the cable for the minor east -
west street may be painted blue.
Cable Installed on Aerial Spans: The fiber optic cable shall be figure 8 cable. All existing interconnect
cable in risers, conduits, handholes, and controllers shall be removed. All existing interconnect cable
not used to support the new fiber optic cable shall be removed. All existing state owned poles used
exclusively for existing interconnect shall be removed if the new fiber optic cable does not utilize the
pole. Existing conduit risers designated for re -use with fiber optic interconnect shall have the existing
weatherhead removed. The lashing wire shall be either a dielectric or stainless steel lashing filament,
and shall be securely tied off when terminated near each support pole. If the fiber optic cable is figure
8, the messenger cable portion shall be removed from the controller cabinet to the top of the nearest
conduit riser. Removal of the messenger portion of the figure 8 fiber optic cable shall be performed
with no additional payment. All fiber optic cable runs damaged as a result of the removal of the
messenger shall be replaced, free of splices, with no additional payment.
Placement of Cable into Conduit Risers: The Contractor shall provide conduit risers as indicated on
the plans. Kellum grips and/or other hanger devices shall be used to support the vertical drop of
cable and to prevent any possible kinking of the cable after installation. The top of risers shall have a
hexnut type watertight service entrance connector with an oval shaped grommet. The grommet shall
be either neoprene or rubber. The voids between the fiber optic cables(s) and the grommet shall be
sealed with silicone.

Minimum Bend Radius: For static storage, the cable shall not be bent at any location to less then ten
times the diameter of the cable outside diameter or as recommended by the manufacturer. During
installation, the cable shall not be bent at any location to less than twenty times the diameter of the
cable outside diameter or as recommended by the manufacturer.
Prior to the Fiber Optic Cable Installation: Prior to the installation of the cable, the manufacturer shall
submit to the Contractor a factory -performed Optical Time Domain Reflectometer (OTDR) trace result
for each reel of cable. The Contractor shall in turn submit the trace results to the Engineer for
approval. The Engineer shall inspect the test results for any unacceptable results. Any unacceptable
reels shall be replaced with no additional payment.
After the Fiber Optic Cable Installation: Each section of the cable shall be tested for continuity and
attenuations as a minimum. If the attenuation is found not to be within the acceptable nominal
values, the Contractor shall use an Optical Time Domain Reflectometer (OTDR) to locate points of
localized loss caused by bends or kinks and try to relax these bends or kinks. If this is not successful
the Contractor shall replace the damaged section of the cable with no additional payment. Splices
will not be allowed to repair the damaged section. The Contractor shall provide the Engineer with a
written report showing all the values measured during these tests.
Cable Termination: Terminations shall be made using the method recommended by the connector
manufacturer. If breakout kits are used, they shall terminate each fiber in the controller cabinet using
buffer tubing and jacketed material. If mechanical splices are used, they shall terminate each fiber in
the controller cabinet using the splice tray.
INTERCONNECT TRACE WIRE
A 14 gauge copper trace wire shall be installed with all underground fiber optic interconnect cable.
This wire shall be in accordance with 913.15 (f) 4. The trace wire shall be continuous from intersection
to intersection, end in the controller cabinet at each end. Installing this cable will be paid under the
item, "Fiber Optic Cable'.

CONFLICT MONITOR CHANGEOUT PROCEDURE
Conflict Monitors use NEMA inputs and are interchangeable among manufacturers.
TCT LSM12 or LNM12 series monitors must be installed at intersections in TCT closed loop
systems for proper operation. TCT LNM12 series monitors must be installed at intersections
equipped with PRE-EMPTION. These monitors should normally be reserved for such
intersections due to their limited availability. Options on this series of monitor shall not be
used unless directed by the Engineer or marked on the timing sheet.
1. MINIMUM FLASH TIME set to 10 seconds. The SERIAL and MODEL numbers
shall be recorded on the Work Activity Report.
2. The intersection should be green for the phase(s) that would flash yellow. If the
Technician does not know which Phases flash yellow, he can refer to the
Preferentiality listing on the Intersection listing.
3. The 10 Amp breaker should be turned 'off. This puts the signal on flash and
removes the power from the controller and the conflict monitor.
4. The old conflict monitor's MS connector(s) shall be loosened and removed. The
conflict monitor program card must be removed and installed in the replacement
monitor.
5. The new conflict monitor can then be installed in the cabinet and the MS
connector(s) connected to the new monitor.
6. The 10 Amp breaker can then be turned back on. The controller should reinitialize
in the preferential phase. The conflict monitor may have a preexisting "conflict"
registered in it's memory and may require a "reset" to get the signal to go to signal.
7. If the conflict monitor reinitializes in minor phases, there are other problems not
associated with the conflict monitor that should be checked out.
S. Record the SERIAL and MODEL numbers of the removed monitor on the Work
Activity Report, sign cabinet card showing the time, date and tech performing the
conflict monitor exchange.
9. Routine Conflict Monitor change outs will be performed within the month listed in
Exhibit "B".
10. The Annual Monitor Change -out will be performed during the months of October
