The St. Joseph County Health Department is a highly qualified applicant with the networks in place needed to fulfill all grant requirements. When the SJCHD is chosen by the Indiana State Department of Health as one of the two pre-selected communities, our office would be willing to supply an official Letter of Support (LOS) for the full grant application. Jefferson Blvd. South Bend, IN 46601 The St. Joseph Cowity Health Department (SJCHD) is applying to the Indiana State Department of Health (ISDH) to be considered on their application for the Communities Putting Prevention to Work funding opportunity through the American Recovery and Reinvestment Act of 2009 (FO# CDC-RFA-DP09-912ARRA09, CFDA# 93.724). The Recovery Act includes $650 million for evidence-based clinical and community-based prevention and wellness strategies to reduce chronic disease rates, with $1._5-4 million potentially available for our area. ISDH is eligible to apply on behalf of two small cities/rural areas. We would like to be included on State's application in order to bring these potential funds to South Bend. In order to be considered by the State as one of the two cities, we need to prove that we are a highly qualified applicant with an extensive support network in place. We are not yet writing the grant; we are only applying to State to be considered for their application. In our Letter of Intent (LOI) due to State on October 20, 2009, we must include brief, yet official notes in support of the potential application from city planners and developers. We would Greatly appreciate a quick note to show your support of our application to State. An official Letter of Support may be required if we are accepted as one of the two applicants.
