If any of the above quotes are missing or not signed, either in part or in whole, by the client, ENS Group reserves the right to reject the Statement of Work or amend it in order to meet all of the requirements outlined throughout this document. Additional quotes or amendments may add to the cost of labor or materials required for successful implementation. However, ENS and City of South Bend reserve the right to add to the scope, schedule, or cost of the project by following the processes identified in the Change Management Section of this document: Discovery High-level discovery work was completed during the quoting process for the project. However, further discovery work will be executed during planning sessions to gather client requirements. Plan Development A key factor in success of the project is to develop a detailed plan for the project. This plan will be used by ENS technicians to properly configure the system to function in a way that is aligned with the operational goals & objectives of City of South Bend. Knowledge transfer regarding features, functionality, and user options is provided to City of South Bend during and upon completion of the executing phase of this project. CORE SWITCH DEPLOMENT PROJECT SCOPE OVERVIEW The City of South Bend LAN -WAN Implementation project, as described in this Project Management Plan, will be defined as successful after the following milestones are met: • Discovery & Planning • Equipment Installation and Configuration • Cutover from existing systems to new systems • Administrating training and knowledge transfer • Final Documentation • Project Close -Out EQUIPMENT INSTALLATION AND CONFIGURATION The following identifies and describes the planned installation and configuration of various known high level items. General Guidelines • All devices will be configured using passwords unique to City of South Bend • ENS will follow best practice procedures for securing the network devices Local Area Network ENS will work to install & configure the new systems in accordance with planning meeting discussions and the overall project plan. ENS will coordinate with the City of South Bend staff in order to smoothly transition from the old systems to the new ones, minimizing disruptions and downtime. ENS technicians will test and resolve/troubleshoot issues related to the activities in this SOW to ensure proper operation of the system. Network installation steps will include: • Install & configure Cisco switches 0 2 Cisco 4500X switches will be installed in the City of South Bend's rack in Data Realty • Switches will be "stacked" for full redundancy 0 2 Cisco 4500X switches will be installed in the City of South Bend's Police Department • Switches will be "stacked" for full redundancy o Multiple 1G copper modules are included for to provide local connectivity, any other connectivity may require the purchase of additional modules 0 3 Cisco 3850 switches are provided for future connectivity in the City County Building when the 6509 chassis is removed Statement of Work for City of South Bend 4 Wide Area Network The installation of the systems will take place at City of South Bend's main location and branches as appropriate. ENS will work to install & configure the new systems in accordance with planning meeting discussions and the overall project plan. ENS will coordinate with the City of South Bend staff in order to smoothly transition from the old systems to the new ones, minimizing disruptions and downtime. ENS technicians will test and resolve/troubleshoot issues related to the activities in this SOW to ensure proper operation of the system. Network installation steps will include: • Uplink Connectivity o The 450OX switches being installed in the City of South Bend's datacenter will provide 1OG connectivity to multiple locations with redundant single mode fiber connections • 6509 in the City County Building using new X2 1OG modules • 450OX in the Police Department • Modules for both of these locations are appropriate for up to lOkm This will provide the core connectivity for future network moves and connectivity • Additional modules may be needed as the network is expanded • Network Design o The ENS engineer with review the current status of the network and the plans for the immediate future and create the design steps to get there. o This will include documenting additional hardware that will be required, updated network map for future connectivity and roadmap for network changes o Based on the current information about one week of time will be required for the detailed design CISCO ASA AND FIREPOWER DEPLOYMENT PROJECT SCOPE OVERVIEW EQUIPMENT IMPLEMENTATION The implementation of the new security equipment will take place at City of South Bend's CCB and Police Department facilities. ENS will work to implement the new equipment in accordance with planning meeting discussions and the overall project plan. If applicable, ENS will coordinate with City of South Bend staff in order to smoothly transition from the old equipment to the new, minimizing disruptions and downtime. ENS personnel will test and troubleshoot issues related to the activities covered in the scope for this engagement to ensure proper operationof the new equipment. Implementation steps will include: ENS Group will install and configure two (2) Cisco ASA5525-X Adaptive Security Appliances to replace the existing ASASS20S at the Internet perimeter. The new ASA5525-Xs will be licensed for the following features: o FirePOWER Intrusion Prevention System (IPS) with a 3- or 5-year subscription, depending on option purchased o AnyConnect Secure Mobility Client with support for up to 50 simultaneous connections from Windows, Macintosh, Linux, iOS, and/or Android endpoints with a 3- or 5-year subscription, depending on option purchased The ASA5525-X platform is rated for 255 Mbps throughput with FirePOWER IPS enabled. If other FirePOWER features (e.g., AMP for Networks and/or URL filtering) are added at a later time, supported throughput will decrease. An ASA5525-X running all FirePOWER features is rated for 167.5 Mbps. Adding SSL decryption reduces total throughput to 33.5 Mbps. ENS Group will install and configure one (1) instance of the Cisco FirePOWER Management Center as a virtual appliance on a VMware ESX host provided by City of South Bend. Minimum requirements for this virtual appliance include: 0 8 GB RAM Statement of Work for City of South Bend 5 o 250 GB disk space o 4 virtual CPU o 1 virtual NIC So that the aforementioned FirePOWER Management Center can map users' IP addresses to their corresponding Active Directory usernames, ENS Group will install and configure the Cisco FirePOWER User Agent on two (2) Active Directory domain -member servers. The Agent software must be configured with a service account with sufficient permissions to read login and logout events from the domain controllers' security logs. USER TRAINING AND KNOWLEDGE TRANSFER Training - System Administrator. Limited system administrator training is provided with this project. City of South Bend staff will be shown the fundamentals of how to manage the new system, including: • Cisco FirePOWER Management Center configuration, operation, and maintenance CISCO AMP DEPLOYMENT PROJECT SCOPE OVERVIEW SOFTWARE IMPLEMENTATION The implementation of the new security software will take place at various City of South Bend facilities. ENS will work to implement the new software in accordance with planning meeting discussions and the overall project plan. If applicable, ENS will coordinate with City of South Bend staff in order to smoothly transition from the old software to the new, minimizing disruptions and downtime. ENS personnel will test and troubleshoot issues related to the activities covered in the scope for this engagement to ensure proper operation of the new software. Implementation steps will include: • ENS Group will deploy the Cisco Advanced Malware Protection (AMP) for Endpoints agent onto up to 800 Active Directory domain -member computers. This deployment is typically executed via Group Policy. The Agents will be licensed for either a 3- or 5-year subscription term, depending on option purchased. USER TRAINING AND KNOWLEDGE TRANSFER Training - System Administrator. Limited system administrator training is provided with this project. City of South Bend staff will be shown the fundamentals of how to manage the new system, including: • Cisco Advanced Malware Protection (AMP) for Endpoints configuration, operation, and maintenance CITY OF SOUTH BEND RESPONSIBILITIES General • City of South Bend will provide adequate rack space, power (on UPS), ventilation, cabling, and cable management required for installation and implementation of equipment. • City of South Bend will provide access to server rooms and other required spaces for ENS technicians to perform their work. • City of South Bend will provide secure storage & staging areas as needed for new hardware as needed. • City of South Bend will ensure all internal wiring for voice or data is terminated properly and clearly labeled. • City of South Bend will provide timely response to requests and documents from ENS relating to all aspects of the project and project management processes, including and pertaining to such items change orders, open action item reports, and project close out reports. If the City of South Bend does not acknowledge or respond in Statement of Work for City of South Bend - 6 writing to these project documents within S business days, the Client agrees that the project documents provided by ENS are considered approved and accepted by the Client. OUT OF SCOPE ISSUES During the design phase of this project, ENS and City of South Bend have mutually determined that certain items are not included, and should therefore be considered out -of -scope for this particular fixed -fee project. If the client seeks to pursue these options, ENS may certainly be used to do so. However, the fixed project fee for this engagement does not include the options/issues listed below: Out of Scone for Cisco Infrastructure ENS Group has identified that certain items are not included, and should therefore be considered out -of -scope for this particular fixed -fee project. If the client seeks to pursue these options, ENS may certainly be used to do so. However, the fixed project fee for this engagement does not include the options/issues listed below: 1. Networking on Equipment other than the Equipment Purchased through ENS for this project. This project includes the installation and updates to anew and an existing networking infrastructure. No VLAN, VPN, Routing, DMZ creation or changes are included other than those specified in the Installation section above. 2. Racks, KVM, Ventilation, and Power. Equipment Racks (Telco and/or Server), KVM Switches, Air Conditioning, UPS devices, PDUs, associated cables/cords, or electrical hard wiring is not included as part of this project. This includes but is not limited to HVAC, duct work, circuit breakers, breaker panels, conduit, wiring for power, outlets, backup power generators, etc. 3. Tech Support. Any and all end -user technical support is not included as part of this project. 4. Training. Limited training and knowledge transfer may be provided as part of this project as defined specifically in the Project Scope section of this agreement. However, in-depth training related to vendor specific items is considered out of scope. S. Bandwidth, Circuits, and Troubleshooting related to Internet and/or WAN connectivity. No bandwidth, WAN circuits, or Telco lines are included in the project, nor are any ISP, telecommunication, and/or broadband vendor specific issues and troubleshooting for any data, PRI trunks, and/or phone services included in this project. 6. Troubleshooting related to Internet connectivity. Any ISP specific issues or troubleshooting are not included within the scope of this project. 7. Troubleshooting related to LAN connectivity. Any LAN network specific issues or troubleshooting are not included within the scope of this project. B. Cabling. Structured Cabling, Patch Panels, and/or Ethernet Cables (copper or fiber) are not included as part of this project. ENS will provide the specifications for the cabling required as needed; however, the cabling and cables (structured or patch) themselves are not included within the scope of this project. The cables (to ENS and vendor specifications) may be purchased by City of South Bend, either from ENS or another vendor. ENS will arrange for a preferred cabling vendor to provide a quote to meet our cabling recommendations if requested. Any work for tracing, toning or the labeling of cables, other than those specifically called out in the Installation and Configuration section, will be outside of the scope of this project and will require a change request. 9. ALL OTHER. Unless specifically stated or documented, no other services are included in this fixed -fee project. Statement of Work for City of South Bend 7 PROJECT COMPLETION The project will be deemed complete when all deliverables are accepted and test plan is completed and accepted by both ENS Group and City of South Bend • Once project work has started, weekly progress meetings will be held between City of South Bend and ENS staff (project manager, lead tech, account manager, etc...) to ensure that the project is staying on track. This will be done on an as needed basis as defined by stakeholders and project manager. In regards to these meetings, we ask that any questions that come up during the technical installation be saved and brought up at these meetings and not asked directly to the technician on site. This will ensure that everyone possess the proper information to ensure that the proper resolution is made to any concerns. • If there is an item that City of South Bend asks ENS to skip during the project, those item(s) will also need to be documented & initialed, noting a formal request not to complete that task(s). Skipping any such task(s) are not guaranteed to expedite project completion. Likewise, skipping any previously agreed upon tasks will not reduce the agreed upon payment due for the project. Once the implementation is completed, all deliverables are completed, punch list items identified during the Integration Test Period are completed; ENS will send City of South Bend the final closeout document and the project will be considered complete unless City of South Bend responds in writing within S business days otherwise. • Upon successful completion of all project work, the ENS team will hold an internal Lessons Learned meeting to go overall aspects of the project. At this time our Network Operations Center (or NOC) will be brought into ensure the proper level of knowledge transfer is made from the project technician(s) to ensure that all needed information is added to ENS's documentation library. By doing this, our goal is to have a vast and documented knowledge of your network so that when ENS is engaged in the future for any questions/issues, ENS will have the familiarity of your network, thus speeding up the discovery process when your organization needs assistance. • At the project closeout meeting ENS will provide City of South Bend full administrative access to the devices configured. ENS will maintain sole access to these systems until the close of the project. • ENS and City of South Bend agree that once the project commences, all aspects of the project's execution phase will happen contiguously to be completed within 60 days. City of South Bend agrees that the full payment will not be delayed due to any delays created by the City of South Bend. • ENS Group will notify the client that all deliverables outlined in this SOW have been implemented at which time a S business day period, known as the Integration Test Period, will commence during which issues discovered can be listed on an exceptions document provided by ENS. After the Integration Test Period has completed, the project will be considered closed with exception of the items mutually agreed in writing by both parties on the exception list. Items not listed on the exceptions document will be billed at the prevailing hourly rate. Statement of Work for City of South Bend 8 PROJECT COSTS Fixed Fee Pricing City of South Bend will be billed on a Fixed Fee basis, and invoice(s) will reflect the total of all fees due and payable under this SOW. This project's fixed fee amount is $24,000, which includes all fees associated with project completion (i.e. technical expertise, project management, etc...). However this excludes all hardware and software purchases, which will be addressed as needed in separate quote documents. This fixed fee rate applies to scheduled services that are performed during Standard Business Hours (meaning 8:00 a.m. to 5:00 p.m. local time, Monday through Friday, excluding holidays). Up to 8 hours of time outside Standard Business Hours is included for replacing active devices and the cutover. The rates above only apply to Services specified in this SOW as it may be amended by one or more Change Order(s). Note that the project implementation services will stop, be delayed, or rescheduled to later dates at the discretion of ENS Group if milestone project payments are not received by the Client according to the payment terms associated with the milestone progress payment invoices. For any other costs related information please refer to your previously signed Terms of Service Agreement (TOSA). If a copy of the TOSA is unavailable, please contact your Account Manager for a copy of the signed agreement. Statement of Work for City of South Bend 9 CONTACT PERSON(S) Each party will appoint a person to act as that party's point of contact as the time for performance nears, and will communicate that person's name and information to the other Party's Contact Person. The client contact person is authorized to approve materials and services provided by ENS, and ENS may rely on the decisions and approvals made by the client contact person (except that ENS understands that City of South Bend may require a different person to sign any Change Orders amending this SOW). The client contact person will manage all communications with ENS, and when services are performed at a Customer -Designated Location, the client contact person will be present or available. PAYMENT TERMS A project closeout meeting may occur at an agreed upon time immediately after the Integration Test Period. Depending on the size and scope of the project, the project closeout meeting may be in -person, web -based, a telephone conference call, or an email exchange at the discretion of ENS Group. At this time any open and in scope items will be identified and agreed upon to be completed. However, with the project closeout meeting the project will be considered closed unless otherwise duly agreed in writing by ENS and City of South Bend, City of South Bend will pay 100% of the outstanding invoices containing amounts authorized by the Fixed Fee Pricing section of this SOW in full within 15 days of the project closeout meeting or according to progress payments outlined in this SOW. EXPIRATION AND TERMINATION This SOW expires and will be of no force or effect unless it is signed by City of South Bend, transferred in its entirety to ENS so that it is received within thirty (30) days from the date written on its cover page, and then signed by the ENS Group. CHANGE ORDERS Any change orders that affect the scope, schedule, or cost of this project plan will be immediately communicated by requesting party to all parties involved. In these cases a Change Request Form will be provided. A sample Change Request Form is included as Appendix A. Upon receipt of a completed Change Request Form, City of South Bend representatives will sign off to approve the change requested. Only after a signed copy of the Change Request Form has been processed will the requested change be executed. This process will allow ENS to provide their clients with the most efficient projects possible which in turn will result in projects being delivered on -time, within budget, and to quality standards. Statement of Work for City of South Bend 10 16. ENS can perform additional services that are not defined in this proposal on a separately billed basis. These services include, but are not limited to, server side maintenance, workstation deployment, infrastructure consulting, and Internet connection consulting. For pricing information, contact your ENS sales representative. 17. All work listed above will be performed during normal business hours (Monday - Friday, 8:OOAM - 5:OOPM, excluding holidays). All work performed outside these hours will be billed at 1.5 times the quoted fee. The client will be notified before any additional billing is incurred. The liability of ENS Group with respect to any transaction, servicing, contract ofsale or anything done in connection therewith, whether in contract; tort, negligence, or under any warranty or otherwise shall not exceed the price of the goods and services supplied and invoiced by ENS Group for that individual transaction; and Customer agrees that Customer shall not seek monies or damages beyond the price ofgoods and services supplied and invoiced by ENS Group. ENS Group shall not be liable forspecial, indirect incidental or consequential damages,- and Customer agrees that Customer shall not seek special, indirect, incidental or consequential damages. ENS Group shall not be liable for loss or damages caused by contingencies beyond its control including, but not limited to, strikes, fires, acts of God or other accidents; and Customer agrees that Customer shall not seek to recoverfrom ENS Group for loss or damages caused by contingencies beyond ENSGroup's control including, but not limited to, strikes, fires, acts of God or other accidents. Customer agrees that ENS Group shall not be responsiblefor lost company data or network downtime due to hardware orsoftware failure of any kind and acknowledges that it is the responsibility of Customer to insure data backups are being performed. Customer understands that crashes and/or disruptions during work of the type set -forth in this agreement are possible and shall not hold ENS Group responsible for any of the effects, director indirect consequential orincidentaL GENERALITIES: 1. The existing environment must be fully operational and running before the start of this project. 2. ENS will not be held liable for lost data due to the past or future failure of any hardware or software that performs backups. 3. Work on this project will not begin unless all related hardware and software has been delivered and is available to the installation technicians. 4. Accurate IP address and HOST/DOMAIN name information provided by the ISP must be provided to a representative of ENS prior to installation. 5. All telecommunications line information provided by the line carrier and/or ISP must be provided to a representative of ENS prior to installation. 6. Correct telephone numbers and point of contact information for the line carrier and ISP must be provided to a representative of ENS prior to installation. 7. The client must provide all licenses, software, and documentation required for configuration. 8. This proposal does not include the installation or configuration of any equipment except for that specifically indicated in this Project Management Plan. 9. Unless specifically incorporated into this Project Management Plan, the client is responsible for the operation and configuration of any routers. 10. Additional time spent working with the ISP and/or Telco vendors to solve Internet or WAN -related problems outside of the client's local office are tracked and billed on an hourly basis. 11. The telecommunication line must be already installed to the DMARC and tested by the line carrier prior to the beginning of this project. if required, the DMARC must be extended to the equipment room prior to the beginning of this project. 12. Appropriate utility power must be available and tested for all networking equipment prior to the beginning of this project. The power circuits must be active and have the appropriate power receptacles installed for the network equipment. 13. The client must provide full access to workstations upon request. 14. The client will provide full access and administrative rights to all servers as required. 15. ENS will not be responsible for ensuring the proper operation of any 3rd party applications installed on the server during or after the completion of this project.
