Klosinski, Assistant City Engineer <br />SUBJECT: Agenda Request Form — Amendment to Professional Services Agreement <br />Project No. 114-053: Wastewater Treatment Plant Secondary Treatment <br />Improvements <br />DATE: July 19, 2016 <br />The Department of Environmental Services (DES) is requesting the Board of Public Works <br />approval of an Amendment to the Professional Services Agreement for Project No. 114-053 <br />Wastewater Treatment Plant Secondary Treatment Improvements with Arcadis U.S. <br />This Amendment includes the addition of design services to design a slab for the Aeration <br />Gallery Tunnel which will carry vehicle traffic and extend walkway space, and detail safety <br />handrail improvements along the Aeration Gallery walkway. Refer to Amendment for details. <br />In addition to the above design services, the Amendment also provides compensation to the <br />Engineers of Record for the review of substitution requests made by the General Contractor. <br />The substitution requests were evaluated at the request of the General Contractor and the <br />Division of Utilities. It should be noted that some of the costs associated with this work may be <br />applied as a deductive change order to the Construction Contract which requires the General <br />Contractor to cover the costs for the review of Substitution Requests. This will be addressed <br />with the General Contractor through a future Change Order. Refer to Amendment for details. <br />The increase to the subject amendment will increase the contract by $37,620.00. <br />The primary funding sources for this work will be Utilities, Wastewater Engineering (Account <br />Number 641-0630-793-31-02). If there are any questions regarding the subject project, please <br />contact Al Greek (Ext. 4210) or Jacob M. Klosinski (Ext. 9496) at the Division of Utilities. <br />
