REGULAR MEETING JUNE 14, 2016 201 <br />No Bid Bond was required. <br />Local Business Preference submitted. <br />HARTMAN JANITORIAL SUPPLY <br />824 E. Troy Avenue <br />Indianapolis, IN 46203 <br />Bid was signed by: Kris Feldhake <br />Contractor's Non -Collusion Affidavit, Non -Debarment Affidavit, Certification Regarding — <br />Investment with Iran, Employment Eligibility Verification, Non -Discrimination <br />Commitment for Contractors and Certification of Use of United States Steel Products or <br />Foundry Products was submitted. <br />No Bid Bond was required. <br />Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the above bids were <br />referred to Central Services for a bid tab and review and recommendation. <br />OPENING OF PROPOSALS — LAWN MAINTENANCE & MENTORSHIP PROGRAM <br />(EDIT) <br />Mr. Gilot advised that this was the date set for the receiving and opening of sealed Proposals for <br />the above referenced project. The name of the following company submitting the sole proposal <br />was read into the record: <br />GREATER IMPACT LAWN CARE <br />615 Cushing St. <br />South Bend, IN 466t6 <br />Submitted by: Kory Lantz <br />Mr. Gilot stated the proposal will be reviewed and scored based on a matrix system, by a team _ <br />made up of City representatives. Upon a motion made by Ms. Maradik, seconded by Mr. Relos <br />and carried, the above proposal was referred to Community Investment and Engineering for <br />review and recommendation. <br />EXTEND BID AWARD — SIX (61 MORE OR LESS 2016 OR NEWER 3/4 TON FOUR <br />WHEEL DRIVE PICK UP TRUCKS (SPEC K) (PARKS DEPARTMENT CAPITAL) <br />Mr. Jeffrey Hudak, Central Services, advised the Board that on February 23, 2016, bids were <br />received and opened for the above referenced vehicles. On March 22, 2016 the Board awarded <br />the bid to Gates Chevrolet, 636 West McKinley Avenue, Mishawaka, Indiana 46546 in unit <br />prices. Mr. Hudak requested that the Board extend the award for two (2) additional trucks in the <br />unit price amount of $29,054.00 each for a total of $58,108.00. Mr. John Martinez, Park <br />Department, stated they were able to find additional funds to add the CNG option as requested <br />by the Board at their Agenda Review Meeting. Therefore, Ms. Maradik made a motion that the <br />recommendation be accepted and the bid be awarded as outlined above. Mr. Relos seconded the <br />motion, which carried. <br />AWARD BID — THIRTY-NINE (39) MORE OR LESS 2016 OR NEWER, ALL WHEEL <br />DRIVE POLICE PATROL VEHICLES SPECS SUBJECT TO POLICE DEPARTMENT <br />CAPITAL LEASE FUNDING) <br />Mr. Jeffrey Hudak, Central Services, advised the Board that on April 26, 2016, bids were <br />received and opened for the above referenced vehicles. After reviewing those bids, Mr. Hudak <br />recommended that the Board award the contract to the lowest responsive and responsible bidder <br />Bloomington Ford, Inc., 2200 South Walnut Street, Bloomington, Indiana 47401, in the base bid <br />unit price amount of $23,961.35, plus Alternate A for twenty-seven (27) vehicles, for a total not <br />to exceed 5979,420.65. Therefore, Ms. Maradik made a motion that the recommendation be <br />accepted and the bid be awarded as outlined above. Mr. Relos seconded the motion, which <br />carried. <br />
