REGULAR MEETING <br />yi s .. .. <br />RESOLUTION NO. 31-2006 <br />JUNE 12, 2006 ~ ~ 9 <br />A RESOLUTION OF THE BOARD OF PUBLIC WORKS OF THE CITY OF <br />SOUTH BEND WRITING OFF AS UNCOLLECTIBLE $13.500 IN FALSE ALARM <br />FINES <br />WHEREAS, the City of South Bend, pursuant to Chapter 12, et seq., of the <br />Municipal Code regulates alarm systems; and <br />WHEREAS, the Ordinance imposes fines for violating of the same; and <br />' WHEREAS, upon a fourth violation a $100.00 fine is imposed and a $200.00 fine <br />is imposed for the fifth and each false alarm thereafter that is committed in a calendar <br />year; and <br />WHEREAS, fines imposed in the calendar years of 2000-2002 cannot be <br />collected because the statute of limitations expired; and <br />WHEREAS, violations that occurred in 2003 cannot be enforced because the <br />notification provisions of the .Ordinance were not followed; and <br />WHEREAS, the Indiana State Board of Accounts has instructed the City to write <br />off all uncollectible accounts after every attempt to collect them has been made; and <br />WHEREAS, the violations occurring in calendar years 2000-2003 are in fact <br />uncollectible; and <br />' WHEREAS, the~`City of South Bend by and through its Board of Public Works, <br />desires to write off the accounts listed on the attached documents. <br />NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF PUBLIC <br />WORKS OF THE CITY OF SOUTH BEND, that the attached documents totaling <br />$13,500.00 be written off the accounts receivable~of, the City of South Bend as <br />uncollectible. <br />DATED June 12, 2006. Jacob, Clerk <br />ADAPT RESOLUTION~NO. 32-2006: <br />Upon a motion made by Mr. Gilot, seconded by Mr. Tnks and carried, the following <br />Resolution was adopted by the Board of Public Works, subject to review by the City <br />Controller: <br />RESOLUTION NO. 32-2006 <br />WRITE OFF OF AMBULANCE SERVICES OUTSTANDING ACCOUNTS FOR <br />YEAR 2003 <br />WHEREAS, the City of South Bend, through its Emergency Medical Services <br />Department, charged the persons listed on the attached documents for ambulance <br />services; and <br />WHEREAS, the charges have not been paid to the City of South Bend after at <br />least three notices were sent to the person who received the services; and <br />,, <br />WHEREAS, CBM Accounts Service, Inc. has made every attempt to collect the <br />aforementioned charges, and has not been successful; and <br />
