g ~ ~ '~ <br />ADDENDUM TO CONTRACT <br />BY AND BETWEEN <br />REDEVELOPMENT COMMISSION, CITY OF SOUTH BEND, INDIANA <br />AND <br />SBC GLOBAL SERVICES, INC., <br />DBA AT&T GLOBAL SERVICES <br />THIS ADDENDUM is entered into this day of September, 2009, by <br />and between the City of South Bend, Indiana, Department of Redevelopment, acting by <br />and through its Redevelopment Commission, having its offices at 1200 County-City <br />Building, 227 West Jefferson Boulevard, South Bend, Indiana 46601 ("City") and SBC <br />Global Services, Inc. dba AT&T Global Services, a Delaware Corporation, with offices at <br />One AT&T Plaza, Dallas, Texas 75202 ("Contractor"). <br />The purpose of this Addendum is to modify, delete, or amend certain terms and <br />conditions set forth in the attached contract prepared by Contractor ("Form Contract"). <br />This Addendum and the Form Contract are incorporated into each other and, when read <br />together, shall constitute one integrated document and referenced hereafter as the <br />"Contract". Any inconsistency, conflict or ambiguity between this Addendum and the <br />Form Contract shall be resolved by giving precedence and effect to the Addendum. <br />Contractor Name: SBC Global Services, Inc. dba AT&T Global Services, a Delaware <br />Corporation <br />Contractor Address: One AT&T Plaza, Dallas, Texas 75202 <br />Title of Form Contract: Master Agreement and attachments <br />Form Contract: <br />The attached Form Contract consists of five (5) pages with terms on one side of the <br />pages. <br />Term: <br />The Contract term begins on 9/8/09 (Delivery Date) and ends on 9/12/09 (Cutover Date). <br />Consideration: <br />Total consideration for the term of the Contract is Three Thousand Six Hundred Thirty <br />and O1/100 Dollars ($3,630.01). <br />By mutual agreement of the parties, the following terms and conditions are deleted from <br />the Form Contract: <br />1. Any provision requiring the City to provide insurance. <br />2. Any provision requiring the City to provide indemnity. <br />
