• <br />EXHIBIT A <br />129 S. Michigan St. Interior Demolition <br />Project No. -107-064 <br />Work shall consist of the following: <br />The Contractor shall do all demolition and repairs to deliver the space in clean condition, <br />with (4) walls and ready for Tenant's finish (including removal of wallpaper and other <br />wall coverings, securely patching all holes in walls and prepping walls for Tenant's <br />paint), the carpet removed, an HVAC, electrical and plumbing in good order, the water <br />line and drain currently in the south wall professionally capped (and related hole in wall <br />repaired), the current sink remaining in the back room and storage area, and the former <br />tenant's adhesive window signs removed. <br />The contract shall have a time limit of ten (10) calendar days. The contract time will <br />start when the Notice to Proceed is delivered and signed. If the contractor fails to <br />complete the work within the contract time, then the contractor will pay to the owner the <br />amount for liquidated damages at a rate of two hundred fifty dollars ($250.00) per <br />calendar day until the completion of the project. <br />The contractor agrees to indemnify, defend and hold harmless the City of South Bend, its <br />agents, officers and employees, from all costs, losses, claims and suits, including court <br />costs, attorney fees, and other expenses, arising from or out of the negligent performance <br />of this Contract by Contractor, or because or arising out of any defect in the goods, <br />materials or equipment supplied by the Bidder. <br />• The following requirements shall also be met by the contractor: <br />1. Insurance Requirements <br />All Contractors and subcontractors during business with the City of South <br />Bend shall present a Certificate of Insurance showing coverage in the <br />following minimum amount: <br />General Liability-Premises-Completed Operations or Products Bodily <br />Injury and Property Damage Combined Single Limit-$5,000,000. or <br />$700,000 per person. <br />There shall be no exclusion for explosion, collapse or underground hazard. <br />Workmen's Compensation: Statutory State of Indiana <br />Employer's Liability - $100,000. <br />Auto Liability: Bodily Injury and Property <br />Damage Combined Single limit <br />$1,000,000. <br />City of South Bend shall be named as additional insured on the Certificate. <br />• <br />
