<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Bernath, LLC dba Seal Master Indianapolis, 1010 East Sumner Avenue,
<br />Indianapolis, Indiana 46627, for the above referenced project, indicating a final cost of
<br />$62,454.60. Upon a motion made by Mr.. Relos, seconded by Ms. Maradik and carried, the
<br />Project Completion Affidavit was approved.

<br />above referenced project, indicating a final cost of $32,900.00. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Relos and carried, the Project Completion Affidavit was approved.
