REGULAR MEETING DECEMBER 8. 2008 Chapin Park, Procession - December 17, 800 Block Park, Ave., Inks/Murphy Neighborhood Candlewalk 2008; 6:00 p.m, north to Riverside, west Approved Association 2008 to 8:00 P.M. on 900 block Riverside, south on 800 block Forest, ending going east on 400 block Manitou. East to House Winter to 4:30 p.m. Main, south to Western, Walk 2009 west to Lafayette, north on Lafayette, return to beginning APPROVE TRAFFIC CONTROL DEVICES Upon a motion made by Mr. Inks, seconded by Mr. Murphy and carried., the following traffic control devices were approved: RENEWAL: Paid Handicapped Reserved Parking Space SI'C-Yn APPLICANT: Thomas Stewart LOCATION: East side of Lafayette, north of Jefferson Blvd., third space from comer (Permit #011) REMARKS: All criteria has been met NEW INSTALLATION: No Parking LOCATION: North side of Western Avenue from Kosciusko to Olive The Division of Engineering, Streets, Traffic and Lighting, and Sewers submitted their Safety I zn Reports for November 2008. These reports reflect injuries /accidents for each month and provide for a comparison. There being no further discussion, upon a motion made by Mr. Inks, second--(] by Mr. Murphy and carried, the reports were accepted and filed. REMONSTRATE Mr. Gilot stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to remonstrate as submitted by William and Barbara Summers. The Consent indicates that in consideration for permission to tap into the public sanitary sewer system of the City, to provide sanitary sewer service to 61135 US 31 South, South Bend, Indiana, (Key 9001-1051-1358), the Summer's waive and release any and all right to remonstrate against or oppose any pending or future anriexation of the property by the City of South Bend. Therefore, Mr. Inks made a motion that the Consent be approved. Mr. Murphy seconded the motion, which carried. FILING OF CERTIFICATE OF INSURANCE Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Certificate of Insurance for the following business was accepted for filing: American Structurepoint, Inc., Indianapolis, Indiana RATIFY APPROVAL AND/OR RELEASE CONTRACTOR AND EXCAVATION BONDS Mr. Anthony Molnar, Division of Engineering, recommends that the following, Contractor Excavation Bonds be ratified pursuant to Resolution 100-2000 and/or released as follows: I Business Bond Type Approved/ Released Effective Date Moore's Concrete, Inc. Contractor Approved 11/18/12008 Underground Contractors, Iric. Excavation Approved 11/18/2008 Mr. Gilot made a motion. that the Bonds approval and/or release as outlined above be ratified. Mr. Inks seconded the motion, which carried. APPROVE CLAIMS Mr, Murphy stated that a request to pay the following claims in the amounts indicated has been received by the Board; each claim is fully supported by an invoice; the person receiving the
