These reports reflect injuries /accidents for each month and provide <br />for a comparison. There being no further discussion, upon a motion made by Mr. Inks, second--(] <br />by Mr. Murphy and carried, the reports were accepted and filed. <br />REMONSTRATE <br />Mr. Gilot stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to <br />remonstrate as submitted by William and Barbara Summers. The Consent indicates that in <br />consideration for permission to tap into the public sanitary sewer system of the City, to provide <br />sanitary sewer service to 61135 US 31 South, South Bend, Indiana, (Key 9001-1051-1358), the <br />Summer's waive and release any and all right to remonstrate against or oppose any pending or <br />future anriexation of the property by the City of South Bend. Therefore, Mr. Inks made a motion <br />that the Consent be approved. Mr. Murphy seconded the motion, which carried. <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Certificate of Insurance <br />for the following business was accepted for filing: <br />American Structurepoint, Inc., Indianapolis, Indiana <br />RATIFY APPROVAL AND/OR RELEASE CONTRACTOR AND EXCAVATION BONDS <br />Mr. Anthony Molnar, Division of Engineering, recommends that the following, Contractor <br />Excavation Bonds be ratified pursuant to Resolution 100-2000 and/or released as follows: I <br />Business <br />Bond Type <br />Approved/ <br />Released <br />Effective Date <br />Moore's Concrete, Inc. <br />Contractor <br />Approved <br />11/18/12008 <br />Underground Contractors, Iric. <br />Excavation <br />Approved <br />11/18/2008 <br />Mr. Gilot made a motion. that the Bonds approval and/or release as outlined above be ratified. <br />Mr. Inks seconded the motion, which carried. <br />APPROVE CLAIMS <br />Mr, Murphy stated that a request to pay the following claims in the amounts indicated has been <br />received by the Board; each claim is fully supported by an invoice; the person receiving the <br />
