REGULAR MEETING FEBRUARY 24, 2015 52 <br />Massage Establishment Renewal <br />Chinese Massage <br />Spa/Jingri Crain <br />Spa <br />2614 South MichiRelos/ <br />gan <br />Street Henthorn <br />APPROVE TRAFFIC CONTROL DEVICE <br />Upon a motion made by David Relos, seconded by Patrick Henthorn and carried, the following <br />traffic control device was approved: <br />NEW INSTALLATION: Handicapped Accessible Parking Space Sign <br />LOCATION: Main Library at 304 South Main Street (Add One <br />Additional Handicap Parking Space North of the Two <br />Existing Spaces on the East Side of Main Street <br />Immediately South of Wayne Street.) <br />REMARKS: All criteria has been met. <br />FILING OF CERTIFICATE OF INSURANCE <br />Upon a motion made by David Relos, seconded by Patrick Henthorn and carried, the Certificates <br />of Insurance for the following businesses were accepted for filing: <br />Gibson Insurance Agency, Property Insurance for Eddy Street Parking Garage <br />RATIFY APPROVAL AND/OR RELEASE CONTRACTOR AND EXCAVATION BONDS <br />Mr. Anthony Molnar, Division of Engineering, recommended that the following Contractor and <br />r le- n_ <br />Excavation Bonds be ratified pursuant to Resolution 100-2000 anaror reieawu as iuiivws. <br />Approved/ Effective Date <br />Business Bond Type Released <br />Enviro Forensic Investigations, Inc„ Excavation Released February 03, 2015 <br />S & L Concrete & Excavating, LLC Excavation Released March 05, 2015 <br />Mr. Gilot made a motion that the Bonds approval and/or release as outlined above be ratified. <br />Mr. Relos seconded the motion, which carried. <br />APPROVE CLAIMS <br />Mr. Relos stated that a request to pay the following claims in the amounts indicated has been <br />received by the Board; each claim is fully supported by an invoice; the person receiving the <br />goods or services has approved the claims; and the claims have been filed with the City Fiscal <br />fWfio.Pr and certifier/ for nocuracv_ <br />Name <br />Amount of Claim <br />Date <br />City of South Bend <br />$1,442,933.91 <br />February 24, 2015 <br />Therefore, Mr. Relos made a motion that after review of the expenditures, the claims be <br />approved as submitted. Mr. Henthorn seconded the motion, which carried. <br />PRIVILEGE OF THE FLOOR <br />Kenny Hayes, Local 645, expressed appreciation for the support of continuing the common wage <br />laws and the recognition by Gary Gilot, Mayor Pete Buttigieg and Eric Horvath of the value of <br />fair benefit package and wages and the importance of the apprenticeship training program. <br />ADJOURNMENT <br />There being no further business to come before the Board, upon a motion made by Gary Gilot, <br />seconded by David Relos and carried, the meeting adjourned at 11:51 a.m. <br />BOARD OF PUBLIC WORKS <br />Gary A.
