<br />APPROVAL OF LICENSE APPLICATION — LAZ PARKING MIDWEST LLC
<br />Mr. Gilot stated that the Board is in receipt of favorable recommendations from the Building
<br />Department, Engineering, and Fire Departments for a License Application from LAZ Parking
<br />Midwest, LLC for a Public Parking Facility at 1228 N. Eddy St., South Bend, Indiana. Upon a
<br />motion made by Mr. Relos, seconded by Mr. Henthom and carried, the License Application was
<br />approved.
<br />APPROVE TRAFFIC CONTROL DEVICE
<br />Upon a motion made by Ms. Roos, seconded by Mr. Henthom and carried, the following traffic
<br />control device was approved:
<br />RENEWAL: Handicapped Accessible Parking Space Sign
<br />LOCATION: No. 2 in front of County -City Building
<br />REMARKS: All Criteria has been met.
<br />APPROVE WASTEWATER DISCHARGE PERMIT DNIFIRST CORPORATION
<br />In a Memorandum to the Board, Ms. Kim Thompson, Division of Environmental Services,
<br />requested a Wastewater Discharge Permit application from Unifirst Corporation, 251 East
<br />Sample Street be approved. There being no further discussion, upon a motion made by Ms. Roos,
<br />seconded by Mr. Henthorn and carried, the permit was approved.
