CLAIMS REVIEW MEETING OCTOBER 21 2014 330 <br />A Claims Review Meeting of the Board of Public Works was convened at 8:36 a.m. on Tuesday, <br />October 21, 2014, with Board Members David Relos, Patrick Henthorn, and Brian Pawlowski <br />present. Board President Gary Gilot and Board Member Kathryn Roos were absent. <br />APPROVAL OF AGREEMENT — BRIAN DONOGHUE COIT/IT PROFESSIONAL <br />SERVICES) <br />Mr. Relos stated that the Board is in receipt of a Professional Services Agreement between the <br />City of South Bend and Brian Donoghue, 853 Sorin Street, Apt. C, South Bend, Indiana, 46617, <br />for 2014 Information Technologies consulting in an amount, not to exceed, of $17,400.00, for <br />696 hours at $25.00 per hour. Mr. John Murphy, City Controller, stated Mr. Donoghue will be <br />working on a cataloguing system for work orders to improve on the current system. Therefore, <br />upon a motion made by Mr. Henthorn, seconded by Mr. Pawlowski and carried, the agreement <br />was approved. <br />APPROVE CLAIMS <br />Mr. Pawlowski stated that a request to pay claims has been received by the Board; each claim is <br />fully supported by an invoice; the person receiving the goods or services has approved the <br />claims; and the claims have been filed with the City Fiscal Officer and certified for accuracy. Mr. <br />Pawlowski stated the following claims were submitted to the Board for approval: <br />Name <br />Amount of Claim <br />Date <br />City of South Bend <br />$1,475,072.35 <br />10/14/2014 and <br />10/17/2014 <br />City of South Bend <br />$2,225,258.75 <br />10/17/2014 <br />St. Joseph County Housing Consortium <br />$9,000.00 <br />10/09/2014 <br />St. Joseph County Housing Consortium <br />$1,342.00 <br />9/25/2014 <br />St. Joseph County Housing Consortium <br />$50,625.00 <br />10/01/2014 <br />Mr. Henthorn made a motion that after review of the expenditures, the remaining claims be <br />approved separately as submitted. Mr. Relos seconded the motion, which carried. <br />ADJOURNMENT <br />There being no further business to come before the Board, upon a motion made by Mr. Relos, <br />seconded by Mr. Henthorn and carried, the meeting adjourned at 8:40 a.m. <br />BOARD OF PUBLIC WORKS <br />Gary A.
