Henthorn, ember <br />ari J. ws r, Member <br />ATTEST: <br />� lerk <br />REGULAR MEETING SEPTEMBER 23 2014 <br />The Regular Meeting of the Board of Public Works was convened at 10:35 a.m. on Tuesday, <br />September 23, 2014, by Board President Gary A. Gilot, with Board Members Kathryn E. Roos, <br />Patrick Henthorn, and Brian Pawlowski present. Board Member David Relos was absent. Also <br />present was Board Attorney Cheryl Greene. <br />ADDITIONS TO THE AGENDA <br />Mr. Gilot noted the addition to the agenda of two Traffic Control Device requests and a Sidewalk <br />Cafe permit request. <br />APPROVE MINUTES OF PREVIOUS MEETING <br />Upon a motion made by Ms. Roos, seconded by Mr. Henthorn and carried, the minutes of the <br />Agenda Review Session, Regular Meeting and Claims Meeting of the Board held on September <br />4, September 9, and September 16, 2014, were approved. <br />PUBLIC HEARING — MASSAGE ESTABLISHMENT LICENSE - THE VIBRANCE <br />CENTER <br />Mr. Gilot advised that this was the date set for the Board's Public Hearing concerning an <br />application for a new massage establishment license submitted by Colleen Kielton on behalf of <br />the Vibrance Center, 328 N. Michigan St., Ste. B3 and B4, South Bend, Indiana. It was noted <br />favorable recommendations were received from the Fire Department, Police Department, and the <br />Building Department. Ms. Colleen Kielton, Business Owner, spoke on behalf of her <br />establishment noting they have been in business for one (1) year as a holistic center, but are now <br />requesting to add a massage license. There being no one else present wishing to address the <br />Board concerning this matter, the Public Hearing was closed. Therefore, upon a motion made by <br />Ms. Roos, seconded by Mr. Henthorn and carried, the license was approved. <br />OPENING OF BIDS — 2014 UNIFORM AND FLOOR MAT SERVICE AND RENTALS <br />VARIOUS DEPARTMENTS <br />This was the date set for receiving and opening of sealed bids for the above referenced service. <br />The Clerk tendered proofs of publication of Notice in the South Bend Tribune and the Tri- <br />County News, which were found to be sufficient. The following bids were opened and publicly <br />read: <br />BSE INC. <br />139B Gatlin Drive <br />Griffith, IN 46319 <br />Bid was signed by: Breanne Stover, President <br />Contractor's Non - Collusion Affidavit, Non - Debarment Affidavit, Certification Regarding <br />Investment with Iran, Employment Eligibility Verification, Non - Discrimination <br />Commitment for Contractors and Certification of Use of United States Steel Products or <br />Foundry Products was submitted. <br />Indiana Local Business Preference Claim Submitted but Not Eligible. <br />$10,000 Bid Bond was submitted. <br />we <br />
