Our monthly statements will include <br />a 1.5% fuel surcharge. Payment is due upon receipt of our invoice. Work can be suspended on <br />any account which is 35 days past due until the account is paid in full. If you have any <br />questions or comments concerning our services or charges during the course of the work, <br />please bring them to our attention immediately so that any problem can be resolved quickly. <br />If this proposal is acceptable to you, please sign the enclosed copy and the following General <br />Conditions form and return a copy to us for our files.
