Is your business based upon licensing intellectual property from research conducted at <br />a public or private university, college, or community college located within St. Joseph <br />County, Indiana? If yes, please include a description of the research based aspects of <br />the business (attach additional sheet if more space is needed): <br />The Innovation Park at Notre Dame is based, in part, on developing and /or licensing <br />intellectual property from the University of Notre Dame and, possibly, otherarea public <br />or private universities located in St. Joseph County, Indiana. The research -based <br />aspects of the business will emerge from a variety of sources, either as the result of a <br />University license of its intellectual property to a member of the faculty or students for <br />commercial development or to a third party with an interest to develop the intellectual <br />property into a commercial venture with the assistance of a faculty- creator of the <br />intellectual property under the auspices of the Innovation Park and its facilities. <br />20. Does your business financially support a Municipal Facility (i.e. municipally owned <br />park, recreation center, cultural, arts, or entertainment facility)? N/A <br />21. Is your company incurring any of the cost of extension or oversizing of municipal <br />infrastructure serving the project site? This includes water, sewer, drainage facilities, <br />wastewater treatment facilities, road and street improvements, street lighting, traffic <br />control and related public improvements (attach additional sheet if more space is <br />needed): <br />The City of South Bend has incurred the costs of extension of municipal utilities to the <br />project site. The University of Notre Dame is providing all utility infrastructure on the <br />project site and incurring the costs of connecting such utility infrastructure to the <br />municipal utilities provided by the City of South Bend. In addition, the University of <br />Notre Dame funded the improvements to Twyckenham Drive and Edison Road in and <br />around the property. <br />6 <br />(Rev 12/29/06) <br />
