The property sought to be rezoned is located at <br />2515 East Jefferson Boulevard, South Bend, Indiana 46615 <br />2. The property is owned by The South Bend Osteopathic Hospital, Inc. <br />3. A legal description of the property is as follows: <br />"Lots 3,4,5,6,7 and 8 as shown on the recorded Plat of <br />Tuxedo Park First Addition to South Bend, St. Joseph County, <br />Indiana, together with the strip of land 35 feet in width, <br />more or less, running along the entire East side of said Lot 8 <br />from the North line of Jefferson Boulevard North to the South <br />line of Marshall Street, being the same and identical strip of <br />land which was formerly known as 26th Street and which was heretofore <br />vacated as a public street, by action of the Board of Public Works <br />and Safety under Vacation Resolution Nuxber 2824, in St. Joseph <br />County Indiana." <br />(CONTINUE ON THE REVERSE SIDE, IF NECESSARY) <br />
