j <br /> REGULAR MEETING DECEMBER 27, 2004 <br /> APPROVE CONSTRUCTION CONTRACT — SHERMAN AVENUE - QUEEN <br /> STREET TO ROSE STREET — NORTHERN CONSTRUCTION SERVICES <br /> CORPORATION - PROJECT NO, 104-025--(GENERAL FUND) <br /> Mr. Gilot advised that in accordance with the bid awarded on December 13, 2004, to <br /> Northern Construction Services, Corporation, Post Office Box 1299, Niles, Michigan, in <br /> the amount of$78,918.25 for the above referred to project, a Contract in said amount was <br /> being submitted for Board approval. Upon a motion made by Mr. Inks, seconded by Mr. <br /> Littrell and carried, the Contract was ratified and the appropriate Certificate of Insurance, <br /> Performance Bond and Labor and Materials Payment Bond as submitted were filed. <br /> APPROVAL OF SERVICE AGREEMENTS — BOARD OF PUBLIC SAFETY <br /> MEMBERS <br /> —JESUSA RODRIGUEZ <br /> - ELMER L. CARR <br /> - DONALD ALFORD <br /> Mr. Gilot stated that the Board is in receipt of three (3) Service Agreements between the <br /> City of South Bend and Ms. Jesusa Rodriguez, South. Bend, Indiana; Mr. Elmer L. Carr, <br /> South Bend, Indiana; and Mr. Donald Alford, South Bend, Indiana. for Board of Public <br /> Safety Services. Mr. Gilot noted that the Agreements are in the amount of$3,400.00 per <br /> year, for January 1, 2005 through December 31, 2005, for each individual. Therefore, <br /> upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the Agreements <br /> were approved and executed. <br /> APPROVAL OF AGREEMENT S.R. 933 BRIDGE OVER ST. JOSEPH RIVER <br /> INDIANA DEPARTMENT OF TRANSPORTATION - PROJECT NO. 99-090 (COIT) <br /> Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South <br /> Bend and Indiana Department of Transportation, Indianapolis, Indiana, for the City to <br /> share lighting expenses for the S.R. 933 Bridge over the St. Joseph River. Mr. Gilot <br /> noted that the Agreement is in the amount of $76,000.00. Therefore, upon a motion <br /> made by Mr. Gilot, seconded by Mr. Littrell and carried, the Agreement was approved <br /> and executed. <br /> APPROVAL OF STAFFING AGREEMENT — REBUILDING TOGETHER ST. <br /> JOSPPH COUNTY <br /> Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South <br /> Bend and Rebuilding Together, St. Joseph County, for staff support. Mr. Gilot noted that <br /> the Agreement is in the amount of $14,857.00, reimbursed to the City of South Bend. <br /> Therefore, upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the <br /> Agreement was approved and executed. <br /> APPROVAL OF SUPPLEMENTAL AGREEMENT NO. 1 — DOWNTOWN TWQ- <br /> WAY TRAFFIC STUDY—DLZ INDIANA—PROJECT NO. 104-056 (COIT) <br /> Mr. Gilot stated that the Board is in receipt of a Supplemental Agreement between the <br /> City of South Bend and DLZ Indiana, 2211 East Jefferson Boulevard, South Bend, <br /> Indiana, to add to the scope of the project to include intersections on Lafayette. <br /> Therefore, upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried the <br /> Supplemental Agreement No. 1 was approved and executed. <br /> APPROVAL OF AGREEMENT — RIVERSIDE TRAIL PHASE I — INDIANA <br /> DEPARTMENT OF TRANSPORTATION—PROJECT NO. 102-040 COIT <br /> Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South <br /> Bend and the Indiana Department of Transportation, Indianapolis, Indiana, for the above <br /> stated project. Mr. Gilot noted that the Agreement is in the amount of $240,000.00. <br /> Therefore, upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the <br /> Agreement was approved and executed. <br /> APPROVAL OF SUPPLEMENTAL AGREEMENT NO. 1 — IRELAND ROAD FROM <br /> BURNETT DRIVE TO MICHIGAN STREET — AMERICAN CONSULTING — <br /> PROJECT NO. 104-040 (COLT) <br /> Mr. Gilot stated that the Board is in receipt of a Supplemental Agreement between the <br /> City of South Bend and American Consulting, 7260 Shadeland Station, Indianapolis, <br /> Indiana, to amend the Agreement to include right-of-way services for additional parcels. <br /> Mr. Gilot noted that the Supplemental Agreement is an increase in the amount of <br />
