Inks, Member <br /> AT-'TEST: <br /> fill,F e f <br /> Angela K. .IAcob, ClerkJ <br /> REGULAR MEETING OCTOBER 25 2004 <br /> The regular meeting of the Board of Public Works was convened at 9:32 a.m. on Monday, October <br /> 25,2004,by Board President Gary A.Gilot,with Mr. Carl P.Littrell and Mr.Donald E.Inks present. <br /> Also present were Board Attorney Thomas Bodnar and Attorney John Livingston. <br /> AGENDA ITEMS ADDED/TABLED <br /> Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the following items were <br /> added/tabled: <br /> Construction Contract - River Park Service Area Improvements <br /> - Construction Contract - U.S. 31 Industrial Park Access Drive Extension <br /> - Construction Contract - Site Clearing - LaSalle Neighborhood <br /> - Construction Contract - Bendix Drive at NICTD Railroad Crossing <br /> - Consulting Services Agreement - University of Cincinnati - Tabled <br /> - Proposal - Limno Tech- Tabled <br /> APPROVE MINUTES OF PREVIOUS MEETING <br /> Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the minutes of the regular <br /> meeting of the Board held on October 7 and 11, 2004 and a special meeting of October 15, 2004, <br /> were approved. <br /> OPENING OF BIDS-NEW EMS FIRE STATION NO. 13 -PROJECT NO. 1004-037 TIF BOND <br /> This was the date set for receiving and opening of sealed bids for the above referred to project. The <br /> Clerk tendered proofs of publication of Notice in the South Bend Tribune and the Tri-County News <br /> which were found to be sufficient. The following bids were opened and publicly read: <br /> ZIOLKOWSKI CONSTRUCTION INC. <br /> 1005 South Lafayette Boulevard <br /> South Bend, Indiana 46601 <br /> Bid was signed by Mr. Ben Ziolkowski <br /> Non-Collusion Affidavit was in order <br /> Non-Discrimination Commitment farm was completed <br /> Five per cent (5%) Bid Bond was submitted <br /> BID: $1,256,090.00 <br /> Alternates: <br /> Alternate 1 Exterior Storage Shed $23,320.00 <br /> Alternate 2 Emergency Generator $3,895.00 <br /> Enclosure Fence <br /> =Alternate 3 Concrete Service Walk $2,480.00 <br />
