J .'23 <br /> REGULAR MEETING JULY 12, 2004 <br /> AWARD BID - ONE 1 2004 3/4 TON EXTENDED CAB FOUR WHEEL DRIVE PICK-UP <br /> TRUCK WATER WORKS WATER WORKS FUND <br /> Mr. Matthew Chlebowski, Central Services, advised the Board that on June 28, 2004, one (1) bid <br /> was received and opened for the above referred to vehicle. After reviewing the bid, Mr. Chlebowski <br /> recommends that the Board award the contract to the lowest responsible and responsive bidder, <br /> Terry's Ford,'63 North Harlem,Peotone,Illinois in the amount of$21,764.00. Therefore,Mr.Gilot <br /> made a motion that the recommendation be accepted and the bid be awarded as outlined above. Mr. <br /> Littrell seconded the motion which carried. <br /> AWARD QUOTATION-OLIVER SCHOOL SITE MOWING/MAINTENANCE-PROJECT NO. <br /> 104-047 (2003 PLAN IMPLEMENTATION FUND <br /> Ms. Tony Zozulya, Community and Economic Development, advised the Board that on June 28, <br /> 2004,one (1)quotation was received and opened for the above referred to project. After reviewing <br /> the quotation, Ms. Zozulya recommends that the Board award the contract to the lowest responsive <br /> and responsible bidder, Infolink, Inc., 1657 Commerce Drive, South Bend, Indiana in the amount <br /> of$298.00 per mowing. Therefore, Mr. Gilot made a motion that the recommendation be accepted <br /> and the bid be awarded as outlined above. Mr. Littrell seconded the motion which carried. <br /> AWARD BID - ST. JOSEPH AND JEFFERSON STREET PARKING LOT IMPROVEMENTS - <br /> PROJECT NO. 104-042 <br /> Mr. Tim. Williams, Community and Economic Development, advised the Board that on June 28, <br /> 2004, bids were received and opened for the above referred to project. After reviewing those bids, <br /> Mr. Williams recommends that the Board award the contract to the lowest responsive and <br /> responsible bidder, Walsh and Kelly,Inc.,24358 State Road 23, South Bend,Indiana in the amount <br /> of$96,794.98. Therefore, Mr. Gilot made a motion that the recommendation be accepted and the <br /> bid be awarded as outlined above, subject to a pre-conference to identify the scope of the project. <br /> Mr. Inks seconded the motion which carried.. <br /> APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - BIKE <br /> TRAIL LIGHT REPLACEMENT - MARTELL ELECTRIC - PROJECT NO. 103-083 COIT <br /> Mr. Gilot advised that Mr.Toy Villa,Public Construction Manager,has submitted the Final Change <br /> Order on behalf of Martell Electric, 1 125 South Walnut, South Bend, Indiana, Indicating that the <br /> contract amount be increased by $1,378.00 for a new contract sum including this Change Order in <br /> the amount of$25,806.00. Additionally submitted was the Project Completion Affidavit indicating <br /> this new final cost. Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the <br /> Final Change Order and the Project Completion Affidavit were approved subject to the filing of the <br /> appropriate three-year Maintenance Bond. <br /> APPROVE FINAL CHANGE ORDERAND PROJECT COMPLETION AFFIDAVIT-LASALLE <br /> PARK PUBLIC WORKS IMPROVEMENTS - MCINTYRE JONES INC. - PROJECT NO. 103- <br /> 064 (CDBG) <br /> Mr. Gilot advised that Mr.Toy Villa,Public Construction Manager,has submitted the Final Change <br /> Order on behalf of McIntyre Jones, Inc., 2526 West Sixth, Mishawaka, Indiana, indicating that the <br /> contract amount be increased by$4,695.28 for a new contract sum including this Change Order in <br /> the amount of$131,286.63. Additionally submitted was the Project Completion Affidavit indicating <br /> this new final cost. Upon a motion made by Mr. Inks, seconded by Mr.Littrell and carried,the Final <br /> Change Order and the Project Completion Affidavit were approved subject to the filing of the <br /> appropriate three-year Maintenance Bond. <br /> APPROVAL OF LETTER OF CONCURRENCE - COUNTY OF ALLEGAN MICHIGAN <br /> Mr. Gilot stated that the Board is in receipt of a Letter of Concurrence between the City of South <br /> Bend and the County of Allegan,Michigan. They are installing a new 800 MHz system for county- <br /> wide coverage for their fire and ambulance services. Three of their ten channels are co-channels <br /> with the City of South Bend. Since AIlegan County is within a certain distance of South Bend,the <br /> Federal Communications Commission states that a Letter of Concurrence must be agreed upon by <br /> both parties. Therefore,upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried,the <br /> Letter of Concurrence was approved and executed. <br />
