<br /> Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the recommendation to
<br /> award the above referred to abandoned vehicles was approved.
<br /> AWARD QUOTATION-LEIGHTON PLAZA JEFFERSON ENTRANCE REPAIRS-PROJECT
<br /> NO. 104-049 (LRSA)
<br /> Mr. Toy Villa, Public Construction Manager, advised the Board that on June 15, 2004, quotations
<br /> were received and opened for the above referred to project. After revievwring those bids, Mr. Villa
<br /> recommends that the Board award the contract to the lowest, responsive and responsible bidder,
<br /> Youngs Excavating, Post Office Box 2766, South Bend, Indiana in the amount of $12,246.00.
<br /> Therefore, Mr. Gilot made a motion that the recommendation be accepted and the quotation be
<br /> awarded as outlined above. Mr. Inks seconded the motion which carried.
<br /> INVOKE EMERGENCY POWERS
<br /> Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, emergency powers for a
<br /> sewer repair at Elwood and College were approved.
<br /> APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - WABASH
<br /> SANITARY SEWER EXTENSION PROJECT - PROJECT NO. 104-045 (BOND)
<br /> In a memorandum to the Board,Mr. Jason Durr, Engineering,requested permission to advertise for
<br /> the receipt of bids for the above referred to project. Therefore, upon a motion made by Mr. Gilot,
<br /> seconded by Mr. Littrell and carried, the request to advertise for the receipt of bids was approved.
<br /> APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - SALE OF
<br /> ABANDONED VEHICLES
<br /> Ina memorandum to the Board,Ms.Kathy Eli,Code Enforcement,requested permission to advertise
<br /> for the receipt of bids for the sale of abandoned vehicles. Therefore, upon a motion made by Mr.
<br /> Gilot,seconded by Mr.Inks and carried,the request to advertise for the receipt of bids was approved.
<br /> APPROVE CHANGE ORDER - SAMPLE STREET SEWER REPAIR - 1500 BLOCK -
<br /> UNDERGROUND SERVICES - PROJECT NO. 104-044 (SEWAGE WORKS)
<br /> Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order
<br /> No. 1 on behalf of Underground Services,225 South Lafayette, South Bend, Indiana indicating that
<br /> the Contract amount be decreased $9,293.75 for a new Contract sum including this Change Order
<br /> in the amount of $119,155.25. Upon a motion made by Mr. Inks, seconded by Mr. Littrell and
<br /> carried, the Change Order was approved.
<br /> APPROVE CHANGE ORDER - TRAFFIC SIGNAL MODERNIZATION IRELAND ROAD -
<br /> MICHIANA CONTRACTING - (LPA)
<br /> Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order
<br /> No. E 1 on behalf of Michiana Contracting, South Bend,Indiana indicating that the Contract amount
<br /> be increased $8,735.65 for a new Contract sum including this Change Order in the amount of
<br /> $473,074.81. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Change
<br /> Order was approved.
<br /> APPROVE CERTIFICATE OF SUBSTANTIAL COMPLETION - FREDRICKSON PARK
<br /> LANDFILL - PROJECT NO. 103-018
<br /> Mr. Gilot advised that Mr. Darrell Baker, Engineering, has submitted a Certificate of Substantial
<br /> Completion for the above referred to project. Upon a motion made by Mr. Gilot, seconded by Mr.
<br /> Littrell and carried, the Certificate of Substantial Completion was approved.
<br /> APPROVAL OF SUPPLEMENTAL AGREEMENT NO. 3 - ADDITIONAL APPRAISAL
<br /> SERVICES -BRICK ROAD/OLIVE ROAD RECONSTRUCTION-DLZ INDIANA (AIRPORT
<br /> ECONOMIC DEVELOPMENT BOND)
<br /> Mr. Gilot stated that the Board is in receipt of a Supplemental Agreement between the City of South
<br /> Bend and DLZ Indiana,2211 East Jefferson, South Bend,Indiana,for the above referred to project.
<br /> Mr. Gilot noted that the Supplemental Agreement is in the amount of$28,728.00, bringing the
<br /> amount of the Supplemental Agreement to $604,695.40. Therefore, upon a motion made by Mr.
<br /> Littrell,seconded by Mr.Inks and carried,the Supplemental Agreement was approved and executed.
