REGULAR MEETING JUNE 14, 2004 <br /> APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - OLIVER PLOW <br /> WORKS DEMOLITION - PHASE D - PROJECT NO. 100-092D (HUD EDI GRANT <br /> In a memorandum to the Board,Mr. Jason Durr,Engineering,requested permission to advertise for <br /> the receipt of bids for the above referred to project. Therefore, upon a motion made by Mr. Inks, <br /> seconded by Mr. Littrell and carried, the request to advertise for the receipt of bids was approved. <br /> APPRO'V'E CHANGE ORDER - PALAIS ROYALE RETAIL SPACE ZIOLKOVVSKI <br /> CONSTRUCTION - PROJECT NO. 104-020 (LANDLORD IMPROVEMENT FUND) <br /> Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order <br /> No. 1 on behalf of Ziolkowski Construction, 1005 South Lafayette, South Bend, Indiana indicating <br /> that the Contract amount be increased $20,637.94 for a new Contract sum including this Change <br /> Order in the amount of$567,637.94. Upon a motion made by Mr. Inks,seconded by Mr.Littrell and <br /> carried, the Change Order was approved. <br /> APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - <br /> BROOKFIELD STREET IMPROVEMENTS - RIETH RILEY CONSTRUCTION COMPANY - <br /> PROJECT NO. 103-006 (CDBG) <br /> Mr. Gilot advised that Mr. Toy Villa, Construction Manager,has submitted the Final Change Order <br /> on behalf of Rieth Riley Construction Company, Inc., 25200 State Road 23, South Bend, Indiana, <br /> indicating that the contract amount be decreased by$10,090.92 for a new contract sum including this <br /> Change Order in the amount of$31,234.08. Additionally- submitted was the Project Completion <br /> Affidavit indicating this new final cost. Upon a motion made by Mr. Gilot, seconded by Mr. Inks <br /> and carried,the Final Change Order,the Project Completion Affidavit,and the Waiver of Lien were <br /> approved. <br /> APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - <br /> MISHAWAKA AVENUE IMPROVEMENTS -MCINTYRE JONES INC. -PROJECT NO. 104- <br /> 009 (GENERAL FUND) <br /> Mr. Gilot advised that Mr. Toy Villa, Construction Manager,has submitted the Final Change Order <br /> on behalf of McIntyre Jones, Inc, 2526 West Sixth Street, Mishawaka, Indiana, indicating that the <br /> contract amount be decreased by $4,070.76 for a new contract sum including this Change Order in <br /> the amount of$67,889.76. Additionally submitted was the Project Completion Affidavit indicating <br /> this new final cost. Upon a motion made by Mr. Inks,seconded by Mr. Littrell and carried,the Final <br /> Change Order, the Project Completion Affidavit, and the Waiver of Lien were approved. <br /> APPROVE PROJECT COMPLETION AFFIDAVIT - WASTEWATER TREATMENT PLANT <br /> ANAEROBIC SLUDGE DIGESTER CLEANING-SYNAGRO MIDWEST INC.-PROJECTNO. <br /> 103-035 - SE14'AGE WORKS) <br /> Mr. Gilot advised that Mr. Robert Toppel, Environmental Services, has submitted the Project <br /> Completion Affidavit,Land Application Certification, and Waiver of Lien for the above referred to <br /> project,indicating a final cost of$112,900.00. Upon a motion made by Mr. Gilot, seconded by Mr. <br /> Littrell and carried, the Project Completion Affidavit, Land Application Certification and Waiver <br /> of Lien were approved. <br /> APPROVE CHANGE ORDERNO. 1 FINAL AND PROJECT COMPLETION AFFIDAVIT-ST. <br /> JOSEPH COUNTY JAIL SITE PREPARATION-PHASE 11- RITSCHARD BROTHERS INC. - <br /> PROJECT NO. 103-042A COIT <br /> Mr. Gilot advised that Mr. Darrell Baker,Engineering has submitted Change Order No. 1 (Final)on <br /> behalf of Ritschard Brothers, 1204 West Sample, South Bend, Indiana, indicating that the contract <br /> amount be increased by$6,997.28 for a new contract sum including this Change Order in the amount <br /> of$152,211.20. Additionally submitted was the Project Completion Affidavit indicating this new <br /> final cost. Upon a motion made by Mr.Littrell, seconded by Mr.Inks and carried,Change Order No. <br /> 1 (Final)and the Project Completion Affidavit were approved subj ect to the filing of the appropriate <br /> three-year Maintenance Bond. <br />
