Green Tech/Republic Services <br /> • 329 IAC 11-13.5-14(a)(1):All facility floors and the loading bay tunnel must be maintained so as to <br /> prevent standing water within the facility structure.Alt drainage and liquids originating from: (A) <br /> storage, handling, and processing MSW; (B)cleaning floors; or(C)washout water from a MSW <br /> vehicle; must be properly directed to a sanitary sewer, a holding tank constructed and operated in <br /> accordance with any applicable local approvals, or the equivalent of a sanitary sewer or holding <br /> tank. <br /> • Violation Details: At the time of inspection on March 12, 2025, erosion of the tipping floor had <br /> resulted in large depressions which exposed rebar in multiple areas. In addition, a large hole had <br /> developed in the east end of the tipping floor.Water and waste had become trapped in this hole. <br /> • Status Update October 10, 2025:At the time of inspection, the previously documented hole in the <br /> tipping floor and exposed rebar were observed. Mr. Havemann provided receipts for concrete <br /> used to attempt a repair on April 18, 2025, and a photo of the temporary repair to the tipping floor <br /> was provided via text message on October 15, 2025. However,the April 18, 2025, repair was <br /> unsuccessful. A quote to cut out and replace the damaged section of the floor has been obtained, <br /> but the work is not yet contracted.A second attempt to temporarily repair the hole in the tipping <br /> floor has been contracted, but a date for the repair has not been established. <br /> • Status Update January 14, 2026:At the time of inspection,the hole in the tipping floor was being <br /> filled with sand and rock daily to create a workable surface. Mr. Havemann stated that the <br /> company has decided to resurface the entire tipping floor and have the work done by a company <br /> based in DeMotte, IN.
