<br />Name: Bill Lamie Date: April 11, 2025 <br />Address: 202 S Jacob St. <br /> South Bend, IN 46615 <br />Phone: <br /> <br />RE: Shingle Roof Quote <br />Millwood Roofing and Construction Inc. propose to complete the following: <br />Scope of Roof Work: 14 SQ Roof (upper roof and south sunroom roof) <br />• Tear off existing roof material and dispose/recycle of all trash and debris. <br />• Keep the property clean, trash picked up, and broom swept each day before leaving using “New <br />Roof No Mess” procedures. <br />• Inspect the roof deck and make repairs as needed at an additional cost of $5.00/SF <br />• Install new aluminum roof edging. <br />• Apply ice and water shield at the eaves, valleys, and around roof penetrations (soil pipes, <br />chimneys, etc.) <br />• Replace all wall flashings and step flashings. <br />• Remove all box vents and install continuous ridge vent to replace. <br />• Replace all counter flashings. <br />• Replace soil/vent pipe flashings. <br />• Install synthetic underlayment. <br />• Install new Certianteed Landmark architectural shingles in a color to be selected by homeowner. <br />• Install new Certainteed Shadow Ridge ridge-cap shingles. <br />• Fasten shingles using 4 nails per shingle. <br />• Install soffit vent holes with plugs along the eaves of the upper roof. <br />• Complete and thorough cleanup of the property. <br /> <br />Labor and Material Total $15,425.00 <br /> <br />Insulation Options: <br /> <br />Proposal #1 <br />• Prep attic space by applying plastic protection over floors and contents in attic. <br />• Install black foam board over window and vents. <br />• Apply R-30 closed cell foam insulation to underside of roof deck and gable walls. <br />• Soffit vents will not be needed with this option and have been deducted in this price. <br />• Cleanup areas as needed. <br />Add $15,718.00 <br /> <br />Proposal #2 <br />• This proposal will require attic contents to be removed from attic by others. <br />• Pull up floor boards in attic space. <br />• Suck out all existing insulation and dispose. <br />• Apply R-21 open cell foam to 2nd floor ceiling. <br />• Install new ¾” T&G OSB floor across new insulation. <br />• Cleanup areas as needed. <br />Add $17,454.00 <br /> <br />
