leaptodigital.com 2.30.3 <br />Lead-Based Paint Pamphlet: If the property was constructed prior to 1978, Buyer hereby acknowledges receipt of a copy of the pamphlet titled, “Protect <br />Your Family from Lead in Your Home”, informing them of the potential risk of lead hazard exposure from renovation activity to be performed in their <br />home, at the address written on the front side of this Agreement. Buyer received this pamphlet on the date of this Agreement and before the commencement <br />of work. <br />Change Orders and Costs: Any alteration or deviation from the scope of work involving extra costs will be set forth in a Change Order agreement <br />between Mad City and Buyer(s). Occasionally, due to unforeseen problems or circumstances, extra work will be necessary to complete the project in a <br />workmanlike manner and will be done pursuant to an aforementioned Change Order. Mad City reserves the right to cease work on the project and terminate <br />this Agreement if Buyer(s) refuses to execute a change order for extra work Mad City deems essential. The completion date shall be extended accordingly <br />as a result of any change order. <br />Third-Party Financing: Buyer(s) agrees and understands that if he/she finances the work with a separate financing company, the financing documents <br />provided separately to Buyer(s) will include the number of monthly payments and the amount of each payment, including any finance charge. Buyer(s) <br />understands that Mad City is neither a broker nor a lender. If Buyer(s) chooses to amend this agreement to include third-party financing after this contract is <br />signed and if Buyer(s) does not qualify for the full amount of the balance due on this Agreement, Buyer(s) is obligated to pay in full at the time of <br />Substantial Completion. <br />Indiana Consumer: Buyer(s) acknowledges that Mad City may utilize a third party or parties, including subcontractors, vendors, or other persons that are <br />not a party to this Agreement, to lease or furnish labor, services, material, equipment, or machinery to, or on behalf of, Mad City in connection with the real <br />property improvement pursuant to this Agreement. <br />Indiana Right to Cancel: You may cancel this contract at any time before midnight on the third business day after the later of the following: <br />(A) The date this contract is signed by you and (name of real property improvement supplier). <br />(B) If applicable, the date you receive written notification from your insurance company of a final determination as to whether all or any part of your claim <br />or this contract is a covered loss under your insurance policy. <br />See attached notice of cancellation form for an explanation of this right. <br />READ & ACCEPTED <br />Fred Jeanette Slaski/Spence <br />10/08/2025 <br />Date <br />Page 5 of 23
