Name, address , and telephone number; <br />2 . A de scription of information provided by the bidder or subcontractor; and <br />3 . A statement of whether an agreement was reached , and if not , why not , including any reasons for <br />concluding that the MBE was unqualified to perform the job. <br />*Proper demonstration of Good Faith Efforts requires your initials next to all of the above boxes. Any <br />omissions shall be considered grounds for rejection of the bid by the Board of Public Works.
