Only <br />the noted items are affected, and the remainder of the original contract document is to remain in its <br />entirety. <br />This Addendum modifies the Contract Documents as follows: <br />1.01 CITY OF SOUTH BEND CONTRACTOR'S BID FOR PUBLIC WORKS <br />I. REPLACE all pages of the Contractor's Bid for Public Works with the attached pages I <br />through 20. Included sections: Contractor's Bid for Public Works, Non -Collusion and <br />Non -Debarment Affidavit, Certification Regarding Investment with Iran, Employment <br />Eligibility Verification, Non -Discrimination Commitment, Certification of Use of United <br />States Steel Products or Foundry Products, Bid/Proposal Form, and MBE/WBE Utilization <br />Plan documents. <br />END OF ADDENDUM <br />
