Attachment <br />Supplemental to Form 96 Section H — +Question I <br />Milestone Contractors will assign a General Superintendent to oversee all facets of the <br />proposed work. After a general safety plan is implemented and reviewed, the <br />Superintendent will be responsible for managing demolition/milling crews to remove the <br />specified streets, and then direct paving crews to re -construct the streets, as the <br />plans/specifications dictate. Other incidental items such as curb ramps and/or concrete <br />curbing may be required, depending on the chosen streets. At all times, Milestone will <br />be responsible for proper traffic control and will coordinate these operations with the <br />owner and any affected residents. See sample workflow below: <br />• Contract is Signed <br />• Schedule prepared <br />• Safety Plan Reviewed <br />• Traffic control Setup <br />• Excavation/Demolition crews begin (phased per plan) <br />• Waste Material is hauled offsite <br />• Base material installed <br />• Underdrains installed <br />• Curbs installed <br />• New Stone/Asphalt paving, per specs <br />• Concrete driveways installed <br />• Monument/signs installed <br />• Landscape/dress up of disturbed areas <br />• Final Walkthrough with owner <br />
