Print legibly and complete all sections to ensure efficient processing. <br />2. Assemble all required information and materials before filing application. <br />3. Thoroughly review all applicable Municipal Code Sections listed above. <br />4. Licenses expire January 31. <br />5. Apply for renewal by January 1 to avoid lapse in licensure. <br />6. Add 10% to license fee for renewal after January 31. <br />7. Include $5.00 license application fee payable to City of South Bend. <br />8. License fee payable to City of South Bend due at issuance - $250.00 <br />9. Register daily transactions through LeadsOnline (instructions included). <br /> <br /> <br />APPLICATION PROCESS: <br /> <br />1. Thoroughly review Municipal Code Section 4-51. <br />2. Submit Application with $5.00 Processing Fee. <br />3. Police background check of applicant, inspections by the Fire, Building, Water Works and <br />Code Enforcement Departments. <br />4. License issued on completion of inspections and receipt of payment. <br />5. Record keeping and inspections required to comply with Municipal Code Section 4-51.
