Proposed To: Everstory Partners – Attn: Tim Rogers <br />Riverview Cemetary <br /> <br /> <br /> CONTRACT <br /> <br /> <br /> <br /> <br /> The undersigned property owner(s), hereinafter called the “Owner,” does hereby contract and authorize the undersigned South Bend Roofing & <br />Siding Company, Inc., hereinafter called the “Contractor,” to furnish the necessary materials and labor to install the improvements listed below, subject to <br />the following terms and conditions, on the premises located at: <br />PROJECT ADDRESS <br />IMPROVEMENT SPECIFICATIONS* <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />Terms: Contract Price Sales Tax Down Payment with 100% due upon completion <br /> Acceptance of this contract must be made on or before: Approximate Start Date: Will Call to Schedule <br /> This contractor carries insurance for public liability, property damage and workmen’s compensation. The Contractor agrees to remove all waste materials from the premises upon completion of the work. All surplus <br />material remains the property of the Contractor. All material warranties are those specified by the manufacturer furnishing the materials. If the customer does not have a satisfactory credit rating, this contract may be cancelled by the <br />Contractor. This Contract shall not be binding on the Contractor unless and until it is approved by an officer in preparation of work and material specified under the above improvement specifications. All aluminum storm door, window, <br />and special order products cannot be cancelled, and the full contract price shall be paid the contractor as liquidated damages for the breaking of the contract. <br /> If all work has been completed and payment is not made, then the contractor will have the rights to hire an attorney and file a lien if needed. All legal costs involved will be added to the contract and a 1.5% interest per <br />month to any unpaid balance. The undersigned Owner hereby acknowledges that he/she has read the contract and understands that all materials to be furnished and work to be done are shown under the Improvement Specifications <br />listed above. It is further acknowledged by the undersigned that this contract constitutes the entire agreement between the parties and that no other agreements or stipulations verbal or written, have been made or implied. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />SCOPE – Gutter Replacement (Riverview Cemetery – Building at Entrance of Property). <br />Set up in areas approved by owner. <br />1. Remove existing gutter and downspouts from the building. <br />2. Install all new 6” seamless K-style gutter. <br />3. Install all new 3x4 downspouts. <br />4. Includes all necessary hardware and accessories for a fully finished system. <br />5. All gutter and downspouts to be installed in color approved by the Historical Society. <br />6. We will keep a clean jobsite during construction and clean up all debris to pre-work conditions. <br /> <br />TOTAL JOB (gutter replacement) = $3,050 <br /> <br /> <br />PERMIT INCLUDED (Including Working with Historical Society for Proper Permit and Approval). <br />NO DOWN PAYMENT REQUIRED.
