Letter of Notice for Random Testing Date: (Exatnple: April 8, 2016) Re: Testing Procedure for MCAUhndiana State Pipe Trades (ISPTA) Identification Card Xour oame has Ueen randonnly picked by MCAUISPTA computer generated selection program to' take a drug test as soon as possible in order to update your MCAUISPTA identification card. Our computer program selects the names of one twelfth (1/12) of all participant cardholders for update each month. We wmit to re-emphasize that your selection for a test is purely a random one. You wilt be taking the same test, which you initially took when you received your MCAUISPTA card. In order to avoid any potential interruption in the status of your card, you must be tested by ,(Example date: April 15, 2016). Tests are to be taken on your own time at any one of the facilities identified on the attached list. You will be mailed a $35.00 MCAUISPTA expense reimbursement check if your test is negative/valid. Reimbursement checks will be void after 60 days from the date of the check. All MCAUISPTA participants who are working may have their MCAUISPTA card verified with the database administrator in order to determine if their card is "valid" or "not valid" tinder MCAIISPTA Program. This procedure will protect everyone's confidentiality. Please remember that if you fail to take the test by the date listed above, the result may be treated as a positive test. 'r'rPlease be aware of the closing time ofthe collection facility you choose to report to for your random test. If you start the testing process you will be expected to finish the process (bv providing a sufficient m•fue specimen) within 2 hours OR by the time the facility closes, which ever' is sooner. • If you fail to finish the process, it will be considered a refusal to test which has the same consequences as a positive test result.
