FY 2025-2029 Five-Year Consolidated Plan City of South Bend, IN <br /> If data is not available for the categories "number of persons becoming and exiting <br /> homelessness each year," and "number of days that persons experience homelessness," <br /> describe these categories for each homeless population type (including chronically homeless <br /> individuals and families, families with children, veterans and their families, and <br /> unaccompanied youth): <br /> Chronically homeless individuals and families—The Chronically Homeless is defined as: an individual <br /> or family that is homeless and lives or resides in a place not meant for human habitation,a safe haven or in <br /> an emergency shelter;has been homeless and living there continually for at least one year or on at least four <br /> separate occasions in the last three years;and has an adult head of household(or a minor head of household <br /> if no adult is present in the household) with a diagnosable substance use disorder, serious mental illness, <br /> developmental disability(as defined in Section 102 of the Developmental Disabilities Assistance and Bill <br /> of Rights Act of 2000 (42 U.S.0 15002)), post-traumatic stress disorder, cognitive impairments resulting <br /> from brain injury, or chronic physical illness or disability, including the co-occurrence or two or more to <br /> those conditions. According to Point In Time count data, St. Joseph County reported 118 chronically <br /> homeless persons in 2018 and 87 in 2024. There were six (6) chronically homeless unaccompanied youth <br /> in 2023 and there was one(1) chronically homeless unaccompanied youth in 2024. There appears to be a <br /> need in the area for outreach and services for the chronically homeless adult population. <br /> Families with children — The Point-In-Time count for 2023 identified 45 homeless households with <br /> children in St. Joseph County, for a total of 150 persons. Numbers remained relatively stable in 2024 at 46 <br /> homeless households with 145 persons. These numbers have been consistent, though high for a single <br /> county in the Indiana Balance of State CoC. <br /> Veterans and their families —The Center for the Homeless has a Veterans Annex called the Millers <br /> Veterans Center. Homeless Veterans often require additional services, with mental health services being <br /> the most common cited need.The number of homeless Veterans in St.Joseph County has slightly increased <br /> from 32 in 2023 to 33 in 2024. <br /> Unaccompanied youth—Teens aging out of foster care,and young adults in the 18-24 age transition stage <br /> are difficult to locate.
